True posterior ischemic optic neuropathy associated with herpes zoster ophthalmicus.

Although previous reports of ischemic optic neuropathy resulting from herpes zoster have appeared in the literature, these reports have not been convincing of a true optic neuropathy. The case presented is a true posterior ischemic optic neuropathy due to inflammation of the medial posterior ciliary artery, diagnosed on the basis of a deep, steep-sided altitudinal visual field defect. The herpes zoster infection also resulted in retinitis, damage to the iris sphincter, and corneal scarring. The effects of herpes zoster on the visual system are reviewed.

Primary liposarcoma of the orbit. Problems in the diagnosis and management of five cases.

The clinicopathologic features and the results of diagnostic studies were reviewed in five patients with primary orbital liposarcoma. The major clinical complaints were diplopia and proptosis; visual function was typically well maintained. In three cases, computed tomographic (CT) scans displayed lesions with a central fat density rimmed by a variably radiodense pseudocapsule, leading to the erroneous clinical impression that the lesions were cystic in character. Ultrasonography ruled out a truly cystic tumor by demonstrating internal acoustic interfaces. Magnetic resonance imaging (MRI) in one case showed hyperintense signals in T-1-weighted images, suggesting the presence of fat within the lesional tissue. In the other two cases, CT scans showed reticulated densities accentuating the septa of the orbital fat in the absence of clear-cut circumscription. A potentially useful feature pointing to a neoplasm was bowing of an involved extraocular muscle, which is usually straightened and splinted in either Graves' disease or inflammatory pseudotumor. Histopathologically, two lesions were well-differentiated liposarcomas and three were myxoid liposarcomas, all featuring univacuolar signet ring lipoblasts. Scattered bizarre hyperchromatic mesenchymal cells without prominent lipidization were present in the lesions. None of the patients has experienced regional or distant metastases with follow-ups from onset of 1 to 7 years (mean, 5.2 years); three required orbital exenteration after local recurrence; and two who refused exenteration received only orbital radiotherapy.

Coenuriasis in a spectacled langur (Presbytis obscura): praziquantel treatment and the antibody response to cyst antigens.

A diagnosis of coenuriasis was made in a spectacled langur raised in captivity. Multiple cysts removed from the subcutaneous tissues and later from the abdominal cavity were identified as coenuri, typical of the genus Taenia. Post-surgical treatment of the remaining cysts with praziquantel was assessed with whole body computerized tomography (CT). CT at 6 weeks post-treatment revealed a reduction in size and increased calcification of abdominal cysts as compared to pretreatment CT. Cyst fluid antigens in ELISA assays showed a high titer (1:5, 120) IgG response in the langur serum, while no IgM response could be detected. No decrease in IgG titer was seen 6 weeks after treatment. Immunoblot analyses identified several parasite-specific antigens with apparent molecular weights of greater than 92.5 (3 bands), 88, 41, 37, and 34 kDa.

Intra-articular injection for pain relief in patients awaiting hip replacement.

A double blind randomised trial was carried out to ascertain whether intra-articular injections of saline, bupivacaine or bupivacaine plus triamcinolone would be of value in the relief of hip pain suffered by patients awaiting total hip replacement for osteoarthritis. The majority of patients had good pain relief for 1 month but in general this was not maintained and some patients were much worse after the injection.

Cavernous hemangiomas of the orbit.

Patients with cavernous hemangioma of the orbit seen at the Wills Eye Hospital for the 10-year period, 1972-1981 were reviewed in a retrospective manner. This benign vascular tumor most typically presented as unilateral painless proptosis in a middle-aged woman. The duration of symptoms was between 6 months and 2 years. CT scanning demonstrated a well-circumscribed, homogeneous tumor without bony erosion. Ultrasonography typically showed good sound transmission with moderate internal reflectivity. It is not possible to distinguish clinically a benign cavernous hemangioma from the rare and potentially lethal hemangiopericytoma. Since recurrence and metastasis of hemangiopericytoma may be related to incomplete excision, fastidious dissection and removal of all tumors thought to be cavernous hemangioma before surgery is advised for fear the final pathology may indicate hemangiopericytoma.

Lacrimal gland enlargement in familial sarcoidosis.

Familial sarcoidosis is rare. It is unusual for lacrimal gland enlargement to be the initial manifestation of sarcoidosis. Two sisters are described with lacrimal gland enlargement as the initial manifestation of sarcoidosis. Gallium-67 scan was a useful adjunct to lacrimal biopsy in sarcoidosis. Sarcoid dacryoadenitis responded dramatically to a short term course of low dose prednisone.

On the accuracy of protein synthesis in Drosophila melanogaster.

We have investigated the accuracy of protein synthesis in somatic tissue of young and senescent Drosophila melanogaster by means of two-dimensional electrophoresis. Electrophoretic heterogeneity characteristic of translational error could not be detected at either age, placing the error frequency below about 4 X 10(-4) per codon. As a positive control on the efficacy of the technique, we showed that electrophoretic heterogeneity is detectable in bacterial cells grown in streptomycin, and that the error frequency calculated from these data is in good agreement with independent measurements. Such cells can tolerate an error frequency of about 3 x 10(-3) without cell lethality, or error catastrophe. Since the Drosophila somatic error frequency is at least eight times lower, even in senescent organisms, translational error is unlikely to be a cause of senescence in Drosophila.

The diagnosis and management of epithelial tumors of the lacrimal sac.

Five primary epithelial lesions of the lacrimal sac representing a range of epithelial tumors are presented. The benign lesions initially presented without pain or significant mass but with epiphora and a bloody discharge. The malignant lesions presented insidiously, were large, firm masses and displayed overlying skin telangiectasis rather than the diffuse erythema of dacryocystography revealed a sac partially obstructed by a subtotal space-occupying mass in benign tumors and completely obstructed with malignant lesions.

Cross-linked surface Ig attaches to actin.

In lymphocytes and P3 myeloma cells, cross-linking of surface Ig by the capping and patching phenomenon has been used to demonstrate the induction of a specific association between surface Ig and cellular actin.

Prognostic indicators of alcoholism treatment outcome.

This literature review is a systematic attempt to isolate the personal characteristics of alcoholics generally associated with their prognosis. Forty-five studies which met certain standards for inclusion are summarized here. The description of each study and its corresponding list of characteristics of alcoholics investigated for prognostic value are reported in tables. The tables do not reveal stable predictors across the studies, though some personal characteristics are more generally of predictive value than are others. A number of reasons for the absence of predictor stability are discussed--among them the possibility that certain treatments may work best with certain alcoholic patients.
