1-O-alkyl-2-N-methylcarbamyl-glycerophosphocholine: a biologically potent, non-metabolizable analog of platelet-activating factor.

We prepared unlabeled and 3H-labeled analogs of platelet-activating factor (PAF) containing a N-methylcarbamyl residue at the sn-2 position. PAF and its methylcarbamyl analog competed for binding to high affinity receptors on human polymorphonuclear neutrophils; their respective dissociation constants for these receptors were 0.2 and 1.1 nM. The binding affinities of the two analogs correlated precisely with their capacities to stimulate neutrophil degranulation responses. Unlike PAF, however, the methylcarbamyl analog completely resisted metabolic inactivation by neutrophils and by human sera. Thus, these compounds' biological potencies are determined predominantly by receptor binding: cellular metabolism of the ligands neither contributes to nor appreciably limits their stimulating actions.

Effect of urinary extravasation on renal wound healing in dogs.

It is frequently stated that extravasation of urine after intrarenal surgery is deleterious to renal wound healing causing fibrosis or granulation tissue. A study using dogs was designed to test the hypothesis that the contact of urine with incised renal parenchyma may cause altered wound healing. This study showed that exposure of the renal parenchyma to extravasated urine from open collecting structures did not alter renal healing.

Massive post-ejaculation hematuria.

The case of a twenty-nine-year-old white male with massive post-ejaculation hematuria, an entity distinct from hemospermia, is described. The site of bleeding was found to be large varicosities located at the apex of the prostate. Fulguration of the veins produced complete resolution of the bleeding.

The Barcat balanic groove technique for the repair of distal hypospadias.

The Barcat balanic groove technique for the repair of distal hypospadias creates a neourethra with a ventral penile skin flap and a ventral glandular flap, both of which are based at the meatus. A series of 105 consecutive patients with distal hypospadias operated on by the same surgeon is presented. The primary complication was fistula formation in 15 per cent of the patients.

Protein kinase C regulates leukotriene B4 receptors in human neutrophils.

Three protein kinase C (PKC) activators, viz. phorbol myristate acetate, mezerein, and rac-1-O-myristoyl-2-acetylglycerol, inhibited human neutrophil binding of [3H] leukotriene B4 (LTB4) by reducing the number of high-affinity receptors available to the arachidonic acid metabolite. The inhibitory effect occurred in whole cells and cytoplasts but not in isolated membranes; it appeared to involve the activation of PKC rather than direct competition for binding sites. PKC may govern cellular responsiveness by regulating the receptor-linked bioactions of endogenous mediators like LTB4.

Purported absence of the testis following surgical exploration: the need for further investigation.

Two cases of purported absence of the testes following extensive surgical exploration are reported. On subsequent investigation both boys were found to have testes. It is recommended that when confronted with a patient with a similar history the operative record should be studied and the original surgeon should be contacted to ascertain that either a testis was removed or bind-ending internal spermatic vessels were found. If proper documentation is not available, further evaluation should be done.

Analysis of peritoneal fluid in urinary ascites.

Urinary ascites may be the most common cause of ascites in the neonate, and should be suspected in an infant with abdominal distention, oliguria, hyponatremia, and hyperkalemia. The diagnosis is confirmed when analysis of the peritoneal fluid shows creatinine, urea, and potassium concentrations higher than corresponding serum concentrations. The establishment of adequate urinary drainage by catheterization of the bladder or nephrostomy rapidly corrects electrolyte abnormalities.

Diacylglycerols and mezerein activate neutrophils by a phorbol myristate acetate-like mechanism.

rac-1-O-Myristoyl-2-O-acetylglycerol, rac-1-O-palmitoyl-2-O-acetylglycerol, and rac-1-O-oleoyl-2-O-acetylglycerol acted like phorbol myristate acetate and mezerein in stimulating human neutrophil aggregation. Responses to these agents were equally influenced by cytochalasin B, extracellular calcium and magnesium, arachidonate antimetabolites, and procedures that rendered the cells desensitized to other agonists. The compounds also inhibited the binding of [3H]-phorbol myristate acetate to its receptor on neutrophils. Thus, these agents are biologically homologous. They act by binding to a common receptor. This receptor may function physiologically as a transducer for endogenous glycerides that form in cells challenged by other stimuli.

Neonatal genital trauma.

A neonate with severe scrotal ecchymosis resulting from a breech delivery is presented. The incidence, potential complications, and management are discussed.

Intermittent cryptorchidism: a sign of occult inguinal hernia in pubescent boys.

Intermittent cryptorchidism is reported in 4 pubescent boys. Subsequent exploration revealed indirect inguinal hernias. When a pubescent boy observes the testicle to reside intermittently but primarily in a position cranial to the scrotum, a high probability of a concomitant indirect inguinal hernia is suggested.

Cystectomy: a catastrophic complication of herniorrhaphy.

Injury to the bladder is a known complication of inguinal herniorrhaphy. Infants less than 6 months old may be more at risk because the medial aspect of indirect inguinal hernia sacs are associated closely with the wall of bladder ("bladder ears"). We report 2 cases of almost complete excision of the bladder during routine herniorrhaphy. In 1 patient bilateral ureteroileocecoplasty provided a course of management free of complication.

An unusual case of testicular ectopia.

We report a case of testicular ectopia, which represents the first instance of an ectopic testicle located cranial and lateral to the internal ring between the external and internal oblique musculature.
