Using video resumes to teach deaf college students job search skills and improve their communication.

For several years the National Technical Institute for the Deaf (NTID) at the Rochester Institute of Technology has offered a course that focuses on preparing video resumes. Professionals from the Instructional Television Department and the National Center of Employment for the Deaf have collaborated with communication instructors to develop this option for NTID students. This paper describes the instruction and evaluation process from the perspective of NTID instructors, students, and other technical professionals. Employers who have hired NTID students and those who have no familiarity with deaf employees also participated in the evaluation. Their comments give valuable insights into the often different perspectives of deafness between those who have worked with deaf employees and those who lack such experience. Video resumes were found to be a viable tool in the job search, and the process of preparing the video was found to be a stimulating and rewarding experience for the participating students.

Elective laparoscopic cholecystectomy for "all-comers".

Laparoscopic cholecystectomy is a safe and effective procedure in specialist centres, but its wider application will depend on the ability of general surgeons to become skilled in this technique. 180 patients underwent elective laparoscopic cholecystectomy during a nine-month period at a single district general hospital. All patients who would have been eligible for elective open cholecystectomy were offered the laparoscopic alternative. Laparoscopic cholecystectomy was abandoned in favour of an open procedure in 10 (6%) patients. Median operating time was 55 min. 2 patients had serious morbidity (right hepatic duct injury and a duodenal fistula) and there were no deaths. 3 patients required subsequent treatment for retained common duct stones. 90% of patients were discharged within 48 h of the operation and the median time to resume full activity after discharge was 12 days. Laparoscopic cholecystectomy is a safe and cost-effective technique in a district general hospital.

Spoken and written English errors of postsecondary students with severe hearing impairment.

This investigation compared the spoken and written English errors of 20 hearing-impaired postsecondary students with intelligible speech and poor English language. Error categories used to assess the language samples were function, content, and structure. Spoken and written performances were distinguished only by a greater number of function word errors in writing samples. A trend toward greater complexity in writing was also found. Implications for instruction are discussed.

Renal tuberculosis and accelerated hypertension: the use of renal vein renin sampling to predict the outcome after nephrectomy.

This case report records the rare presentation of accelerated hypertension secondary to long-standing renal tuberculosis and the subsequent beneficial response to surgery predicted by lateralisation of renin levels within the renal veins. The patient's hypertension proved difficult to control medically and he was therefore assessed for possible surgical management. Renin sampling revealed clear lateralisation within the renal veins indicating possible benefits from surgery. Nephro-ureterectomy was performed and, post-operatively, the patient's hypertension, while not completely resolved, was easily controlled with monotherapy. Patients with unilateral renal tuberculosis and hypertension which prove difficult to control should have measurement bilaterally of renal venous renin which may indicate potential benefits from surgery.

Narcolepsy in the elderly: a forgotten diagnosis.

Two cases of narcolepsy presenting in elderly patients are reported. Delay in diagnosis prior to presentation resulted in considerable psychosocial problems for both patients. Satisfactory responses to therapy were achieved with patients regaining independence. Difficulties associated with this diagnosis in the elderly are discussed.

A radioreceptor assay in which iodocyanopindolol is used to determine propranolol and its active metabolites in unextracted serum or plasma.

This rapid, sensitive, simple radioreceptor assay (RRA) for l-propranolol and its active metabolites in unextracted samples requires 5 microL of sample, a beta-adrenergic antagonist, 125-l-labeled (-)cyanopindolol (125ICYP), and turkey erythrocyte membrane receptors (Kd = 40 pmol/L). Equal volumes (100 microL) of diluted sample and 125ICYP are incubated with 500 microL of erythrocyte membranes for 30 min. Cold isotonic saline (2.5 mL) is added, and the mixture is centrifuged. The concentration of drug that inhibited receptor binding by 50% (IC50) was l-propranolol, 1.5 micrograms/L; d-propranolol, 243 micrograms/L; and 4-hydroxypropranolol, 8.8 micrograms/L. Analytical recovery of propranolol added to serum was 93 to 120%; results for 138 clinical samples tested by this method and by liquid chromatography correlated well (r = 0.96). Results correlated strongly (r = 0.98) for 23 clinical samples tested by RRA and analyzed for both propranolol and 4-hydroxypropranolol by liquid chromatography. The sensitivity of this RRA was 0.3 micrograms/L, and both intra- and interassay CVs were less than 12%. Modifications of this method could permit testing of other nonselective beta-blockers.

Use of protein A to remove immunoglobulins from serum in hybridoma culture media.

The levels of protein A-reactive immunoglobulin (PA-Ig) in foetal bovine serum were measured in commercial batches. For tissue culture media incorporating 10% foetal bovine serum, the levels of bovine PA-Ig were of a similar order to those of mouse monoclonal antibodies produced by hybridomas grown in such media. The equilibrium constants were calculated for the binding to protein A-Sepharose of a number of mouse monoclonal antibodies, and of PA-Ig in foetal bovine serum and normal mouse serum. The average affinity of the mouse PA-Ig was 10 times higher than that of the bovine PA-Ig, suggesting that the two could be separated by affinity chromatography on protein A-Sepharose. The mouse monoclonal antibodies, however, displayed a range of affinity 1.5-100 times that of the bovine PA-Ig, indicating that such separation could not be generally applied. The optimal technique involved removing PA-Ig from bovine serum before its inclusion in the culture medium and then purifying the monoclonal antibody on a second protein A-Sepharose column.

Integrated spoken and written English instruction for the hearing-impaired student.

This study was designed to investigate the effect of integrated instruction in both spoken and written English on oral expression of hearing-impaired persons. Twenty-five hearing-impaired young adults were enrolled in English courses that focused on reading and writing skills only (control group) or on reading, writing, and complementary oral language instruction (experimental group). The experimental group demonstrated significant improvement in grammatical correctness of spoken English and in speech intelligibility, while the control group failed to demonstrate a significant improvement in these areas.

Multiple bilateral breast adenomata in identical adolescent Negro twins.

Identical adolescent Negro twins presented with multiple bilateral breast adenomata. There were three pure tubular adenomata, one mixed tubular and fibroadenoma, four glandular fibroadenomata and one typical intracanalicular fibroadenoma. Tubular adenomata are exceedingly rare and this report adds a further three to the literature. Furthermore, the close similarity of the glandular fibroadenomata and tubular adenomata, the occurrence of tubular and fibroadenomatous areas in the same tumour, and the simultaneous occurrence of both tumour types in the same genetic setting, point to a close relationship between these types of neoplasm. The simultaneous occurrence of breast adenomata in identical twins suggests, but does not prove, an important genetic contribution to the aetiology of these tumours. The twins were both virgins, and, therefore, it is clear that pregnancy is not a pre-requisite for the development of tubular adenomata, as has been suggested.

The effect of vitamin C supplements on body weight, serum proteins, and general health of an elderly population.

A double-blind placebo trial of vitamin C has been undertaken in 94 elderly "long-term" inpatients known to have initially low levels of plasma and leukocyte vitamin C (mean values 0.17 mg/100 ml plasma; 10.1 microgram/10(8) leukocytes). At the end of 2 months treatment, plasma and leukocyte vitamin C had increased considerably in those receiving C supplements, and in this group there were slight but significant increases in the mean values for body weight (0.41 kg), plasma albumin (0.46 g/l), and prealbumin (25.4 mg/l) compared with those receiving placebo therapy where there were decreases of 0.60 kg, 0.53 g/l, and 7.0 mg/l, respectively. There was also some clinical improvement as indicated by reductions in purpura and petechial hemorrhages in those receiving vitamin C, but no changes in mood or mobility were observed.

Regulation of bronchomotor tone by lung inflation in asthmatic and nonasthmatic subjects.

We examined the effects of lung inflation on induced airway obstruction in 14 atopic asthmatic and 14 atopic nonasthmatic subjects. Subjects were challenged with aerosols of methacholine (MCh) and pollen antigen (Ag), and the effects of inflation were assessed with partial ad full flow-volume curves and by comparing airway conductance measurements before and after deep inspiration to total lung capacity (TLC). Whereas bronchoconstriction was transiently abolished or reduced with inspiration in nonasthmatics, these effects were absent or diminished in asthmatic subjects. Dissimilarities could not be explained by differences in base-line lung function or degree of obstruction produced. Deep inspiration had a greater effect in reducing airway obstruction produced with MCh than with Ag in nonasthmatics. In addition, atropine pretreatment had no effect on inspiration responses in asthmatics given Ag, suggesting that vagal reflexes were not the cause of an impaired ability to reduce bronchomotor tone by lung inflation. Our findings reveal the existence of an intrinsic means of regulating bronchomotor toe by active changes in lung volume and that such a mechanism is impaired in asthma. We suggest that airway hyperactivity in asthma is perhaps less a reflection of enhanced end-organ responsiveness than a reflection of this impaired capacity.

Financial issues which may affect the graduate dental education system.

This paper identifies issues, develops alternatives, and analyzes and evaluates the impact of trends and predicted changes in health care and health care financing; compares graduate dental education with nonprofessional and professional (especially medical) graduate education; and concludes there are greater similarities than differences in the financing of education in the academic disciplines and the professions. Two generalizations are drawn from this comparison: (1) practical experience under supervision precedes practice, and (2) the student pays for education solely for his or her benefit. Students are remunerated for productive work in lieu of compensated staff. This discussion identifies a distinction between medical and dental education that influences graduate dental education. In medicine, the prevailing practice in the clinical years is for students to observe care of patients provided by qualified physicians, in dentistry, prevailing practice is for students to provide care under the supervision of qualified dentists. A comparison of clinical training in medicine and dentistry discloses the following.
