[An arterial catheter for multiple use in pediatrics (author's transl)].

The authors describe a pediatric catheter for use mainly in highly selective abdominal angiography (splenic, gastrica sinistra, and intestinal arteries). The catheter is made of polyurethane, is 65 cm long and is curved. It has a bore of 5 F and is introduced with a wo 32 guide, allowing a flow-rate of 10 ml/sec. The catheter point is polished by chemical means allowing penetration of the arterial wall without preliminary dilatation. This avoids vascular spasm and damage to the vascular wall at the site of puncture.

Angiography↗

[Anaphylactoid complications of iodide contrast media in arteriography of the lower extremities during general anesthesia].

The authors studied the manifestations of intolerance to certain iodide contrast products injected by the arterial route, under general anaesthesia. The study covers 385 arteriographies of the lower limbs which were divided into two groups, depending on whether a cortisone premedication had been administrated or not. In the premedicated group, no incidents were noted. In the other group, 13 p. 100 of subjects had cutaneous manifestations with Radioselectan. General anaesthesia did not avoid the accidents which depend on the nature and the quantity of the product injected. Corticosteroids prevent these reactions.

Anaphylaxis↗

[A rare tumour of the right hypochondrium in the child (author's transl)].

Pancreatic pathology in the child is rare and usually limited to necrotic or traumatic pseudo-cysts. The authors report a case of multicystic dysplasis localised in the pancreas in a one-year old infant. The preoperative diagnosis of a traumatic cyst was made in error as the result of the discovery of a fracture of the skull. At operation, the "tumour" was seen to be a multicystic mass within the duodenal loop but pediculated under the inferior surface of the pancreas. Histological examination confirmed its dysplasic nature. The presence, as a result of vascular compression, together with the post-operative resolution of hypertension lend further originality to this case.

Diagnosis, Differential↗

[Regional chemotherapy of stomach cancers].

Fifty patients with a stomach cancer received regional chemotherapy by infusion. Two kinds of cancers were treated with this technique: cancers inaccessible with usual surgical treatment and some limited cancers. In most cases, the catheter was introduced by transcutaneous puncture of axillary artery. According to the site of the cancer, the extremity of the catheter was introduced into the left gastric artery, hepatic artery or coeliac trunk. Efficiency of the infusion was checked by arteriography and by gastric fibroscopic examination with intra-arterial injection of evans blue. The duration of the treatment was two weeks or three weeks. The chemotherapy was made on a sequential method, taking account of the tumoral cellular cycle. The results show an objective response in 80 per cent of the cases, 39 per cent of the non operable cancers were later accessible to surgery. This study shows that chemotherapy should be used in the treatment of any stomach's cancers before surgical treatment.

Adult↗

[Trial of a new flexible cassette in general radiology (author's transl)].

The flexible cassette is a detector of undisputable value, particularly in the radiology of patients who have been subjected to trauma, patients who cannot be transported and above all in "locked" joints. Thus the authors think that it is indispensable for all radiologists to have a set of flexible cassettes at their disposal.
