Vascular pedicle of the tensor fascia lata myocutaneous flap.

We analyzed 122 lateral femoral circumflex arteries, using arteriograms and cadavers, to establish more precisely the course of the dominant vascular pedicle of the tensor fascia lata myocutaneous flap. Measurements were also done for preoperative planning.

beta-Lactamase proceeds via an acyl-enzyme intermediate. Interaction of the Escherichia coli RTEM enzyme with cefoxitin.

The use of cefoxitin, a poor substrate of the RTEM beta-lactamase, has allowed the kinetic and spectroscopic characterization of a covalent acyl-enzyme intermediate in the enzyme-catalyzed reaction. The rate of reappearance of catalytic activity in an enzyme sample diluted from an incubation with cefoxitin is nearly identical with the observed Kcat. Burst kinetics are observed with this substrate, consistent with the rate-limiting deacylation of the cefoxitinoyl-enzyme. That the reaction intermediate involves a covalent link between enzyme and substrate was shown by gel filtration after rapid denaturation of an enzyme-[14C]cefoxitin reaction at the steady state. Fourier transform infrared measurements indicate that the intermediate is an acyl-enzyme involving a hydroxyl group of the beta-lactamase. The evident relationship between the acylation-deacylation sequence of the beta-lactamases and the acylation reaction suffered by the D-Ala-D-Ala-carboxypeptidases is discussed.

Beta-lactamase inactivation by mechanism-based reagents.

The mechanistic pathway followed by the E. coli RTEM beta-lactamase has been studied with a view to clarifying the mode of action of a number of recently discovered inactivators of the enzyme. There is clear evidence that the beta-lactamase-catalysed hydrolysis of the 7-alpha-methoxycephem, cefoxitin, proceeds via an acyl-enzyme intermediate. An analysis of the inactivation reactions of all the known beta-lactam derivatives that result in irreversible loss of enzyme activity permits the identification of three structural features required for a beta-lactamase inactivator. The application of these principles suggests a new group of mechanism-based inactivators of the enzyme: the sulphones of N-acyl derivatives of 6-beta-aminopenicillanic acid that are themselves poor substrates for the enzyme. These sulphones are powerful inactivators of the beta-lactamase.

Evaluation of the effectiveness of postoperative radiation therapy for the control of local disease.

Although we agree that postoperative irradiation does improve the local and regional control, our findings indicate that the rate of control, especially local control, is somewhat less than 90 percent. Our data reveal a recurrence rate of 32.7 percent in advanced, stage III and IV, squamous cell carcinomas in the head and neck treated by surgery and postoperative irradiation; this rate may be even greater with longer follow-up. We recommend that surgeons avoid "close" surgical margins (less than 5 mm) when resecting primary cancers and not rely too heavily on radiation therapy to control residual subclinical disease. It appears that continued study is needed to establish the true efficacy of irradiation in the control of subclinical disease, and that local recurrence must be differentiated from regional recurrence when reporting these results.

Kaposi sarcoma of the base of the tongue.

A rare case of solitary Kaposi sarcoma of the base of the tongue is presented. After excision-biopsy the treatment consisted of radio-therapy. A follow-up of two years shows no recurrence.

Calibration of some Wright peak flow meters.

The performance of Wright peak flow meters is described using an absolute and precise calibration method. This showed errors as great as 25% with the peak flow meter and 40% with the mini peak flow meter. At large flow rates errors were less. The expected variation between individual peak flow meters was about 12%. The expected variation between individual mini peak flow meters was between 16% and 22%, depending on flow rate.

Uniqueness of sperm mtDNA as compared to somatic mtDNA in ram.

Sperm mtDNA exhibits unique physicochemical properties compared with mtDNA from somatic tissues in the ram. The buoyant density of sperm mtDNA was 1,6983 g/cm3 while the brain, heart and liver mtDNAs was about 1.7005 g/cm3. The Tm of the liver and sperm mtDNA was 71.0 degrees C and 69.5 degrees C, respectively. The G + C content of sperm mtDNA was 3.0 moles % lower than the liver mtDNA. The contour length of the circular sperm mtDNA was 5.01 micrometers compared with the liver mtDNA with contour length of 5.33 micrometers.

DNase II in bull and ram sperm tail and mitochondria.

Extracts of bull and ram sperm tails were prepared by DTT and CTAB treatment. Such extracts contained deoxyribonuclease II, degrading only native double-stranded DNA at acid pH (3.9--4.5). At least three distinguishable DNase II activities were found in these extracts as judged by their sensitivity to thiol compounds and various anions and cations. The deoxyribonuclease II activity is apparently located in or in association with the mitochondria.

Replicative intermediates of ram sperm mitochondrial DNA.

Replication of mtDNA from ejaculated ram spermatozoa was demonstrated by electron microscopy. Twelve percent of mtDNA circular molecules isolated either from ram sperm cells or from rat liver mitochondria were double-forked. These replicative intermediates appeared in four distinct steps of replication: 3.9%, 21.0%, 27.8%, and 44% of the circular contour length. The biological significance of sperm mitochondrial DNA replication is discussed.

Calibration of some instruments for measuring peak expiratory flow.

The performance of instruments for measuring peak expiratory flow have been investigated. This evaluation was carried out with two designs of spirometer, two types of pneumotachograph and three versions of the Wright peak flow meter. The appropriate flow profiles were generated by means of a special type of flow calibrator. The errors produced by the instruments varied from zero to about 26%.

The burned female breast.

Historically, treatment of the burned female breast has been unsatisfactory. By using the latissimus dorsi myocutaneous flap, many patients can be reconstructed in a single operation. A more aggressive approach in adolescent patients whose breasts are developing may result in more normal breasts. In such patients, nipple reconstruction is delayed until a later time.

Mucormycosis in transplant patients.

Cutaneous mucormycosis in a renal transplant recipient resulted in gangrenous cellulitis of the neck at an internal jugular cannula site. This nosocomial process was due to a histologically confirmed infection, and cultures revealed Mucor sp. The patient died despite surgical debridement and amphotericin B therapy. Cutaneous mucormycosis is a relatively rare entity with only 12 cases previously reported. In renal transplant patients only nine cases of renal transplant patients only nine cases of mucormycosis have been reported and only one was cutaneous. Also, this case reported is the second originating at an intravenous cannula site; the first recorded patient was diabetic.
