Relationship between regional 18F-fluorodeoxyglucose and 13N ammonia uptake in normal myocardium assessed by positron emission tomography: patterns of mismatch and effects of aging.

Increased regional myocardial 18F fluorodeoxyglucose (18FDG) uptake in relation to 13N ammonia (13NH3) uptake--i.e. glucose metabolism-blood flow 'mismatch'--appears to be a strong indicator of myocardial viability in patients with ischemic heart disease (IHD) and regionally reduced contractile function. Reference values of regional 18FDG and 13NH3 uptake have not been determined in healthy subjects with the target age for the development of IHD. We therefore studied healthy middle-aged and old men using positron emission tomography (PET). Twenty-three healthy men aged 51 to 83 years of age were studied. 18FDG and 13NH3 uptake was quantified in 16 myocardial segments with PET and circumferential profile analysis. The relative 18FDG/13NH3 uptake was considerably heterogeneous with 18FDG uptake consistently higher than 13NH3 uptake in the left lateral ventricular wall. This regional 'mismatch' pattern was observed in all subjects, but was most prominent in middle-aged men. The observed age-dependency was the result of a progressive increased in 13NH3 uptake with advancing age in the left ventricular lateral wall. Age-matched reference values of myocardial 18FDG and 13NH3 uptake appears to be important for the discrimination between physiological and pathological glucose metabolism-blood flow mismatch assessed by PET and circumferential profile analysis.

Teacher interactions in mainstream social studies and science classes.

This investigation was designed to change teacher-student interactions in middle school social studies and science classes. Eighteen of 35 teacher volunteers received a six-session inservice emphasizing teacher effectiveness variables. Results indicated significant differences between experimental and control teachers on a pre-post contrast, as well as on a follow-up (maintenance) contrast. Differential effects on the science and social studies teachers were seen. Similarities and differences related to student type, independent of the intervention, were obtained.

[The value of suprapubic bladder aspiration in the diagnosis of bacteriuria (author's transl)].

Urine obtained simultaneously by midstream voiding and supra-public bladder aspiration from 500 patients were compared by bacteriological and, in some, microscopic examination. In a second series a similar examination was undertaken on 1000 samples each, obtained by midstream voiding, catheterisation or suprapublic bladder aspiration from different patients. There was no significant difference in sediments or Stansfeld count between midstream and aspirated samples. Qualitative false-positive results for protein were obtained in 20% of midstream samples from women. The Kass count for significant bacteriuria was false-positive in 3.4% of midstream compared with bladder-aspiration samples, in 7.6% for "questionable bacteriuria". Positive counts of under 10(5)/ml were obtained in 41.3%, of under 10(4)/ml in 26.7% of aspirated samples. Counts under 10(4)/ml in midstream-voided samples are usually considered the result of contamination, at times falsely so. "Mixed infections" were 7.8 times more common in catheter and 11.4 to 11.8 times more common in midstream than in aspirated samples. The presence of microorganisms in suprapublic bladder aspiration is always abnormal. Chemotherapy is indicated only on results from bladder aspiration.

[Monocyte function in chronic uremia (author's transl)].

Monocyte function studies in vitro have been performed in 21 patients with terminal, dialysis-treated uremia and, in parallel, in 21 healthy normal subjects. In uremia the attachment, spreading activity, and reduction of nitroblue tetrazolium were shown to be significantly enhanced, indicating a metabolic activation of the monocytes. The phagocytosis of IgG-coated red cells, however, was significantly impaired. A modification of the monocyte IgG-receptor in uremic conditions is supposed; its relevance for the immunosuppression in uremic states is discussed.
