[The role of echography in splenic injuries in children].

We have reviewed 39 cases of ruptured spleen in children from 1979 to march 1983 at Ste-Justine Hospital in Montreal. We were mainly interested in the role of ultrasonography, its value and limitations. The practical management is exposed. To the best of our knowledge, it is the first report of this kind in pediatrics.

[Splenic injuries: therapeutic orientation. Apropos of 46 cases].

Between 1979 and june 1983, 46 splenic trauma, with or without associated lesions, have been treated at Sainte'-Justine's Hospital. 8 children had a splenectomy, 3 had a laparotomy for other lesions without a splenectomy was done. 35 had a non operative management of splenic trauma. The ultrasound shows the splenic lesions and the blood in the peritoneal cavity. So, the "Routine Abdominal TAP" does not seem necessary in every abdominal trauma. The splenic scintigraphy is, for, now, the best exam to confirm the diagnosis and to assess the resolution of splenic lesion in 1 to 4 months. Transfusions were done in 8 of 11 operated patients and in 11 of the 35 children wo underwent a conservative management. The average stay in the hospital was 11 days in these 35 last patients. No mortality or morbidity were in relation with the conservative management in our series. The exact diagnosis of splenic trauma by ultrasound and scintigraphy allows the pediatric surgeon to choose a non operative treatment, when the child has stable vital signs. Monitoring these patients in intensive care unit is mandatory and allows a careful evaluation of the course of these splenic trauma.

[Prevalence of protein malnutrition on admission in internal medicine. Study of 260 hospitalized adults].

The nutritional protein status of 260 unselected adult patients admitted to an internal medicine hospital unit and of 60 healthy controls was evaluated by measurement of the brachial muscular perimeter and of the weight--height ratio and by assay of serum albumin, prealbumin, retinol-binding protein and transferrin. The values most frequently low were those of retinol-binding protein (66%), prealbumin (42%) and albumin (25%). The sensitivity of short half-life serum protein assays is underlined. Twenty-eight p.cent of patients were found to have protein malnutrition on admission, most of these being women and patients older than 60 years. In view of the scattering of most values in controls according to age and/or sex, it is suggested that individual reference values should be determined for this type of study and that no comparison should be made with data found in the literature.

Folic acid deficiency and neutrophil dysfunction.

Polymorphonuclear leukocyte functions were studied in 92 patients with protein-calorie malnutrition. Serum folic acid levels were higher than 3 ng/ml in 38 patients and 3 ng/ml or less in 54 patients. Significant differences were found between these two groups of patients with regard to phagocytosis (81.5 +/- 1.9 versus 69.2 +/- 2.0 percent, p less than 0.001) and bactericidal ability (90.6 +/- 1.1 versus 84.5 +/- 2.3 percent, p less than 0.05). Correction of folic acid deficiency in 22 patients was associated with recovery of normal phagocytosis (p less than 0.001) but not bactericidal function. Adding folic acid to the serum of eight patients also restored normal phagocytic function (p less than 0.001). A correlation was found in vivo and in vitro between changes over time in folic acid levels and in phagocytosis.

Monoclonal gammopathies in the adult population of Finistère, France.

Three hundred and thirty-four monoclonal gammopathies were detected in the sera of 30 279 adults from Finistère. Monoclonal gammopathies (MG) are more common in Finistère than in Paris and their distribution is not homogeneous. IgG paraproteins are particularly common in the northeast of Finistère whereas IgM paraproteins are more common in the southwest. Family studies and the high degree of inbreeding would support the hypothesis that there is a genetic predisposition to develop MG but the occurrence of paraproteins among three non-consanguineous couples seems to favour the existence of an environmental factor.

Effect of folic acid deficiency on nonspecific immunity (phagocytic activity and nitroblue - tetrazolium reduction).

Fifty three critically ill patients (two of the following criteria : thyroxin - binding - prealbumin less than or = to 24 mg/dl, creatinin - height - index less than or = to 80 percent and transferrin less than or = to 225 mg/dl) were divided into two groups according to the level of folic acid (FA). Group A consisted of 29 with FA less than or = to 3 ng/ml and group B of 24 with FA greater than 3 ng/ml. The phagocytic activity (PA) was significantly lower in group A and significantly corrected by AB sera, whereas the NBT reduction capacities were within the normal range. A correlation is shown between PA and level of FA as well in autologous sera as in AB sera.

[Measurement of cardiac output by electrical impedance plethysmography during respiration and in apnea].

The measurement of cardiac output by electrical impedance plethysmography (non-invasive technique) may be disturbed by a certain number of factors, including respiration. The authors sought to demonstrate that this factor could be eliminated during measurement calculations. By this method, in 18 adult subjects with coronary artery disease, 50 measurements during respiration and 50 measurements in apnoea (paired measurements) of stroke volume and of cardiac output were performed. Stroke volume was calculated using Kubicek's formula: SV = P(L2/Zo2) (dz/dt min)t Results were as follows: highly significant correlation between values for stroke volume obtained during respiration and in apnoea (r ;.9767; n=50; P < 0.001); highly significant correlation between values for cardiac output obtained under the same conditions (r = 0.9744; n=50; P < 0.001). The authors conclude that with the limits of certain technical conditions of measurement, respiration does not disturb measurements of stroke volume and of cardiac output by electrical impedance plethysmography.

[Contribution of motor nerve conduction velocity and Hoffmann reflex in early detection of neuropathy in diabetics. Results and correlations with duration and other long-term complications in 117 patients (author's transl)].

Motor nerve conduction velocity and H. reflex parameters were studied in 85 insulin-treated diabetics classified in groups I to VI, according to the duration of the disease, in 32 maturity-onset diabetics (groupe V) and in 92 healthy control subjects. Each diabetic subject was examined prior to testing for clinical evidence of neuropathy and also had funduscopic examination, fluorescein angiography and an estimation of renal status by serum creatinine measurement and proteinuria. Electrical parameters were significantly different in all groups of diabetics from results in the control group. Neuropathy, as judged by these electrical studies, was frequently present in the absence of clinical expression, worsened with the duration of the disease in groups I to IV and preceeded other degenerative changes of diabetes. H. reflex parameters are an earlier index of neuropathy than motor nerve conduction velocity and may be used as a monitor of diabetic neuropathy.

[Distribution of HLA antigens among the population of Finistère (France) (author's transl)].

We have studied by using HLA system, 200 people, without filiation but having the same geographic origine (Finistère). The results have been compared with those measured for Parisian and Basque population. A significantly decrease of antigen A29, B18 and BW16 frequency have been observed in the population from Finistere. But, antigen B8 and B27 seems to be more frequent than in population from Paris. A linkage desequilibrium is described for A1-B8 and A3-B7 haplotypes as in population from Paris; but the desequilibrium for A11-B5 seens to be characteristic of population from Finistere.

HLA antigens and monoclonal gammapathy.

Despite the strict criteria required to distinguish Multiple Myeloma (M.M.) or Waldström's Macroglobulinemia (W.M.) from Benign Monoclonal Gammapathy (B.M.G.), nosological frontiers are still unclear and accordingly justify a comparative serological study of M.M., W.M., and B.M.G. patients.
