A method for estimating the available energy of incompletely digested carbohydrates in rats.

We developed a method to estimate the available energy from carbohydrates that are unavailable or partially unavailable through direct digestion and absorption. Radiolabeled bacterial and plant cellulose and polydextrose were administered orally to germfree and conventional rats. Label in breath, feces, intestinal contents and carcass were determined with excellent total recovery of the administered label. Comparison of these measurements in germfree and conventional rats was used to calculate the energy directly available by digestion and that available only after fermentation. Although the method overestimates available energy because of the more efficient digestive metabolism of the germfree rat, it provides a reliable, maximum energy value based upon fewer assumptions than previous methods. In conventional rats, 65% of the administered cellulose and 54% of the administered polydextrose were recovered in feces. In conventional rats, appreciable amounts of cellulose were fermented and an available energy value of 3.5 kJ/g was calculated. Calculation of the total availability of polydextrose, taking into account the direct absorption of small amounts of monomers present and the efficiency of fermentation, indicates an available energy value of 4.7 kJ/g.

[Ilizarov techniques of leg lengthening. Problems and results].

Leg lengthening procedures have developed rapidly in the last 20 years, especially distraction epiphyseolysis and corticotomy. We have used both methods since 1977 and carried out 22 distraction epiphyseolyses (20 patients) and 33 corticotomies (28 patients). The average lengthening distance was 8.25 cm and 7.8 cm respectively (minimum 4 cm, maximum 18 cm). The time per cm lengthening (lengthening index) was 45.8 days/cm for epiphyseolysis and 52.2 days/cm for corticotomy. The complication rates were similar in both methods (1.18 and 1.21 pro lengthening procedure). Both techniques are preferably performed by a well trained team of physicians, nurses and physiotherapists for correction of leg length discrepancies.

Functional cloning of a Dictyostelium discoideum cDNA encoding GMP synthetase.

A functional cloning procedure has been used to recover a cDNA coding for the GMP synthetase of Dictyostelium discoideum. The enzyme is encoded by a single gene, which is actively transcribed during growth, but not during development. The open reading frame encodes a protein of 718 amino acids with a predicted molecular mass of 79.6 kDa. The Dictyostelium enzyme has extensive homology with the GMP synthetase of Escherichia coli and regional homology to other glutamine amidotransferases.

Cyclic nucleotide phosphodiesterase of Dictyostelium discoideum and its glycoprotein inhibitor: structure and expression of their genes.

The genes coding for the cyclic nucleotide phosphodiesterase (PD) and the PD inhibitory glycoprotein (PDI) have been cloned and characterized. The PDI gene was isolated as a 1.6 kb genomic fragment, which included the coding sequence containing two small introns and 510 nucleotides of non-translated 5' sequence. From the deduced amino acid sequence we predict a protein with a molecular weight (MW) of 26,000 that, in agreement with previous data, contains 15% cysteine residues. Genomic Southern blot analysis indicates that only one gene encodes the inhibitor. Northern blot analysis shows a single transcript of 0.95 kb. The PDI gene is expressed early in development with little transcript remaining following aggregation. The appearance of PDI mRNA is prevented by the presence of cAMP, but when cAMP is removed the transcript appears within 30 minutes. When cAMP is applied to cells expressing PDI the transcript disappears with a half-life of less than 30 minutes. The PD gene of D. discoideum is transcribed into three mRNAs: a 1.9 kb mRNA specific for growth, a 2.4 kb mRNA specific for aggregation, and a 2.2 kb mRNA specific for late development. The 2.2 kb mRNA is also specific for prestalk cells, and is induced by differentiation-inducing factor. All three mRNAs contain the same coding sequence, and differ only in their 5' non-coding sequences. Each mRNA is transcribed from a different promoter, and by using the chloramphenicol acyltransferase gene as a reporter, we have shown that each promoter displays the same regulation as its cognate mRNA.(ABSTRACT TRUNCATED AT 250 WORDS)

Acceleration of wound healing by local application of fibronectin.

The process of wound healing under the influence of locally applied fibronectin, heparin and thrombin was studied in adult rabbits. Using immunofluorescence microscopy, a higher concentration of fibronectin within the wound area was found on the 1st day after fibronectin treatment. The tensile strength of the wounds was tested on the 6th, 9th and 12th day after operation. On the 9th day the tensile strength of the fibronectin-treated wounds was significantly higher than that of the control wounds on the 12th day after operation. The addition of heparin and thrombin, each in combination with fibronectin, did not show any significant effect on wound healing.

[A simple method for 14CO2 measurement in metabolic experiments].

The determination of the fate of a compound following administration can be performed using the disposition method with 14C-labeled substances, which also allow the measurement of metabolism with CO2 as an expired end product. To substitute the laborious CO2-collection in washing bottles as carbonate a simple instrumentation was built for continuous 14CO2-measurement. The air from the metabolic cage is led in thin layer through a chamber fitted to a foot-monitor, the output of which is online for computation. The instrument is sensitive and calibration is easy.

A developmentally regulated and cAMP-repressible gene of Dictyostelium discoideum: cloning and expression of the gene encoding cyclic nucleotide phosphodiesterase inhibitor.

A 1.6-kb genomic fragment containing the coding region for the inhibitor (PDI) of cyclic nucleotide phosphodiesterase (PD) was isolated and sequenced. The genomic sequence includes 510 nucleotides (nt) of 5'-noncoding sequence and the full coding sequence, which contains two small introns. From the deduced amino acid (aa) sequence we predict a 26-kDa protein that, in agreement with previous data, contains approximately 15% Cys residues. The PDI possesses a hydrophobic leader sequence, five potential glycosylation sites, and three internal repeats. Northern-blot analysis showed a single transcript of 0.95 kb. The gene encoding PDI (pdi) was expressed early in development with little transcript remaining following aggregation. The appearance of pdi transcript was inhibited by cAMP, but when cAMP was removed the transcript appeared within 30 min. When cAMP was applied to cells containing pdi mRNA, the transcript disappeared with a half-life of less than 30 min.

Comparison of distraction epiphyseolysis and partial metaphyseal corticotomy in leg lengthening.

We have used Ilazorov's method of distraction epiphyseolysis for leg lengthening since 1977 and his new method of corticotomy or compactotomy since 1983. The first method was carried out in 22 lower limbs with an average lengthening of 8.25 cm (range 4 to 18 cm); included in this group are 2 patients with achondroplasia in whom both legs were lengthened 12 cm. In the second method, a corticotomy is carried out at the metaphyseal-diaphyseal junction followed by distraction in Ilazorov's apparatus (4 x 0.25 mm a day). We lengthened 30 legs; included are 5 patients who had both legs lengthened from between 8 and 12 cm. In the remainder an average of 7.9 cm of lengthening was achieved (range from 4 to 15 cm). The index for the treatment time per cm achieved (time from operation until full weight-bearing per cm of lengthening) was shorter after distraction epiphyseolysis than after corticotomy. Complications were also a little less after the latter procedure. Distraction epiphyseolysis has to be limited to children from aged 12 years until growth ceases. The advantage with corticotomy is that it can be done at any age from 5 to 30 years. Care is needed in both cases to avoid complications.

Correction of clubfoot relapse using Ilizarov's apparatus in children 8-15 years old.

Treatment of relapsed or neglected clubfeet between the age of 8-15 years is at present unsatisfactory. Correction by triple arthrodesis with wedge osteotomies is not possible before the end of skeletal maturation. This caused us to use Ilizarov's external fixator, which made it possible to correct all components of clubfoot simultaneously by continuous guided distraction. Thirteen feet in 12 children were treated. A plantigrade foot was achieved in all cases. All patients were able to wear ready-made shoes. Complications were some slight and two severe pin track infections, temporary edema, and two relapses successfully treated by means of second procedures.

The cyclic nucleotide phosphodiesterase gene of Dictyostelium discoideum contains three promoters specific for growth, aggregation, and late development.

The cyclic nucleotide phosphodiesterase (phosphodiesterase) plays essential roles throughout the development of Dictyostelium discoideum. It is crucial to cellular aggregation and to postaggregation morphogenesis. The phosphodiesterase gene is transcribed into three mRNAs, containing the same coding sequence connected to different 5' untranslated sequences, that accumulate at different times during the life cycle. A 1.9-kilobase (kb) mRNA is specific for growth, a 2.4-kb mRNA is specific for aggregation, and a 2.2-kb mRNA is specific for late development and is only expressed in prestalk cells. Hybridization of RNA isolated from cells at various stages of development with different upstream regions of the gene indicated separate promoters for each of the three mRNAs. The existence of specific promoters was confirmed by fusing the three putative promoter regions to the chloramphenicol acetyltransferase reporter gene, and the analysis of transformants containing these constructs. The three promoters are scattered within a 4.1-kilobase pair (kbp) region upstream of the initiation codon. The late promoter is proximal to the coding sequence, the growth-specific promoter has an initiation site that is 1.9 kbp upstream of the ATG codon, and the aggregation-specific promoter has an initiation site 3 kbp upstream.

[Metabolism of 14C-labeled urea in conventional and bacteria-free guinea pigs].

This report deals with the utilization of 14C-labeled urea in antibiotically decontaminated, bacteria-free guinea pigs using intragastric resp. intraperitoneal administration route. With conventional animals 52% of the intragastrically administered 14C dose can be demonstrated in the exhaled air, 1.2% in organs and 41% in the urine. 1.8% were found in intestinal and faecal contents. In bacteria-free animals 29% of the activity appeared in the exhaled air, 52% in urine and 2% remain in the intestinal tract, feces and animal body. Intraperitoneal administration of 14C urea revealed 17% resp. 14% in the exhaled air, 72% resp. 66% in urine and 2% resp. 3% in intestinal tract, feces and animal body in conventional resp. bacteria-free guinea pigs.

[The validity of selected nuclear medicine imaging procedures (brain, skeletal, and hepatobiliary diagnosis)].

In a retrospective study we tested several scintigraphic investigations for a total of 2,014 patients. As reference we used the results of biopsy, autopsy or the epicritic final diagnosis before emission, summing all clinical signs as well as the results of additional alternative methods and other imaging methods. Criteria of validity (sensitivity, specificity, diagnostic accuracy) are used in a consistent manner. Brain perfusion scintigraphy detected acute cerebral perfusion disorder with a sensitivity of 0.87, differentiating for reversible and irreversible lesions. Carotid angiography had the same diagnostic validity. Because there is no loss of brain substance, the validity of CT and static brain scintigraphy was lower. Skeletal scintigraphy was highly sensitive for the detection of primary and secondary bone tumours as well as for the detection of inflammations and occult fracture. RHS-scintigraphy of the liver, presently used for tumour and metastasis diagnostics after sonography and CT, had a sensitivity of 0.79. It was more sensitive for detecting inflammatory RHS-liver disease (sensitivity 0.95). Biliary scintigraphy was seen under the surgical aspect only for selected questions. It was sensitive for the detection of biliary cyst disorder and the elucidation of complaints after biliary tract sanitation (sensitivities 0.92 and 0.84) and for the detection of duodenogastric reflux (0.7), but was of limited diagnostic value for the differentiation of hepatocellular and obturation icterus with a sensitivity of 0.5.

[Results of surgical therapy of patellar chondropathy].

150 knees were re-examined after operative therapy because of chondromalacia patellae. We found good and very good results in 52.7%. The results after lateral release were statistically significant better than the results after after the other methods. Because of 82% good results the lateral release operation can be recommended.

[Personal experiences using the Chêneau brace].

Idiopathic scoliosis under 20 degrees were treated by physiotherapeut methods alone. At scoliosis with curves between 20 and 45 degrees a correction by means of Chêneau type orthosis; is additionally used during growing age. The over all results of this combined therapy are good.

[Personal experiences in the conservative therapy of patellar chondropathy].

150 knees were re-examined and rated with a modified Bentley-score to estimate the results of the conservative treatment of chondromalacia patellae. 25 several methods in 74 various combinations were used in the treatment. Only injections of Arteparon, short waves and iontophoresis with histamine were applied as single treatment in some knees. None of these methods, applied as single or in combination with other ones, gave results, which were statistically significant better than those of the other ones. In the conservative treatment of chondromalacia patellae the polypragmasy should be stopped and arthro-muscular rehabilitation should be regarded as the principle task.

Rescue of a Dictyostelium discoideum mutant defective in cyclic nucleotide phosphodiesterase.

One of the developmentally induced gene products that is essential for chemotaxis of Dictyostelium amoebae is a cyclic nucleotide phosphodiesterase. The enzyme can be secreted or exist in a membrane bound form. This enzyme is missing in the mutant HPX235 which, as a consequence, does not aggregate unless exogenous cAMP phosphodiesterase is supplied. We have introduced multiple copies of the cloned phosphodiesterase gene into mutant amoebae and restored aggregation. The formation of anatomically correct fruiting bodies, which does not occur when exogenous enzyme is added, is also restored by transformation with the gene. The construct that we have used gives rise only to secreted phosphodiesterase and therefore the membrane bound form of the enzyme is not absolutely required for normal aggregation and morphogenesis.

[The effectiveness of electromyostimulation following operations of the capsule-ligament system of the knee joint in athletes].

The most important factor in rehabilitation after knee joint surgery is the influencing of the atrophy of the femoral musculature and the recovery of the arthromuscular balance. Electromyostimulation (EMS) should be an additional possibility of treatment. Longitudinal section studies were performed over 6 months in a group of 45 patients after cruciate ligament surgery and for a period of 3 months after meniscus surgery in a group of 61 patients, in each case with different aftertreatment programmes (with EMS vs without EMS). Muscle function control methods were used to assess the condition of the status of function of the musculature (M. vastus medialis). The effect achieved by EMS can be objectivated especially by means of the muscular tone parameters "elastic retraction" and "total compressibility". In the group of sportsmen with EMS treatment (low frequency constant frequency programme with mini-stimulant twice daily 15 minutes) these parameters are about 30% above those of the group without EMS. Timely and on-target use of EMS after knee joint surgery yields a better starting level for rehabilitation and is in our opinion mandatory as a matter of principle especially in high-performance sportsmen.
