To what extent does the size of a ventricular septal defect correlate with haemodynamic data derived from cardiac catheterisation?

A simplified model based on Gorlin's formula was used to derive an index of the cross sectional area of ventricular septal defects from commonly used cardiac catheterisation data. This index was compared with the area of the defect measured during operation and expressed as a ratio to the cross sectional area of the aorta. The highly significant linear relation (r = 0.94) between this index of the defect area and the size of the defect measured at operation means that the severity of a ventricular septal defect can be assessed from haemodynamic data obtained at cardiac catheterisation.

[Tuberculosis in patients with malignant neoplasms].

Out of 810 patients autopsied at our institute between 1972 and 1985, tuberculosis was demonstrated in 12 (1.5%), all between 40 and 80 years of age, regardless of cancer type. Of 4,272 patients who underwent surgery for malignant neoplasms, 481 were examined for M. tuberculosis by culture in different specimens. All of eight positive cases were with sputum or bronchial mucus specimens, regardless of the site of primary cancer. Tuberculosis complicating malignant thymoma or Hodgkin's disease was found to be generalized and severe. Remarkable growth of elongated bacteria was observed in host macrophages in these immunodeficient patients, suggesting their altered intracellular environment. These findings warrant special attention to tuberculosis in hospital care of cancer patients, who are often immunodeficient.

The comparative hemodynamic effects of propranolol in chronically instrumented puppies and adult dogs.

To determine if there are age-related changes in response to propranolol, we studied the hemodynamic effects of this beta-adrenergic-blocking agent in 19 awake chronically instrumented puppies and compared the response to that of 9 awake chronically instrumented adult dogs. Following intravenous administration of 1 mg/kg propranolol, heart rate, cardiac output, stroke volume, systemic arterial mean blood pressure and systemic vascular resistance were reduced similarly in all age groups of animals, However, puppies were more sensitive to the beta-adrenergic-blocking effect of propranolol than adult dogs when stressed with isoproterenol.

The electrophysiologic effects of procainamide in the immature heart.

The electrophysiologic effects of procainamide in young animals have not been established. To test the effects of this drug on the immature heart, we studied eight nonsedated, chronically instrumented puppies (age range 12-28 days, median 16 days; weight range 0.7-1.1 kg, median 0.8 kg). The electrophysiologic study was performed before and 30 minutes after procainamide was given intravenously (20 mg/kg infusion). At the time of the study, the serum concentrations of procainamide (8.2 +/- 1.7 micrograms/ml to 6.2 +/- 1.6 micrograms/ml; mean +/- SEM) were in the usual adult human therapeutic range (4-10 microgram/ml). Procainamide in puppies caused a significant lengthening of the atrial and ventricular refractory periods, a significant decrease of the sinus node recovery time and the sinus of atrial echo zone, a significant decrease in the heart rate, and a significant increase in the sinoatrial conduction time.

The comparative hemodynamic effects of isoproterenol in chronically instrumented puppies and adult dogs.

Although isoproterenol is the sympathomimetic amine used most often to augment cardiovascular function in infants and children, little is known about the hemodynamic effects of this drug on the immature cardiovascular system. To compare the hemodynamic effect of isoproterenol in the young animal with that in the mature animal, we infused the drug into 11 puppies of 12-38 days of age, 12 puppies of 56-89 days of age and 10 adult beagle dogs. The animals were chronically instrumented and nonsedated during the study. Isoproterenol was infused in doses of 0.05, 0.125, 0.25 and 0.5 microgram/kg/min. Isoproterenol decreased systemic arterial mean blood pressure, increased heart rate and had no effect on renal blood flow in all three groups of animals. While cardiac output was increased and systemic vascular resistance decreased in the adult dogs, neither changed in puppies. Stroke volume increased slightly in adult dogs, but decreased in puppies. This refractoriness to change of cardiac output in immature dogs may be a function of elevated resting cardiac output, developmental changes of contractility or ventricular compliance or developmental changes of vascular smooth muscle responsiveness.

Spectrum of cardiovascular lesions in mucocutaneous lymph node syndrome: analysis of eight cases.

Eight cases of mucocutaneous lymph node syndrome with cardiovascular complications are described. The clinical presentation in all but one patient conformed to the previously described features of this syndrome. Five patients had cardiac enlargement, six had electrocardiographic abnormalities and three had a murmur of mitral insufficiency. Each of the four patients who died had aneurysms of the coronary arteries, usually associated with organizing thrombi. In addition to involvement of the coronary arteries, the renal, mesenteric, vertebral, splenic, hepatic and iliac arteries were affected to individual cases. One patient required triple aortocoronary bypass surgery and one patient underwent excision of a brachial arterial aneurysm. Further long-term follow-up studies of the patients who have survived the acute stage of the syndrome are indicated to establish this spectrum of late complications and sequelae.

A case of rhabdomyoma in infancy.

A case of cardiac rhabdomyoma was presented. She was diagnosed as intraatrial tumor by cardiac catheterization and angiocardiography at 7 months of age. Two months later, she was undergone open heart surgery, which revealed larger rhabdomyoma in the left atrium obstructing the mitral valve.

Correlation between pulmonary vascular resistance and pulmonary arterial diastolic pressure in congenital heart diseases with left to right shunt.

Our studies recently conducted on the correlation between pulmonary vascular resistance and pulmonary arterial diastolic pressure in 467 cases, identified an extremely high correlation between the two. The correlation coefficient between pulmonary arterial diastolic pressure (PAdp) and pulmonary to systemic resistance ratio (Rp/Rs) was 0.799 and that between pulmonary vascular resistance index (RP-index) and PAdp was found to be 0.791. And a still higher correlation was observed between pulmonary vascular resistance and pressure gradient between PAdp and left atrial mean pressure (LAmp), where the correlation coefficient between Rp/Rs and (PAdp-LAmp) was 0.851 and that between Rp-index and (PAdp-LAmp) was 0.813. Those correlations are found to be higher in degree than the correlations between pulmonary arterial mean pressure or pulmonary arterial systolic pressure and pulmonary vascular resistance, leading to the conclusion that the pulmonary arterial diastolic pressure is of extremely high clinical importance as an index for increase in pulmonary vascular resistance.

Spectrum of infective endocarditis during infancy and childhood: 20-year review.

The medical records of the 29 patients under 18 years of age with infective endocarditis (IE) seen over a 20-year period by our department were reviewed to provide an overview of the spectrum of IE during infancy and childhood. None of the 29 patients had had previous cardiovascular surgery. The mean age at onset of IE was 7 years 2 months; 3 patients (10%) were under 2 years of age at onset. One patient during the early years died following 4 months of treatment with various antibiotics. Three patients underwent urgent surgery, and 17 patients with healed IE had elective surgery. All of the 20 patients who were operated on survived. The remaining 8 were followed with medical treatment alone. Positive blood cultures were obtained from 24 (83%) patients, and streptococci were still commonly found (38%). Ventricular septal defect (VSD) accounted for 66% of underlying heart diseases and rheumatic heart diseases for 14%. Vegetations were detected in 12 (67%) of 18 patients observed by echocardiography. Among these 12 patients, 1 with VSD underwent urgent tricuspid valve replacement and VSD closure because of worsening congestive heart failure due to progressive tricuspid regurgitation. Echocardiography identifies patients at high risk with IE, though the presence of a vegetation on echocardiography does not necessarily of itself dictate surgical intervention.

Long-term outcome of coronary abnormalities in patients after Kawasaki disease.

From January 1973 through December 1992, a total of 302 patients (183 males, 119 females) with Kawasaki disease (KD) underwent coronary angiography. The age at onset of KD ranged from 2 months to 12.3 years (median 1.4 years). The age at the first angiographic evaluation ranged from 6 months to 17 years (median 3.5 years). Most of the patients (85%) had suffered from KD before 1985 and thus were treated without benefit of gamma-globulin. Follow-up varied from 6 months to 25.8 years (median 13.6 years). Coronary abnormalities were confirmed in 71 (23.5%) of 302 patients; the left coronary artery (LCA) alone was involved in 36 cases, the right coronary artery (RCA) alone in 10 cases, and both arteries in 25 cases. Serial angiographic evaluation of the 42 cases revealed different attitudes in the progress of coronary abnormalities. All large aneurysms showed a tendency to regress, although some progressed to stenotic lesions. Moderate aneurysms stayed unchanged, regressed, or progressed to stenosis or obstruction. Small aneurysm never became stenotic and frequently regressed to normal internal diameter. Aneurysms of the RCA tended to regress relatively early during the follow-up period, whereas those of the LCA gradually progressed to stenotic lesions. In 7 of 35 patients with RCA lesions, aneurysms progressed to complete obstruction and subsequent recanalization within 0.5 to 7.7 years (median 3.6 years) after the onset of KD. Most of the patients with coronary artery sequelae after KD remain asymptomatic. Serial angiographic observation is indicated for those patients who develop large coronary aneurysms during the acute phase of KD. The standard 12-lead electrocardiogram, chest roentgenogram, and exercise stress test are less sensitive for detecting and evaluating patients with coronary sequelae. For the screening of myocardial ischemia after KD, stress thallium 201 scintigraphy with dipyridamole infusion is recommended.

Congestive heart failure during early infancy in patients with ventricular septal defect relative to early closure.

The hemodynamic features of 44 patients requiring surgical closure of a ventricular septal defect (VSD) during early infancy were evaluated. The VSD was closed within the first 6 months of life in 29 patients (group A) and during the second 6 months in 15 patients (group B). The left-to-right (L-R) shunts were significantly greater in group A than in group B, although in all patients they were more than 50%. In contrast, the left ventricular end-diastolic volume was significantly greater in group B than in group A, whereas the right ventricular end-diastolic volume was greater in group A; the difference was not statistically significant. The step-up in blood oxygen saturation in the lower right atrium, which was significantly more in group A, indicated the L-R shunt across the patent foramen ovale (PFO). The L-R shunt across the VSD during diastole was also detected in all patients angiographically. With early infantile VSD, the larger the L-R shunt the greater was the likelihood of early surgery. Right ventricular volume overload caused by the L-R shunt across a PFO as well as through the VSD during diastole is a useful indicator of a large shunt in infants with VSD.

Postnatal exposure to chlorinated dioxins and related chemicals on lymphocyte subsets in Japanese breast-fed infants.

Effects of postnatal exposure to polychlorinated dibenzo-p-dioxins (PCDDs), polychlorinated dibenzofurans (PCDFs) and coplanar polychlorinated biphenyls (Co-PCBs) on lymphocyte subpopulations were investigated in the peripheral blood of 36 breast-fed Japanese babies. As a result, estimated total intakes of these chemicals in toxic equivalent quantity (TEQ) converted into 2,3,7,8-tetrachlorodibenzo-p-dioxin (2,3,7,8-TCDD) equivalents from the breast milk positively and negatively correlated with the respective percentages of CD4+ and CD8+ lymphocytes in the blood of breast-fed babies. Consequently, the ratios of CD4+ to CD8+ T cells showed significant increasing tendency with the estimated total TEQ intakes. Therefore, our study suggests that exposure to background levels of the highly toxic organochlorine compounds through the breast milk influences the human neonatal immune system.

Effects of postnatal exposure to polychlorinated dibenzo-p-dioxins (PCDDs), polychlorinated dibenzofurans (PCDFs) and coplanar polychlorinated biphenyls (Co-PCBs) on thyroid hormone status were studied in the peripheral blood of 36 breast-fed Japanese infants. Estimated total intakes of these chemicals in toxic equivalent quantity (TEQ) converted into 2,3,7,8-tetrachlorodibenzo-p-dioxin (2,3,7,8-TCDD) from the breast milk significantly and negatively correlated with the levels of triiodothyronine (T3) and thyroxine (T4) in the blood of breast-fed babies. Therefore, exposure to background levels of the highly toxic organochlorine chemicals through the breast milk may cause some effects on thyroid hormone status in Japanese infants.

Congenital rubella syndrome and physical growth: a 17-year, prospective, longitudinal follow-up in the Ryukyu Islands.

The growth of 363 children who were born with congenital rubella syndrome (CRS) after the 1965 rubella epidemic in the Ryukyu Islands in southern Japan was followed prospectively from two through 17 years of age. The growth of children with cataract was obviously retarded (in terms of both height and body weight) when compared with that of other CRS children; the growth of the latter group (height and weight) was almost the same as that of the general healthy population by school age, although all CRS children were small in the preschool years. All children with CRS had smaller head circumferences than control children, with the smallest for the cataract group. The data indicate that children supposedly exposed to maternal clinical rubella before the 60th day of gestation grew poorly at least until the age of 17 years, whereas those supposedly infected after the 60th day began to return to a normal growth pattern from early school age.
