The authors mention an observation of lingual necrosis revealing a Horton disease. The set of the usual aspects of the disease was not comprehensive and the inflammatory syndrom was rather weak; nevertheless the diagnosis of a giant cell panarteritis was admitted and a strong cortico-therapy quickly set up which greatly improved the patient. After a brief recall of the main clinical and biological signes of a Horton temporal arteritis, the circumstances, symptomatology evolution and treatment of this particular form of the disease are reviewed through 22 observations found in medical literature.
Within the cranio-spinal cavity we can consider three compartments: blood, cerebro-spinal fluid and nervous parenchyma and thus, three barriers (Blood-Cerebro-Spinal Fluid, Blood-Brain, Cerebro-Spinal Fluid-Brain). The morphological studies of these barriers were performed with exogenous tracers such as horseradish peroxidase, cytochrome C and ferritin or endogenous tracers such as autologous antiperoxidase immunoglobulins. 1. The blood-brain barrier is exogenous and endogenous tracers proof. It is found on the level of the brain capillary endothelium with tight junctions and rare plasmalemmal vesicles. 2. The blood-cerebro-spinal fluid barrier is found on the level of choroid plexus and of leptomeningeal vessel. In the former, the tracer is stopped by the tight junctions (zonula occludens type) of the choroid plexus epithelium. Besides, there is no morphological evidence of transepithelial passage from blood to cerebro-spinal fluid. In the later, the barrier is, almost always, found on the level of the vascular endothelium. 3. The parenchymatous-cerebro-spinal fluid interface cannot be called a barrier because the diffusion of the tracers is not restricted either by the astrocytic marginal layer or by the ependyma. The circumventricular organs other than choroid plexus are morphologically characterized by the free diffusion of tracers in their perivascular connective space. Subcommissural organs capillaries alone behave like those of the brain. The spinal cord capillaries, in opposition to those of the brain, are characterized by a perivascular connective space, for 40 p. 100 of them. The significance of this fact is still unknown.

The effect of parathyroidectomy on left-ventricular function was evaluated in chronic-haemodialysis patients with advanced hyperparathyroidism. Radionuclide angiocardiography (22 patients) and ultrasound echography (8 patients) revealed a significant increase in left-ventricular ejection fraction 1--2 weeks after parathyroidectomy. This improvement was associated with an augmented cardiac index (radionuclide method) and with an increase in mean velocity of circumferential myocardial fibre shortening (echocardiography). Circulating blood volume and erythrocyte space, as well as arterial blood-pressure, had changed little after parathyroidectomy, whereas plasma calcium, phosphate, and immunoreactive parathyroid hormone were significantly lower after surgery. Thus, correction of severe hyperparathyroidism led to a significant improvement in cardiac performance.

The authors discuss the clinical and bacterial characteristics and progressive changes occurring in Yersinia pseudotuberculosis septicemia, basing their observations on one personal case and 30 adult cases reported in the published literature. The roles played by a generally poor condition of the patient and martial overloading are emphasized, and the changes brought about by the introduction of antibiotic therapy evaluated.

Cultures of rat hepatocytes have been used to study the interaction of chrysotile asbestos fibers with epithelial cells. The hepatocytes were incubated for 20 h with chrysotile asbestos (UICC B and a preparation of short fibers: 90% less than 2 micron) at concentrations of 1 or 10 micrograms/ml. Morphologic studies were then performed by means of transmission electron microscopy. Fibers were incorporated into plasma membrane invaginations, cytoplasmic vacuoles and phagolysosomes. These observations demonstrate that rat hepatocytes can engage in phagocytosis of chrysotile fibers. The results are compared to those previously obtained with mesothelial cells and tracheobronchial epithelium, exposed to asbestos in vitro. The advantages of the hepatocyte model to investigate the effects on cells of the association of asbestos fibers with genotoxic agents in relation to carcinogenesis are discussed.
Women may choose to initiate, reinitiate, or discontinue hormone replacement therapy (HRT) at any time beyond the perimenopausal period. HRT, exercise, and nutrition are reviewed in terms of their potential benefits as primary and secondary preventive therapies against coronary heart disease, osteoporosis, breast and genital cancers, and the maintenance of cognitive function among older postmenopausal women. Lifestyle alternatives involving nutrition and exercise that offer many of the same benefits as HRT are discussed. Since both pharmacologic and lifestyle interventions offer significant benefit for primary and secondary prevention of disease and disability, each should be offered to women for consideration as they enter the perimenopausal period. Additionally, each can be recommended for initiation even at much older ages and subsequent to adverse health occurrences, such as the experience of breast or genital cancer or a cardiac event. Each should be sustained over the long term. The decision whether to discontinue these interventions among the most elderly will be influenced by other quality of life considerations.
