Monitoring the iodine dose.

The administration of large amounts of iodinated contrast media can result in acute renal failure. Many diagnostic examinations, including pyelography, computed tomography and digital vascular imaging, require the injection of significant amounts of contrast media. Additionally, these examinations may be performed within 24-48 hours of each other, increasing the likelihood of a dose-induced acute renal failure. To avoid this complication, we have developed and implemented a system to monitor and control the patient's dose of iodine. This system effectively alerts individuals to the iodine dose the patient has received within the last 48 hours. The information is being used to compile a study concerning the relationship between iodine dose and the occurrence of acute renal failure.

Occupational injuries in radiology: strategies for cost containment.

Occupational injuries represent a measurable part of the operating costs in the health care environment. Injury rates in hospitals have been documented to be higher than those in industry; one possible cause may be the cost-based reimbursement setting which provides no incentive to reduce occupational injuries. With the advent of prospective payment, the costs associated with occupational injuries must be reduced. This paper will discuss the common hazards in radiology, as well as action that can potentially reduce these costs. Prospective payment has given a new meaning to cost containment, forcing the radiology administrator to seek all possible means to contain costs.

The role of leukocytes in the serum acute-phase reaction accompanying inflammatory reactions.

The effects of whole-body X-irradiation with a dose of (7 Gray) were studied on the time course of systemic responses (fever, leukocytosis, serum coeruloplasmin concentration) in turpentine-induced inflammation and on the acute-phase reaction (coeruloplasmin, haptoglobin and seromucoid) produced by various stressful conditions (turpentine-abscess, immobilization, restraint conditions). Results showed that in the irradiated animals the lack of the leukocytic and febrile response to turpentine administration was accompanied by a normal coeruloplasmin response. Therefore, we have concluded to the involvement of more than one mediator in the development of systemic reactions accompanying inflammation. The leukopenia induced by whole-body irradiation could not prevent the development or reduce the magnitude of the acute-phase response in any of the models analysed. This observation appears to make doubtful the exclusive role of leukocytes in the mediation of the acute-phase reaction.

Clinical trials with MTDQ /6,6'-methylene-bis (2,2,4-trimethyl-1,2-dihydroquinoline)/ an antioxidant with radiation sensitizing effect.

Daily doses of 1320 mg MTDQ have been administered orally to 7 patients for 100 subsequent days. Biological half-life and serum concentration measurements were performed. Function of vital organs, as well as routine laboratory findings revealed that MTDQ did not induce any toxic changes. The effect of MTDQ on the radiosensitivity of hypoxic tumor cells was studied in a combined treatment modality including 12 patients. The action on tumor regression was found encouraging. It has been found remarkable, too, that due to the effect of MTDQ and radiotherapy some of the nonspecific and general symptoms (e. g. pruritus) also disappeared.

Basal cell carcinoma treated with MTDQ and irradiation.

Patients with basal cell carcinoma of the skin were treated with combined MTDQ (6,6'-methylene-bis-(2,2,4-trimethyl-1,2-dihydroquinoline)) administration and irradiation. Significantly better results were obtained with a skin exposure of 2 000 R combined with MTDQ than with the same dose alone. The results were comparable to those obtained with an exposure of 4 000 R. MTDQ administration induced decrease of tissular malonaldehyde concentration and suggested the peroxide-decomposing action of the radiation sensitizer.

[Rapidly growing carcinoma of the penis].

After a brief review of the cancer of the penis the case of a 24-year-old man is described. After five months of the rapidly progressing cancer of the penis no radical operation could be performed in the intact tissue. The importance of biopsy is stressed which has to be repeated, occasionally involving the preparation of series of slides.

Dose contribution to regional lymph nodes from gynecological remote afterloading cobalt-60 sources.

Dose contribution to the regional lymph nodes from high dose rate gynecological intracavitary technique was studied. For computer, calculating radiation dosage, rectangular lymphograms were taken. The average dose to point B and obturator nodes was nearly equal, 29-28% of point A dose. The common iliac nodes received the lowest dose, 14% of point A dose on the average. Due to the wide variation in the position of the uterus (deviation and dislocation), the dose at each of the two corresponding points or nodes (right and left side) was never equal. The effect of the uterus deviation and dislocation on the dose distribution was also studied in computer model display. We managed to describe this effect by an exponential curve. Because of the wide variation in the position of the uterus, only the individual dose calculation to the lymph nodes makes it possible to determine the most accurate amount of additional external irradiation.

Peripheral blood lymphocyte counts and survival in breast cancer.

The lymphocytes in the peripheral blood of patients with breast cancer were studied. Peripheral blood lymphocyte counts were found to be significantly lower in the short-survivors when compared with the long survivors. Lymphocyte count may be a host factor, that influences survival in breast cancer.

[Changes of peripheral leukenic lymphocytes during radiotherapy and clinical symptoms of indirect radiation effect].

Leukaemic lymphocytes are much more sensitive even to local irradiation than non-leukaemic ones. The substantial drop in the number of lymphocytes id partly due to indirect radio-biological effects. It appears likely that the antigens liberated by the damaged tumour cells under the action of radiation augment immunoreaction thus causing additional pathological cells to be destroyed (radioinduced immune processes).

Vitiligo and malignant melanoma.

Six patients with vitiligo and malignant melanoma are reported. The relationship between vitiligo and melanoma seems to be a firm one. This process is supposed to represent an expression of induced autoimmunity.

Longitudinal study of heart disease in a Jamaican rural population. I. Prevalence, with special reference to ECG findings.

A long-term epidemiological study of heart disease in a representative rural community in Jamaica was started in 1962-63 and the first follow-up survey was carried out in 1967-68. This report describes the prevalence of several cardiovascular characteristics at each survey, and their associations with other measurements. The nature of the electrocardiographic abnormalities and their relationship with symptoms of effort pain and prolonged chest pain suggests that much of the disease seen in this population is ultimately ischaemic in origin despite evidence that classical myocardial infarction and severe coronary atheroma are relatively infrequent. Nevertheless both the symptoms and the electrocardiographic abnormalities had features that were not completely typical of occlusive disease of extramural coronary arteries. These findings are discussed in terms of the four conditions-hypertension, conventional coronary heart disease, small artery disease, and cardiomyopathy-that are believed to account for most cases of heart disease in this community, and it is concluded that the overall pattern of disease cannot be explained by any single disorder of overriding importance. The evidence suggests that all may be important contributors.
