Bone biopsies from 37 haemodialysis patients were embedded in a methyl methacrylate:hydroxyethylmethacrylate mixture and consecutive sections taken for: (1) acid phosphatase activity (osteoclasts), (2) pyronin stain for RNA (osteoblasts), and (3) fluorescence for tetracycline. Images of the three sections were traced using a camera lucida and the tracing quantitated with a digitiser. Osteomalacia (OM) was diagnosed by increased osteoid with decreased tetracycline and hyperparathyroidism (HPT) by an increased osteoclast count. OM was present in 8 patients, HPT in 11, OM + HPT in 16, and no disease in 2. Biochemistry could not predict the histology. Histologically, though HPT could be satisfactorily diagnosed, problems still remain in the definition of OM, particularly when combined with HPT.

Gastroesophageal reflux was investigated in 80 patients with duodenal ulcer by analysis of symptomatology and the acid reflux test. Resting gastroesophageal sphincter pressure (GESP) and postvagotomy reduction in basal and pentagastrin stimulated gastric acid secretion were also studied. Reflux symptoms were present in 40% of the patients, and this incidence was significantly reduced two months after vagotomy. In patients studied late after operation reflux symptoms were still less frequent than before operation, but not significant. After vagotomy, no significant changes in the fasting GESP or in gastroesophageal reflux as determined by the pH glass electrode were demonstrated. Thus, the decrease in reflux symptoms may be explained by the significant reduction in gastric acid secretion. Denervation of the cardia and the lower esophagus does not influence GESP or gastroesophageal reflux.
Nine cases of acquired toxoplasmosis, 8 from rural backgrounds, are described, illustrating the wide variety of clinical pictures. Evidence of familial infection was found whenever sought, and was also found in cattle in one case. The availability of an easily performed screening test should make diagnosis of this common disease more frequent.

Over a two-year period, 18 patients with postinfarction ventricular aneurysms were treated surgically. There were no operative or early deaths, and only one (5.5%) late (20 months) death. The indications for surgery included angina, congestive heart failure, and serious arrhythmias. Electrocardiograms and plain chest roentgenograms were unreliable in making the diagnosis. All patients had cardiac catheterization and, at operation, a definite localized aneurysm was demonstrable. In 15 patients (83%), coronary artery bypass grafts were performed at the time the aneurysm was excised. Good results should be expected from surgical treatment of symptomatic ventricular aneurysms. Simultaneous bypass grafting should be performed when indicated.

Although no consistent results can be demonstrated when freshly isolated strains of Neisseria gonorrhoeae are tested for bacteriocin activity on chocolate blood agar, such activity can be demonstrated on GC base medium (Difco), enriched with a defined supplement. At the present time, using six indicator strains, 75% of isolates of N. gonorrhoeae show characteristic patterns of inhibition. These observations are encouraging and suggest that ;gonocin' typing may be possible.
Gonococcal infections can no longer be considered exclusively urogenital but it is difficult in a busy routine laboratory to identify gonococci from non-genital sites, especially if other commensal neisseria are present. Unfortunately, conventional serum agar sugars sometimes give equivocal results. A fermentation medium is described for the identification of gonoccoci. On preliminary testing this medium gives rapid and reliable results and should prove to be much more helpful to the routine laboratory than conventional serum agar sugars. Its potential usefulness, as demonstrated by the results of our first 200 examinations, would seem to justify further study in the routine work of other laboratories.
