The technique of computed tomographic sialography (CTS) has been demonstrated to be valuable in the diagnosis of masses of the major salivary glands. Forty-one CT sialograms were performed in 35 patients using acinar glandular filling with oily contrast material. Twenty-two mass lesions and seven cases of inflammatory disease were identified. There were two instances of mild parotitis following the procedure. CTS performed with this technique was found to be a safe, accurate method for evaluating salivary gland masses.
A report of a fatal anaphylactic reaction to oral penicillin is presented and the unusual set of circumstances leading to its fatal outcome is reviewed. The initial signs and symptoms are analyzed and the use of the proper and immediate treatment is advised.
The ability of protamine sulfate to effect the quantitative precipitation of 2,3,7,8-[3H]tetrachlorodibenzo-p-dioxin (TCDD):Ah receptor complexes from rat liver cytosol has been developed into a new assay for the identification, quantitation, and characterization of the Ah receptor. The method is reliable, uncomplicated, and rapid, and can be applied to large numbers of samples. The major advantage of the assay is that protamine sulfate appears to selectively precipitate the Ah receptor protein and does not precipitate a number of other proteins that bind [3H]TCDD nonspecifically.

A recent clinical report noted that vitamin E administration was followed by a dramatic elevation of high-density lipoprotein cholesterol (HDL-C). Since HDL-C is inversely associated with coronary heart disease, we conducted a randomized, double-blind, placebo-controlled clinical trial of vitamin E administration (800 IU/day) for 16 weeks. The subjects were 30 adults aged 30-60 years, with 15 participants in each group. We measured fasting HDL-C, total cholesterol, and triglycerides at baseline and at 8 and 16 weeks. Vitamin E had no effect on HDL-C; the mean changes at 8 and 16 weeks in the placebo group were -0.3 mg/dL and -2.6 mg/dL, and in the vitamin E group -0.4 and -0.9 mg/dL. Aside from a marginal decrease in calculated low-density lipoprotein cholesterol, at 16 weeks there were no significant differences for any of the lipids. These data, in conjunction with other information in the literature, indicate that vitamin E does not alter plasma lipids in normal adults.
The oral and maxillofacial surgeon faces many situations in which maintenance of the patient's airway can be a problem. Facial trauma, severe infection, and trismus all can compromise the airway and pose a dangerous situation. The use of high-frequency jet ventilation (HFJV) via transtracheal puncture offers excellent airway protection with minimal patients discomfort prior to induction of anesthesia. The use of a large, cuffed endotracheal tube for blind awake nasal intubation, or in conjunction with tracheostomy, can be eliminated in many situations. HFJV offers minimal trauma to nasal passages and larynx while preventing aspiration. During emergence, discomfort is reduced to a minimum, and the patient can communicate verbally. These same advantages apply to patients who receive outpatient oral surgery. Therefore, in the choice of an anesthetic technique, HFJV should be a definite consideration.
