Immunochemical properties of some monoclonal IgE antibodies to 4-hydroxy-3-nitrophenylacetyl (NP).

Several hybridoma cell lines secreting NP-specific, murine IgE antibodies were generated by fusion of P3-X20 (gamma, kappa) tumour cells with spleen cells from (BALB/c X C57B1/6)F1 (CB6F1) mice previously immunized with NP-ovalbumin. Four subclones (designated NP-epsilon-3.57, NP-epsilon-15.88, NP-epsilon-91.58 and NP-epsilon-95.31) were propagated in vivo and milligram quantities of the corresponding IgE antibodies were purified from ascitic fluid by gel filtration, ion exchange chromatography and affinity chromatography. Immunological analyses and sodium dodecyl sulphate polyacrylamide gel electrophoresis (SDS-PAGE) indicated that NP-epsilon-15.88, NP-epsilon-91.58 and NP-epsilon-95.31 all possessed lambda 1 (or possibly lambda 3) light chains; and that NP-epsilon-3.57 possessed lambda 2 light chains; NP-epsilon-95.31 also expressed the P3-X20 derived, MOPC-21 kappa light chain. Radioallergosorbent test (RAST) titration curves, generated from the interaction of the four monoclonal IgE antibodies with NP-BSA attached to paper discs (NP-BSA-P) were found to be non-overlapping. Measurements of the relative amounts of NP-epsilon-aminocaproic acid (NP-CAP) and 4-hydro-3-iodo-5-nitrophenylacetyl-epsilon-aminocaproic acid (NIP-CAP) that were required to inhibit by 50% the binding of the 4 IgE antibodies to NP-BSA-P indicated that these antibodies were all heteroclitic, since their affinity for NIP appeared to be higher than their affinity for NP. These results, in conjunction with other findings reported in the literature, suggested that the V regions of NP-specific IgE antibodies are similar to the V regions of NP-specific IgM and IgG antibodies, produced by the same mouse strains. Finally, in vitro histamine release measurements demonstrated that two of these monoclonal IgE antibodies could mediate antigen induced histamine release from passively sensitized rat peritoneal mast cells.

[Intracerebellar hematomas; diagnosis and therapy].

This article deals with the diagnosis, therapy and prognosis of intracerebellar haematomas. The characteristic symptoms and the value of computer tomography for an early diagnosis and the surgical therapy are discussed. Prognosis and complications are analysed in comparison with other works. Guiding rules for a rapid diagnosis and the choice of the respective therapy are given and an early surgical therapy is recommended.

[Acute promyelocytic leukemia: retrospective study of 119 patients treated with daunorubicin].

A retrospective study of 119 patients with acute promyelocytic leukemia (APL) treated with similar DNR containing regimens in reported. Antecedent of radiation exposure or cancer was found in 10 patients. At presentation hyperleucocytosis was rare (13/119 greater than 30 000/microliter); variant form was identified in 5 cases. Organomegaly was uncommon and severe metabolic abnormality was never noted at presentation. DIC was observed in 75% of pts; t (15;17) was confirmed in 25/30 pts. Complete remission (CR) rates have increased from 43% to 76% on account of improvement of supportive therapy with adequate DIC management. Addition of ARA C did not improve CR rates (72%). Surprisingly duration of CR seems related to maintenance therapy as 11/26 pts receiving 6 MP-MTX maintenance regimen were long-term survivors as compared to 1/34 comparable pts receiving cyclic monthly courses of chemotherapy.

[Spontaneous healing of a carotid-cavernous fistula].

In a posttraumatic carotid-sinus cavernous fistula, spontaneous healing occurred which was angiographically demonstrated. After this closure had occurred, an aneunysmal enlargement of the internal carotid was proved to exist and confirmed even two weeks later together with a concomitant spasm. The author is of the opinion that the cause of a carotid-sinus cavernous fistula is in most cases a ruptured aneurysms. By partial thrombosing of the aneurysm, a closure of the fistula occasionally takes place and thus the above mentioned spontaneous healing is reached.

[Stimulation of limited sperm motility and mucociliary clearance of the lung with theophyllin-ethylendiamin].

The influence of oral aminophylline (Phyllotemp ret., 2 X 225 mg) on the mucociliary clearance of the lung and on sperm motility was examined in 13 patients with asthenospermia vs. placebo because the anatomic structure of the cilia of the sperm cells and of the respiratory epithelium cells is similar. The mucociliary clearance was measured by means of radioactive tagged (99 mTc) monodisperse particles (erythrocytes from the patients). To attain a reproducible deposition pattern, the particles were inhaled during a slow inspiratory vital capacity maneuvre. The i.v. application of aminophylline (6 mg/kg body weight) to four normal subjects increased the mucociliary clearance in three of the four volunteers vs. placebo. The elimination rate of the particles increased under treatment with aminophylline from 29% +/- 12% to 46% +/- 19%, but only in seven of the 13 patients with asthenospermia. Sperm motility increased in all of the patients from 33% +/- 12% to 54% +/- 14%. No correlation was found between the increase in the mucociliary clearance and the sperm motility. The reasons for this difference will be discussed. Theophylline can be recommended for the treatment of male infertility in cases of asthenospermia.

Improved quality of life with an implantable pump for liver perfusion.

In spite of the current discussion on whether liver perfusion is effective (Grage et al. 1979; Ansfield et al. 1975), the implantable perfusion pump offers the possibility of combining the still questionable therapeutic success with a high quality of life during the time remaining to such patients.

The significance of lectin receptors for the evaluation of hormone dependence in breast cancer.

Benign and malignant breast diseases were histochemically investigated with regard to their secretory function. Thereby, the labelled lectin from peanut (peanut agglutinin = PNA) which possesses a high affinity for D-galactosyl-(1-3)-N-acetyl-D-galactosamine, showed a binding pattern partly indicating disturbed secretory cell mechanisms. Further studies with a labelled antibody directed against a milk fat globule membrane glycoprotein, which was isolated by PNA affinity chromatography from human milk, confirmed the PNA receptor as a marker of a milk protein in breast carcinomas. Additionally, two other monoclonal antibodies which were raised against other components of the milk fat globule membrane were used for a more detailed characterization of the functional situation of mammary carcinomas. These immuno- and lectin-histochemical investigations revealed that tumors with a high secretory activity responded in about 80% to endocrine treatment, whereas breast carcinomas that exhibited no secretory function failed to respond to endocrine therapy. Thereby, the hormone dependence of tumor tissue showed an inverse correlation with the proliferative activity, as determined by the mitosis index.

Regional, cellular, and subcellular distribution of [3H]dexamethasone in rat brain edema.

The regional, cellular, and subcellular distribution of [3H]dexamethasone in brain edema of rats was studied. Edema was induced either by occlusion of the right carotid artery or by a cold lesion of the right temporal lobe. [3H]dexamethasone (0.3 mCi) was injected intravenously. After 30 minutes (unless otherwise stated) the brains and other desired organs were removed. In the control animals, 51% of the total [3H]dexamethasone activity was found in the cerebral hemispheres (27% in the right, 24% in the left), 24% in the cerebellum, and 24% in the brainstem. Time-course studies revealed a rapid decline of [3H]dexamethasone content in all regions of the brain. After 48 hours of ligation of the right carotid artery, 80% of the [3H]dexamethasone could be found in the cerebral hemispheres (48% in the right, 32% in the left), 10% in the cerebellum, and 9% in the brainstem. In the series in which cold lesions were induced, 74% of the [3H]dexamethasone was recovered in the cerebral hemispheres (40% in the right (lesion), 34% in the left (control]. Before the trauma, 75% of the dexamethasone was found in astrocytes and 25% in neurons; after the trauma, 48% was bound to astrocytes and 42% to neurons. At the subcellular level, accumulation took place in the microsomal, lysosomal, and cytoplasmic fractions of the damaged cells. These data demonstrate an increased uptake of dexamethasone into ischemic damaged brain tissue and into neurons, microsomes, and lysosomes.

An unusual accumulation of repetitive sequences in the rat genome.

We have found in the rat genomic DNA a fragment 1300 bp long, containing an unusual concentration of members of different repetitive families. Three different repeats were noticed. An Alu-like repeat, homologous to the mouse B1 sequence, was followed by a fragment containing alternating purine-pyrimidine bases as in Z-DNA. Finally, a third repeat was identified, containing 38 TAGA tetranucleotides as described for reptile DNA.

Non-traumatic intracerebral haemorrhage. Prognostic implications of neurological and computer-tomographical findings in 100 consecutive patients.

One hundred cases of non-traumatic intracerebral haemorrhage were studied retrospectively. The mortality was 41%, although however, the cause of death in 10% of the cases was not directly related to the intracerebral haemorrhage. Clinical neurological and CT findings available in the first few hours after the admission of the patients were selected for the statistical analysis, using a linear stepwise discriminant analysis including jack-knife classification and a simple independence model for the variables. The results indicate that the outcome can be predicted with relatively great accuracy in individual patients, and that the level of consciousness on admission and the age of the patient are the most important factors for prognosis. The addition of CT-based variables did not achieve any significant improvement in the accuracy of the prediction of the outcome.

Pediatric practice in physical therapy. A survey.

This study was undertaken to define the role and functions of the physical therapist in pediatrics, specifically at the advanced clinical competence level. Questionnaires mailed to all members of the Section on Pediatrics of the American Physical Therapy Association were used to collect the data reported here. Of 109 consultative, evaluation, treatment planning, and implementation tasks, 37 were not considered important to the practice of physical therapy in pediatrics. Of the 72 tasks identified as important, 46 were at entry level and 10 at the advanced level. Sixteen tasks were not clearly defined. This initial survey provides useful data to begin to interpret the physical therapist's role in pediatrics. Validation studies are needed to verify that the identified advanced level responsibilities represent skills necessary for practice in pediatrics at the advanced competence level.
