[Use of an immunoenzyme microtechnic for quantitative determination of platelet-associated IgG in pediatrics. Acute idiopathic thrombocytopenia purpura and thrombocytopenia following exchange transfusion].

An enzyme linked immunoassay was developed using the microtiter system to measure platelet-associated IgG (PAIgG). The specificity of the assay was demonstrated in 6 children with acute idiopathic thrombocytopenic purpura (ITP): Elevated values of PAIgG (25-800 fg IgG/platelet) corresponded with low platelet counts. Complete remission was accompanied by normalization of PAIgG levels. Since immune destruction of platelets may be encountered in the development of neonatal postexchange thrombocytopenia, platelet counts and PAIgG levels were determined in pre- and postexchange samples from the recipient and in samples from the donor's blood. Eight double-volume exchange transfusions were studied in 7 newborns. Various patterns of interaction between donor and recipient were observed. Five donors proved to be thrombocytopenic, 3 of them had correspondingly high PAIgG levels, indicating immune destruction of donor platelets prior to transfusion. Three newborns had pre-existing immune thrombocytopenia before the exchange. Significant falls in platelet counts with concomitant rises in PAIgG were found following 2 out of 8 exchange transfusions. The quantitation of PAIgG by means of a micro enzyme linked immuno-assay is reliable and helpful in the detection of immune-mediated thrombocytopenia in all pediatric age-groups.

[Encephalitis following measles-mumps vaccination simultaneous to an EBV-infection (author's transl)].

This is a case-report of a 15-month-old child, who received a measles-mumps vaccination in the course of an asymptomatic EBV-infection. Nine days later the infant incurred encephalitis and recovered substantially within three weeks; the EEG also returned to normal in this time. Ataxia still persisted, however, a half-year later. The child produced serum antibodies against all three viruses, whereby the present low EBNA-titer is indicative of a chronic persistent EBV-infection. This is, according to our knowledge, the first description of a EBV-infection during a measles-mumps vaccination.

Improvement of sheep fecundity by treatment with antisera to gonadal steroids.

Sera from sheep immunized against oestrone (Group E1), oestradiol (Group E2), androstenedione (Group A) and testosterone (Group T) were given to ewes singly or as a mixture (Group M) of all 55 types as a single intravenous injection at the time of the start of mating. The number of lambs produced, the numbers of eggs shed and the display of oestrus were recorded. The ovulation rates were 1.8 in Group E1, 2.1 in Group E2, 1.6 in Group A, 1.8 in Group T and 2.1 in Group M compared with 1.3 for the controls (P, variation among groups, less than 0.001) in the first oestrous cycle. The effect persisted in those animals not conceiving to the first mating--1.3 in Group A, 1.8 in Group E1, 1.9 in Group E2 and 2.0 in Group M compared with 1.3 for the controls; all of the ewes in Group T conceived to mating at a single oestrus. The mean number of lambs born alive per ewe treated was 1.1 for Group A, 1.3 for Group E1, 1.3 for Group E2, 1.5 for Group T, 1.5 for Group M and 1.0 for the controls. The increase in the number of lambs born was due to a higher proportion giving birth to twins (P less than 0.01); no ewe gave birth to triplets. High conception rates were recorded for all treatments.

Improving benzodiazepine prescribing in family practice through review and education.

Indications for and dosages of four commonly prescribed benzodiazepines were recorded at a family medicine centre with the aid of a computerized data collection system. Four guidelines were then developed for appropriate prescribing of these drugs: (a) benzodiazepines should be used less frequently with increasing age; (b) short-acting drugs are preferable to long-acting drugs; (c) patients 65 years of age and over should receive half the daily dose prescribed for younger patients; and (d) use of these drugs for more than 1 month should be discouraged. After a year's observation it was evident that none of the guidelines were being followed. The 30 physicians in the practice were then informed of the findings by an educational program. Another 6 months of observation showed a reduction in the prescribing of benzodiazepines to patients 65 years of age and over, a significant shift to the use of short-acting benzodiazepines, and some reduction in the daily dose and duration of administration of diazepam. Thus, such a review of drug prescribing in family practice can be a practical and effective method of improving prescribing patterns.

The prognostic significance of specific IgG antibodies in insect sting allergy.

Specific IgG antibodies against bee venom and bee venom components were studied in the serum of 40 bee-sting patients, 60 bee keepers, and 31 control subjects. The highest titres were observed in successfully hypothesized patients and in bee keepers. Subclass-typing in bee-sting allergic patients showed the highest antibody levels in subclass IgG2, followed by IgG3, IgG 4 and IgG1. During hyposensitization, a rise in specific IgG antibodies in all subclasses and against bee venom and all its components was found. The allergic reaction to bee stings disappeared with the rise of specific IgG antibodies, as demonstrated by a bee-sting challenge. After 6 months of hyposensitization therapy, the specific IgG antibodies remained above, and the specific IgE antibodies fell below, the pretreatment levels.

The treatment of anorexia nervosa with total parenteral nutrition.

Treatment outcome of 11 anorectic patients receiving total parenteral nutrition (TPN) is compared with outcome of anorectic patients receiving standard behaviorally oriented inpatient therapy. TPN resulted in significantly more rapid weight gain. Most patients could be successfully weaned from TPN to enteral feeding with continued weight gain. Complications during TPN were common including transaminase elevations and electrolyte imbalance. There was one death in the series. The role of TPN in the management of anorexia nervosa is discussed.

Immunological studies on bee-keepers: specific IgG and subclass typing IgG against bee venom and bee venom components.

Specific IgE antibodies against bee venom and its components were studied in 23 bee-keepers. The highest IgG serum levels were observed for whole bee venom followed by phospholipase A. The serum levels of specific IgG antibodies against melittin and MCD-peptide were lower, the lowest serum levels being observed for apamin. After a 5 month absence from bee-keeping a fall in the serum levels of IgG antibodies was observed in all the bee-keepers studied. The investigation of the IgG subclass antibodies 1-4 against bee venom and phospholipase A demonstrated the highest serum levels for IgG 4 and IgG 2, the lowest levels were observed for IgG 1. The lowest IgG serum levels were associated with the least effective protection to bee stings. These findings support the concept that specific IgG antibodies prevent the development of allergic symptoms after bee sting.

Organic substrate effects on and heterogeneity of Necturus proximal tubule function.

The effect of organic substrates such as lactate, alanine, glutamine, glutamate, lysine, and butyrate is compared with that of glucose, and the role of tubular heterogeneity is assessed in Necturus proximal tubule. Fluid absorption (Jv) is enhanced 34% by the presence of all substrates together in the tubular lumen. Addition of glucose to the peritubular perfusion fluid does not increase Jv significantly, whereas the other substrates augment Jv by 181%. Transepithelial potential difference (V3) and peritubular membrane potential difference (V1) are unaffected by these organic substrates. Transepithelial chloride to sodium transference number ratios (tCl/tNa) and transepithelial specific resistance (Rm) are higher in the presence of substrates, possibly because of effects on paracellular conductance. Organic solutes could influence fluid absorption by increasing entry of sodium across the luminal cell membrane, by increasing energy supply to the active transport mechanism, or by decreasing paracellular conductance. Early and late straight segments of the proximal tubule do not differ significantly in V3, V1, and transepithelial tCl/tNa. Early convoluted segments have a higher Jv than do late straight proximal segments, but they have a lower transepithelial specific resistance. We have concluded that net fluid absorption and transepithelial ionic conductance correlates with the supply of certain organic solutes and with specific localization along the proximal tubule.

Enzyme-linked immunosorbent assay of allergen-specific IgG antibodies in bee sting allergic patients hyposensitized with pure bee venom.

An ELISA is presented for detection of IgG antibodies to bee venom. By this method, sera of 11 bee sting allergic patients, who were treated with rapid hyposensitization with pure bee venom, were tested. The highest antibody titers were observed after 30 days of treatment, a maximum rise of 7.4 +/- 1.5 log 2-titer steps. Pure bee venom is shown to be more potent immunologically than whole body bee extract. Prediction of the clinical success, measured by tolerance to a bee sting challenge, is not yet possible using venom specific IgG determinations.

[Rota virus infection in the newborn and infant -- epidemiological aspects (author's transl)].

In a total of 406 sera an age-dependent pattern of complement fixing antibodies against Rotavirus (Nebraska calf diarrhea virus) was found: 85% of the adults had antibodies, infants in the second half-year of life had the lowest percentage (42%). Thereupon a Rotavirus survey was carried out in a newborn ward by means of electron microscopy, enzyme-linked immunosorbent assay and complement fixation test during three weeks in January/February 1978. A chronically high infection rate was discovered. Children acquired intestinal infection despite of complement fixing serum antibodies and very few showed a rise in antibody titer.

[Hyposensitisation treatment with pure bee venom (author's transl)].

Hyposensitisation with pure bee venom was undertaken in eleven persons with proven allergy to bee stings. Rapid hyposensitisation led to side effects in seven of them during dosage increase. Serial determination of total IgA, IgG, IgM and alpha1-antitrypsin revealed no changes, while allergen-specific IgG and IgE and total IgE increased maximally after 30 days. All persons were exposed to one bee sting during the treatment: all tolerated it well without the allergic reactions which had previously been noted.
