Management of the brown recluse spider bite.

A model to study the dermonecrotic effects of a crude fraction of venom from the brown recluse spider (Loxosceles reclusa) was developed using female New Zealand white rabbits. In the rabbit model, surgical excision of the sites of intradermally injected crude venom did not always prevent subsequent necrosis or wound dehiscence despite the use of fluorescein to determine the surgical margin. Heparin and steroids did not prevent the venom-induced necrosis and were much less effective than surgical excision or injecting a specific antivenom. When given within 24 hours, the specific antivenom raised against the brown recluse spider venom blocked or markedly attenuated the toxic effects of the venom in the rabbit model system.

The distribution of haemoglobin concentration in a sample of native high-altitude women.

Two hundred and twenty-two indigenous women living in La Paz, Bolivia at an altitude of 3700 m were screened for the haematological parameters of haemoglobin, transferrin saturation, haematocrit and total iron-binding capacity. The data from 152 nonpregnant healthy women under 45 years of age were utilized in both distributional and maximum likelihood analyses in order to evaluate the prevalence of abnormal haemoglobin levels. The prevalence of anaemia and polycythaemia in the population was low and not considered to be a public health problem. Eleven per cent of the population was found to be iron deficient (less than 15% transferrin saturation).

Atypical squamous cells of undetermined significance in the pediatric population. Implications for management and comparison with the adult population.

OBJECTIVE: To determine the significance of an atypical squamous cells of undetermined significance (ASCUS) diagnosis in patients 18 years or younger. STUDY DESIGN: From June 1994 to June 1995, 630 cervicovaginal smears were performed on patients 18 years or younger (mean age 16.4, range 14-18) at University Hospitals of Cleveland. Of these patients, 69 (10.9%) were diagnosed with ASCUS or ASCUS with a qualifying statement. Follow-up cervicovaginal smears, biopsies and charts were reviewed for a 12-18-month period following the initial diagnosis of ASCUS. RESULTS: The study population was sexually active: 63% were gravid, 21% were multigravid, 68% had a history of sexually transmitted diseases (STDs), and 32% had multiple STDs. Follow-up cervicovaginal smears or biopsies were obtained on 46 patients (32 cervicovaginal smears and 14 biopsies/endocervical curettage cases). Mild to moderate dysplasia was identified in 21.6% of patients (10.8% on cervicovaginal smears and 10.8% on biopsies), and a repeat diagnosis of ASCUS was given in 37%. In patients with a repeat diagnosis of ASCUS, a follow-up cervicovaginal smear or biopsy revealed dysplasia in an additional 13%. The overall rate of dysplasia was 34.7%. CONCLUSION: Regardless of age, the diagnosis of ASCUS in a sexually active patient population has significant implications. Furthermore, we recommend that these patients be managed the same way as high-risk adult patients.

Evaluation of the VIDAS Listeria (LIS) immunoassay for the detection of Listeria in foods using demi-Fraser and Fraser enrichment broths, as modification of AOAC Official Method 999.06 (AOAC Official Method 2004.06).

A multilaboratory study was conducted to compare the VIDAS LIS immunoassay with the standard cultural methods for the detection of Listeria in foods using an enrichment modification of AOAC Official Method 999.06. The modified enrichment protocol was implemented to harmonize the VIDAS LIS assay with the VIDAS LMO2 assay. Five food types--brie cheese, vanilla ice cream, frozen green beans, frozen raw tilapia fish, and cooked roast beef--at 3 inoculation levels, were analyzed by each method. A total of 15 laboratories representing government and industry participated. In this study, 1206 test portions were tested, of which 1170 were used in the statistical analysis. There were 433 positive by the VIDAS LIS assay and 396 positive by the standard culture methods. A Chi-square analysis of each of the 5 food types, at the 3 inoculation levels tested, was performed. The resulting average Chi square analysis, 0.42, indicated that, overall, there are no statistical differences between the VIDAS LIS assay and the standard methods at the 5% level of significance.

[Tumor of the carotid body (paraganglioma)].

Carotid body tumours (paragangliomas) are uncommon but rather an interesting pathology, due to the controversy that still remains regarding their histopathologic behaviour (malignancy?), as well as their diagnosis and management. Because of its nature being composed of chemoreceptor paraganglionic tissue, the carotid body responds to lower partial pressure of oxygen and can cause severe cardiovascular disturbances. Based upon a series of 12 cases, the authors reviewed this pathology in detail, and discussed the methods of diagnosis and treatment, which they considered more appropriate.

[The extra-peritoneal approach in the surgical treatment of atherosclerosis in the aorto-iliac region].

Based upon a series of 27 cases the results obtained with the extraperitoneal approach for the surgical management of atherosclerosis of the aorto-iliac segment, are presented and discussed. The patients, ranging in age from 55 to 87 years (mean 70.9 years) were considered as high risk patients because of associated respiratory, cardiac and renal diseases. The technique of extraperitoneal approach was applied in 8 cases (29.7%) of abdominal aortic aneurysms (AAA), and 19 cases (70.3%) of aorto-iliac occlusive disease (DOAI), with an operative mortality of 3.7%. It has been concluded that the extraperitoneal approach is an attractive alternative to the conventional transabdominal approach, particularly in those patients with significant risk factors, requiring aorto-iliac surgical reconstruction.
