Selection of donors and recipients for living donor liver transplantation.

1. Living donor liver transplantation (LDLT) is increasingly used for adults with end-stage liver disease. 2. Standards for acceptable rates of donor morbidity, and even mortality, must be evaluated in the context of recipient risk of dying while on the waiting list and outcome after transplantation. 3. Use of our current criteria in Colorado for selection of donors and recipients indicated that 15% of recipients could undergo LDLT.

Idiopathic splenic vein stenosis: a cause of gastric variceal hemorrhage.

We report the case of a patient with isolated gastric variceal bleeding. Obesity precluded the use of noninvasive means for assessing splenic vein patency. Splenic vein stenosis was diagnosed by transhepatic portal and splenic venography with pressure measurements. A cause for the stenosis could not be found. Splenectomy was used as a curative measure.

Incarceration of umbilical hernia following transjugular intrahepatic portosystemic shunt for the treatment of ascites.

Transjugular intrahepatic portosystemic shunt (TIPS) is an effective therapy for patients with medically refractory ascites. Many patients with refractory ascites have umbilical herniation. Incarceration of umbilical hernia has been reported following diuresis, paracentesis, and peritoneovenous shunting. We report 2 cases of umbilical hernia incarceration following resolution of ascites after TIPS.

Rapid progression to high-grade dysplasia in Barrett's esophagus after liver transplantation.

There is an increased incidence of malignancies in transplant recipients. Accelerated progression from a premalignant lesion to carcinoma has been reported in transplant recipients with skin cancer and colon cancer. Whereas Barrett's esophagus is a common premalignant condition in the normal population, rapid progression to severe dysplasia or carcinoma has not been widely reported in transplant recipients. We report on a liver transplant recipient who developed rapid progression from Barrett's esophagus without dysplasia to high-grade dysplasia within 9 months after transplantation.

Transjugular intrahepatic portosystemic shunt (TIPS) in patients with refractory ascites: effect on body weight and Child-Pugh score.

OBJECTIVE: This study suggests that patients with medically refractory ascites treated with transjugular intrahepatic portosystemic shunt (TIPS) may have improved in overall clinical status. METHODS: We performed a retrospective study of 35 patients with medically refractory ascites treated with TIPS. Body weight, ascites, and Child-Pugh score were assessed at baseline, at 2 months, and after a mean 8.8-month follow-up interval. RESULTS: After TIPS, there was significant improvement in Child-Pugh score from 9.7+/-1.5 to 8.2+/-2.3. Ascites completely resolved or improved in 23 of 24 patients (96%) who had long term follow-up. Two months after TIPS, there was a significant decrease in weight of 6.1 kg corresponding to a loss of ascites. Between 2 and 8.8 months, there was a significant mean weight gain of 5.5 kg. CONCLUSION: This study suggests that patients treated with medically refractory ascites with TIPS may have improvement in overall clinical status, as measured by increase in lean body mass and improvement in Child-Pugh score.

Drugs that interact with immunosuppressive agents.

Clinically significant drug interactions are common in the liver transplant recipient. In addition to the immunosuppressive agents, these patients simultaneously receive many other medications. This review focuses on the clinically significant drug interactions of commonly prescribed immunosuppressives in the liver transplant recipient. Most reported drug interactions involve cyclosporin A.

Current and prospective therapies for hepatic fibrosis.

The prospects for new, effective treatments for hepatic fibrosis are real. Currently there are several drugs being evaluated in clinical trials, including polyunsaturated lecithin and prostaglandins. In addition, the authors' (J.F.T. and D.A.B.) knowledge of the pathophysiology of hepatic fibrosis has increased dramatically during the past few years. These advances are being incorporated into novel therapies for hepatic fibrogenesis such as TGF-beta antagonists. Hopefully, definitive treatments for hepatic fibrosis will become available in the near future.

Injection sclerotherapy as a cause of Klebsiella pneumoniae peritonitis.

We report a case of Klebsiella pneumoniae peritonitis after endoscopic sclerotherapy and discuss its pathogenesis and risk factors. We also review previous cases in the literature and make recommendations for prophylactic therapy. Endoscopists should be aware of peritonitis as a possible complication of endoscopic sclerotherapy.

EEG mean frequencies are sensitive indices of phenylalanine effects on normal brain.

We previously reported that changes in plasma phenylalanine (PHE) concentrations of 1000 microM or more adversely affected cognitive function and reduced mean frequency of the EEG power spectrum. In the present study, we characterized EEG effects of changes in plasma PHE from physiological to supraphysiological concentrations. Subjects were mentally normal children and adult volunteers with 3 different genotypes for phenylalanine hydroxylase (PHY): homozygous deficient, heterozygous, and homozygous normal. Double-blinded crossover studies were performed at equilibrium during PHE restriction and supplementation. The mean frequency of the power spectrum and the mean across a set of alpha-theta factors showed highly significant, reversible, generalized EEG slowing during PHE supplementation in subjects homozygous for PHY deficiency. Smaller but significant changes in EEG mean frequencies occurred in the heterozygous and normal subjects. Spectral profiles of EEG change were similar in both groups; the mean alpha-theta was more sensitive in the second group. Demonstration of EEG changes with PHE supplementation in normal individuals has important dietary implications.
