Histopathological method for diagnosis of cryptosporidiosis using monoclonal antibodies.

A monoclonal antibody directed against the oocyst wall of Cryptosporidium parvum was produced which strongly reacted with 25 strains of the parasite. This antibody was assessed for use in a diagnostic immunoperoxidase assay performed on tissue sections. It permitted an intense and specific staining of the parasite in five biopsies positive for Cryptosporidium parvum fixed with routine techniques. The procedure described may be useful in screening mucosal biopsies from immunosuppressed patients.

[Acute toxoplasmic myocarditis. A cause of sudden death in a case of acquired immunodeficiency syndrome].

A case of acquired immunodeficiency syndrome with a sudden death due to a toxoplasmic myocarditis is reported. The diagnosis was postmortem. At the autopsy, only the heart was affected by toxoplasmic lesions. The toxoplasmosis is a frequent disease in AIDS patients. So a toxoplasmic myocarditis must be suspected when a severe cardiac dysfunction is observed in these patients.

[Angiosarcoma of the pericardium. Anatomoclinical study of a case].

We report a case of angiosarcoma of the pericardium in a 62-year-old man. Although the lack of myocardial adhesions allowed complete surgical exeresis, and despite chemotherapy, the patient died three months later because of early recurrence. Large, undifferentiated areas were found within the tumor, raising diagnostic problems with other malignant tumors of the heart. Histochemical, immunohistochemical, and ultrastructural studies confirmed the diagnosis of angiosarcoma.

[Favorable outcome of Wegener's disease limited to the lungs. Apropos of a case].

A case of Wegener's granuloma limited to the lungs is reported in a 22 year old man. The pulmonary radiograph showed bilateral nodules. A lung biopsy of a nodule revealed a necrosing vasculitis involving the arteries and the veins and some areas of necrosis in geographical contours around these vessels. The renal function was normal and 4 1/2 years later the outcome remains favourable with the minimum of treatment, the exception being a course of antibiotics post operatively. Fifty seven cases of Wegener's granuloma limited to the lungs have been diagnosed after lung biopsy, transbronchial or bronchial material. The outcome was favourable in 72% of cases. This result was observed in the absence of any treatment in 6 cases (13%). In 2 cases the pulmonary nodules disappeared or developed cavities. In 4 cases the anatomical and/or radiological progress is unknown. Currently the factors leading to such an outcome have not yet been identified.

[Cardiac lesions in acquired immunodeficiency syndrome (AIDS). Apropos of an autopsy series of 25 cases].

The pathologic study of the cardiac lesions in 25 persons who died of AIDS were studied from autopsies. Most of these patients were intravenous drug abusers (14 cases). Heart failure was symptomatic and lead to death in 4 cases. This study showed histological abnormalities in 76% of the cases. We observed 12 myocarditis. In 6 cases, pathogenes were found: Toxoplasma gondii (2), Cryptococcus neoformans (2), Candida (1), Aspergillus (1). A lymphocytic myocarditis was observed in 6 hearts. By immunohistochemical technique, we could distinguish 2 toxoplasmic myocarditis, and in 4 cases, solitary cysts in the myocardium without inflammation. The remaining lesions comprised respectively: 3 lymphocytic pericarditis, 2 marastic endocarditis and 1 dilated myocardiopathy.

[Post-traumatic spinal arachnoid cyst].

A 15 year old boy presents two fractures of the left inferior member (femur and tibia), after a motor-cycle accident. An osteosynthesis by plate and screws was performed, in emergency. In immediate post-operative days, a radicular paresia of the left inferior member appear especially on exterior popliteus sciatic nerve. The electromyogram confirm the diagnosis. However, these is nor pain nor fracture of the sacro-lumbar spine. The consolidation of femur and tibia fractures is normal. The neurologic recuperation is slow but progressive. A year after the accident, because the recuperation is not complete, an MRI is performed. An arachnoidal cyst surrounding the L5 and S1 roots is diagnosed. The surgery is decided. The ablation of the cyst is performed in the same operative time that the ablation of plate. The histology of the paries of the cyst confirm, by hemosiderimic deposits, the traumatic etiology. The arachnoïdal cyst is rare, particularly in the childhood. It is always difficult to know the relation between the cyst and the symptom. It is also impossible to be certain of traumatic etiology: traumatic cyst or traumatised cyst? The surgical indication is lawful by the existence of clinical signs, because 50 per cent of cases are improved or recovered.

[Isolated involvement of the central nervous system in cysticercosis. Anatomo-clinical study of a case].

We observed one case of neurocysticercosis due to Taenia Solium. The patient had stayed at the Green Cap Islands and in Portugal until 1987, when he arrived in France and died one month later. The diagnosis had been suspected from the results of computerized tomography and serological examination. Finally, we confirmed that diagnosis by the post mortem parasitology study of the specimen of brain.

CD 15 antigen in Hodgkin's disease.

A retrospective analysis of 117 cases of Hodgkin's disease treated at the Centre G.F. Leclerc between 1976 and 1985 was performed with three objectives: 1): to re-evaluate the histologic subtype by the Lukes-Rye classification according to recent data; 2): to demonstrate the frequency of CD 15 antigen by an indirect, three stage immunoperoxidase technique on initial node biopsy histologic sections; 3): to study the prognostic value of this antigen. Histologic reclassification disclosed that 9 cases were in fact non Hodgkin's lymphoma. The remaining 108 cases were classified as lymphocyte predominant (n = 11), nodular sclerosis (n = 77), and mixed cellularity (n = 17), with no cases of either lymphocyte depletion or nodular paragranuloma of Poppema and Lennert identified. In these specimens, fixed in Bouin's solution and embedded in paraffin, CD 15 antigen was detected in the Reed-Sternberg cells and the mononuclear variety of Hodgkin cells in 64 patients (59%). The presence of this antigen was independent of histologic subtype, patient age or sex, clinical stage and the presence of systemic symptoms. Both actuarial disease-free and overall survivals showed that the prognosis of Hodgkin's disease is more favorable in CD 15 positive cases.

[Experimental study of submucous vesical injections in rats. Teflon versus silicone].

Correction of vesicoureteric reflux by endoscopic subureteric injection of polytef paste is an expending procedure. But polytef past is thick, hard to inject and can migrate at distant sites of injection. We have tried a silicone paste on experimental studie observes tissue reaction at the site of injection (bladder) and in pelvic nodes, kidneys, liver, lungs, brain. 22 rats were injected: 8 with polytef paste, 14 with silicone paste. The study demonstrated that polytef paste sub mucosal injection is followed by migration nodes and local granulomatous reaction. The silicone paste is easier to inject; none particules of silicone were found in nodes nor in others visceria; granulomatous reaction and fibrous capsule around foreign body were smaller.

[Hemangiopericytoma of the thyroid gland].

Hemangiopericytoma is an uncommon tumour of the thyroid gland. We report the fourth case of thyroid hemangiopericytoma in a 77 year-old woman who presented a goiter known for 23 years. The tumour, 11.5 X 7 X 7 cm in diameter, was located in the left lobe. Its histological features were those of a benign hemangiopericytoma as in two cases previously reported. Light microscopy discovered capillaries surrounded by sheets of plump cells and a reticulin network encircling each cell. A few tumour cells showed slight immunoreactivity with antibody against actin. There was no immunoreactivity with antibodies against desmin, vimentin and myosin intermediate filaments. Today, the electron microscopic analysis which revealed that tumour cells looked like pericytes, remains the best investigation for hemangiopericytoma diagnosis.

[Subcutaneous rhabdomyosarcoma in children. Clinical, immunologic and ultrastructural aspects].

Embryonal rhabdomyosarcoma is the most frequent of tissue sarcomas in children. Its location in the subcutaneous tissue makes it a dermatological diagnosis. In childhood, the exact histological type of the tumour, sometimes difficult to determine, is absolutely necessary since prognosis and treatment differ according to the histogenetic form. Rhabdomyosarcoma is rare in adults. One must rule out malignant pleomorphic histiocytoma which has a more favourable prognosis. Among the antisera recently made available, those directed against desmin, foetal skeletal myosin and/or specific skeletal muscle myofilament seem to be most useful when associated with the anti-myoglobin antibody.

[Deficiency of NK activity evolving in a parallel manner with immune granulopenia].

The role of NK cells and their place in blood lines remain the subject of controversy. The authors report on a 42-year-old patient who exhibited a regressive auto-immune granulopenia following administration of anti-lymphocytic serum and also during immunosuppression by cyclophosphamide after a relapse. The NK activity level closely paralleled the evolution of the granulopenia. The possible relationships between these two phenomena are discussed, and the possible role of membrane antigens common to NK cells and myeloid lines.
