[Percutaneous endoluminal angioplasty of the supra-aortic arterial trunks. Immediate and remote results].

Twelve tight stenoses (9 of the subclavian artery, 1 of the axillary artery and 2 of the brachio-cephalic artery) were dilated with immediate satisfactory results in all cases. Only one complication (regressive lesion of the median nerve) was recorded. Eight patients were followed-up for a mean period of 13 months: all were symptom-free, and digital angiography by the intravenous route showed a stable image of patency. From a review of published dilatations of the subclavian artery it appeared that the method is 95% effective, that its mortality is nil and that no complication, notably embolism, has been reported. In view of the risks involved in surgical procedures and of the advantages of percutaneous transluminal angioplasty, at least in theory, this technique seems to be a suitable alternative to surgery in stenoses of the subclavian arteries.

[Primary sclerosing cholangitis].

The Primary Sclerosing Cholangitis, is a rare disease and the aetiology is unknown. This disease is characterized by an intense inflammation and usually fibrosis of the intra and extrahepatic portion of the biliary tree.

Spinal stenosis caused by a Harrington hook in neuromuscular disease. A case report.

In a 22-year-old woman with an unspecific congenital myopathy spinal stenosis developed 11 years after a T4-L5 spinal fusion. A slowly progressive lumbosacral lordosis developed, and the Harrington hook at L5 tilted into the canal, eroded the dura, and pressed on the nerve roots. Removal of the hook and fusion of L5-S1 relieved the symptoms.

[Netherton syndrome].

The case of a 8 years old girl, who since the delivery had suffered from a generalized, erythematous and scaling eruption of the skin is reported. She also had a sparse, short and brittle hair. Abnormalities such as trichorrhexis nodosa and pilo torti were observed. The finding of a generalized increase of the urinary aminoacids, excretion especially for Glycine and Serine, is added to this features.

[Hyperoctanoatemia and the hepatic encephalopathy of cirrhosis. 150 dosages in 61 patients (author's transl)].

A new and sensitive method for determination of octanoate in serum by gas-liquid chromatography is described. It was validated by mass spectrometry. Octanoate concentrations were determined in the serum of 61 fasting cirrhotic patients of which 47 also had hepatic encephalopathy. Concentrations in arterial and venous blood were higher in cirrhotic patients with encephalopathy than in those without and higher in the latter than in controls. Arterial concentrations were higher than venous concentrations and octanoate and ammonia varied independently. A predominant endogenous origin is likely. Data obtained from studies using palmitic acid labeled at different loci suggest that recovered serum octanoate was formed mostly by incomplete oxidation of long chain fatty acids. Sodium octanoate infusion to rhesus monkeys studied polygraphically induces a temporary coma.

Effects of triiodothyronine on the anemia of chronic renal failure.

The present study was done in order to determine whether treatment with triiodothyronine (T3) would improve the anemia of chronic renal insufficiency. Four patients with stable renal failure and anemia were treated with 75 microgram/day of T3 for a period of four weeks. The patients were not in chronic dialysis, had normochromic normocytic anemia and hypocellular bone marrow, and showed no evidence of external blood loss during the study. They had received no iron, folic acid, or androgenic steroids for at least six months prior to the study. The administered dose of T3 was adequate to depress the serum levels of thyroid-stimulating hormone (TSH); however, there was no detectable improvement in the levels of hemoglobin, hematocrit, bone marrow cellularity, serum and erythrocyte folate determinations, serum iron (Fe), 59Fe half-life, plasma iron turnover rate, percentage Fe incorporated into red blood cells, or in the ratio of surface radioactive counting over bone marrow/liver and spleen. It is concluded that the experimental observations concerning direct bone marrow stimulation by T3 in anephric rats are probably not clinically applicable for the treatment of the anemia associated with uremia.

Percutaneous fixation of supracondylar fractures of the humerus in children.

Of 1,118 supracondylar fractures of the humerus in children treated over a six-year period, 189 are described with a follow-up of more than two years. These patients were treated with closed reduction and percutaneous fixation of the fragments with two wires. Eighty-five per cent of the final results were satisfactory.
