Creating healing environments in critical care units.

The concept of healing environments is not new. The idea of creating an environment that would facilitate healing was popular over 2000 years ago, but it has been only recently that this concept has been applied to the design of hospitals and specifically to intensive care units. Although it is necessary to address the needs of the professional staff who are caring for seriously ill patients, it is also critical to include specific elements that will enhance the patients' healing process. It has been demonstrated that views of nature, natural light, soothing colors, therapeutic sounds, and the interaction of one's family can enhance the healing process. These elements important to a patient's healing process must be considered and balanced with the needs of health care providers in the design of critical care environments.

Through the eye of the beholder: multiple perspectives on quality in women's health care.

Quality is an illusive concept with different meanings to different people. Providers often define quality in terms of patient outcomes, professional standards of practice, predetermined criteria used to measure quality, and even subjective opinion. Patients describe quality in terms of the interpersonal aspects of care, how well they were treated, and the responsiveness of the provider to their needs. This qualitative study using a semi-structured interview defined quality from the perspectives of patients, physicians, nurses, and payers associated with a hospital-based women's service line, and how the attributes of quality varied among the multiple groups. The study also described how stakeholders become aware of quality and how they determined a hospital's quality. From the findings of the study, a conceptual framework of quality in women's health was developed.

Developing collaborative relationships.

Most professionals want to work in a positive, supportive environment. In fact, most professionals spend more of their waking hours in the workplace than they do at home. A positive work environment is not only essential for physical and mental well-being, it's also critical in ensuring a healthy outcome for patients. Collaborative relationships in the workplace can create a collegial and friendly foundation from which practice can be developed.

Integrating a clinical service line for maternal-child health.

Clinical integration is a developmental process that calls on the combined resources, expertise, and knowledge of individuals who may be working together for the first time. The process is dependent on the acknowledgment of the value of integration and a shared vision for the service line. By using a strategic thinking and planning process, the service line can be reshaped to function as an integrated system of programs and services that is sensitive to needs at a local level. While the task of integration is not an easy one, the outcomes of the process lead to a synergy that cannot be realized in any other manner.

Professional interdependence: the art of collaboration.

Nurse-physician collaboration is critical in ensuring quality, cost-effective outcomes in patient care. As an interactive process that synthesizes and integrates the skills, expertise, and resources of individuals to accomplish a goal, collaboration is characterized by interpersonal valuing that changes the relationship and forms a foundation for future interactions. Advanced practice nurses contribute to the development of organizational cultures that foster interdisciplinary collaboration because of their advanced knowledge, skill, and clinical competence, which contributes to the development of collaborative relationships.

Women's health education: the first step to self-care.

Changes in health care emphasize the importance of individuals assuming more responsibility for their own health. Although the concept of health connotes the absence of disease, it also includes the individual's ability to adapt to stressors, manage roles and activities daily, and live life in an exuberant way. Although research supports the relationship of health education and self-care, more must be done to understand what motivates individuals to pursue healthy lifestyles. Women's health education is the first step to self-care and improving the health of women and their families.

System development and integration in healthcare.

Nurse executives are challenged with integrating services within newly formed healthcare systems. The author provides a theoretical framework for system integration. The developmental process of integration in early system development, including the barriers to integration and methods to facilitate the process, are discussed.

Shared governance: what it means to those involved.

Nurses saw shared governance as both a management philosophy and an organizational structure that changes the organizational culture and belief system. Because of these two aspects of shared governance, its implementation is an evolutionary process in which former roles are examined and new roles are learned. Power shifts during the implementation of shared governance create role conflicts and confusion in the decision domains of staff and managerial nurses. Shared governance is highly valued by participants as a mechanism to enhance commitment to the organization, accountability for nursing practice, and the professionalism of both the clinical and managerial staff. The participants valued the opportunities for growth and development of leadership skills that shared governance provided. While negative perspective were shared, they seemed to be secondary to the overall satisfaction with shared governance. Some of the negative perspectives seemed to relate to the length of experience with shared governance, whereas others seemed to be essential elements of the experience.
