Keys to the diagnosis of occult urologic disease in children.

All physicians who care for children should be aware of the many indications for further urologic examination. A straightforward algorithmic approach to urologic diagnosis is not possible. The physician must individualize and carefully weight the indications for the often-times expensive and uncomfortable tests that are required for urologic diagnosis. The reward is ample when a significant correctable lesion is recognized early enough for salvage on the basis of seemingly unrelated signs or symptoms.

Acute unilateral adrenal hemorrhage following ACTH administration in a patient with Cushing's syndrome.

A patient with Cushing's syndrome and a concomitant unilateral adrenal hemorrhage following ACTH administration is described. Although stress is commonly associated with the onset of adrenal hemorrhage, a pathologically documented adrenal hemorrhage has not been previously in association with ACTH administration in Cushing's syndrome. Repeated doses of ACTH should not be given to patients with Cushing's syndrome as this has little diagnostic value and may lead to adrenal hemorrhage in such patients.

One-stage correction of chordee and 180-degree penile torsion.

The surgical management of a patient with 180-degree penile torsion, balanitic hypospadias, and chordee is presented. Complete resolution of the torsion was effected by completely reflecting the penile skin. Correction of the penile curvature was accomplished simultaneously by freeing the corpora spongiosa from the corpora cavernosa.

Management of neoplasms in vesical diverticula.

Two cases of neoplasms occurring in vesical diverticula are presented, and the pertinent literature is reviewed. It is suggested that a patient with a neoplasm in a vesical diverticulum be staged by surgical exploration and be accorded the same aggressive management as any patient with an infiltrating vesical neoplasm.

Hereditary nephritis associated with vesicoureteral reflux and vaginal duplication.

The concomitant occurrence of vesicoureteral reflux and hereditary nephritis has not been previously reported. A case of a patient with hereditary nephritis, vesicoureteral reflux, and vaginal duplication is presented and the related literature reviewed. It is suggested that familial reflux may occur significantly in families with hereditary nephritis.

Preoperative diagnosis of xanthogranulomatous pyelonephritis.

Preoperative diagnosis of xanthogranulomatous pyelonephritis may be correctly made in a significant proportion of affected patients thus preventing unnecessary radical surgery especially in the poor-risk patient. The diagnosis should be suggested in the patient with a history of chronic urinary tract infection and certain radiologic features. These include unilateral renal enlargement (either localized or diffuse), nonfunction on excretory urography, presence of renal and/or ureteral calculi, angiographic demonstration of avascular mass or masses with stretched, attenuated intrarenal vessels, prominent capsular and periureteric vessels, and an irregular impaired nephrogram with prominent avascular areas.

Transurethral resection of prostate via perineal urethrostomy. Prospective study.

A controlled, prospective clinical study was undertaken to determine the incidence and cause of urethral strictures after transurethral resection. Patients were assigned to 1 of 3 groups. Group A consisted of patients in whom it was considered preferable for various reasons to perform prostatic resection through perineal urethrostomy; therefore, these were excluded from randomization. All other patients were randomized using a sealed-envelope method into Group B who underwent resection via perineal urethrostomy and Group C who underwent resection via the entire urethra. Strictures occurred in 6 of 42 patients in Group C postoperatively while patients in Group B remained free of strictures. This difference was statistically significant (p less than 0.05). It was concluded that strictures which occur result from the use of the resectoscope through the entire urethra and not from the use of indwelling catheters, and that performing prostatic resection via perineal urethrostomy is valuable in preventing the occurrence of such strictures.

Paratesticular sarcomas: review of management.

Two patients with paratesticular sarcomas (1 leiomyosarcoma and 1 liposarcoma) are described, making the total number of cases reported 206. A review of the diagnosis and a critical analysis of the therapeutic approaches are presented.

Credé expression of the bladder: a sometimes useful maneuver.

Two children with neurovesical dysfunction are described. The upper tract changes were resolved following the initiation of the Credé expressions of the bladder. It is suggested that the Credé maneuver has a place in the management of selected children with neurovesical dysfunction, particularly when temporization is needed.

Massive renal hemorrhage associated with parturition.

Spontaneous massive renal hemorrhage, during or after parturition, not associated with blood dyscrasias or other recognizable etiology is rare. Two new cases are added to the 7 cases previously reported. The related literature is reviewed.
