A novel lectin-based enzyme-linked immunosorbent assay for the measurement of IgA1 in serum and secretory IgA1 in secretions.

Jackfruit lectin (Jacalin) specifically binds IgA1 and secretory IgA1 (sIgA1). Jacalin was successfully purified, and used as the immobilized phase in novel noncompetitive enzyme-linked immunosorbent assays (ELISAs) for the measurement of IgA1 in serum and sIgA1 in saliva, using jacalin affinity purified material as standards. A non-competitive ELISA for the measurement of total sIgA was also developed using commercially available reagents.

Congenital neutropenia and low serum immunoglobulin A: description and investigation of a large kindred.

A kindred is described in which neutropenia and low serum immunoglobulin A levels has occurred. The affected members have variable clinical courses which do not appear to relate to the degree of neutropenia. The bone marrows show a maturation delay in granulocyte production, with increased numbers of lymphocytes. In vitro cultures of the bone marrows failed to produce granulocyte colonies. Studies of peripheral blood and bone marrow lymphocyte subpopulations give inconsistent results, although a general increase in T suppressor lymphocytes compared with T helper lymphocytes emerges. A therapeutic trial of corticosteroid and then of lithium carbonate was without effect on one of the patients. The pathogenetic basis for this probably X-linked disorder remains unclear.

The effect of danazol on the production of C1 inhibitor in the guinea pig.

The production of C1 inhibitor (C1-INH) in guinea pig liver was studied following administration of danazol. Immunochemical assays of the inhibitor were performed on serial serum specimens. An increase in serum C1-INH was observed following danazol treatment. Immunohistochemical studies confirmed that the liver is the sole site of synthesis of C1-INH in guinea pigs. This appears to occur in certain clones of hepatocytes. Ultrastructural studies revealed an increase in coarse endoplasmic reticulum and mitochondria in danazol-treated animals. In in vitro studies with liver slices, however, more C1-INH was detected in culture supernatants from untreated animals than from danazol-treated animals. This observation suggests that secretion of C1-INH from stimulated hepatocytes may require additional factors which are present in vivo but not in vitro.

Circulating lymphocyte subpopulations in pregnancy: a longitudinal study.

Peripheral blood lymphocyte subpopulations were studied sequentially during the gestation and the postpartum period of 84 obstetrically normal women. Both the percentage and absolute numbers of total peripheral blood lymphocytes decreased in the second and third trimester. Although there was a further decrease in percentage numbers 1 wk postpartum, the absolute numbers returned to normal. Absolute numbers of OKT3+ and of OKT4+ cells decreased in the second and third trimesters; OKT8+ cells decreased in the third trimester only. Absolute numbers of all T cells returned to normal postpartum. The absolute numbers of B lymphocytes, while showing no consistent pattern, decreased significantly in the third trimester and in the second postpartum period; however, the percentage values of all lymphocyte subpopulations did not alter during or after pregnancy. The data do not support the concept that a numerical imbalance in the T cell subpopulations has a causative role in the immunodeficiency of pregnancy.

Acquired hypogammaglobulinemia following infectious mononucleosis.

The case of a 17-year-old male who developed hypogammaglobulinemia following infectious mononucleosis is presented. The family history revealed that a male sibling had died some years earlier with encephalitis also following infectious mononucleosis. Laboratory investigations revealed adequate numbers of T and B cells and normal proportions of helper and suppressor T lymphocytes but poor in vitro responses to mitogens and an absence of hypersensitivity to skin test antigens. Serial serological examinations for Epstein-Barr virus antibodies indicated a primary immune response to this virus in a hypogammaglobulinemic individual. The patient probably represents a case of the X-linked lymphoproliferative syndrome. The unique feature of the present case is the demonstration of Epstein-Barr virus-specific T-cell memory. The significance of this finding and the variable expression of this syndrome in two members of the same family are discussed.

Cellular response to alpha-gliadin in untreated coeliac disease.

An improved technique for the detection of alpha-gliadin sensitised mononuclear cells in the peripheral blood of untreated coeliac patients is described. This method is a modification of the direct LMIF assay, and involves exposure of lymphocytes to alpha-gliadin and the assay of the resultant lymphokine produced using normal leucocytes as indicator cells. All untreated coeliac patients, 14 of 15 treated patients, and two of 28 controls responded to alpha-gliadin. The direct LMIF assay in comparison is less sensitive, and detected sensitivity to alpha-gliadin in only four out of eight patients with untreated coeliac disease. Use of the indirect LMIF technique demonstrates that in untreated as well as treated coeliac patients there are cells sensitised to alpha-gliadin circulating in the peripheral blood. These findings may have pathogenic and diagnostic significance.

Histological and immunological investigation of liver-specific protein (LSP) immunized rabbits compared with patients with liver disease.

In this study 45 patients with a variety of liver diseases did not demonstrate T lymphocyte sensitivity to liver-specific protein (LSP) as assessed by lymphocyte transformation whereas LSP-immunized rabbits developed both cellular and humoral immunity to this antigen. Although these LSP-immunized rabbits demonstrated portal tract inflammation with some hepatocyte necrosis, rabbits immunized with antigens known to contaminate LSP developed similar lesions. These findings contrast with those of other investigators who have reported immune responsiveness to LSP in liver patients and chronic active hepatitis in LSP-immunized rabbits. In determining the significance of these studies it must be emphasized that all preparations of LSP contain an heterogeneous group of antigens and that the specific sensitizing antigen has yet to be identified.

Mitogen responsiveness in viral hepatitis and chronic active hepatitis: the role of reversible suppressive influences.

Depressed phytohaemagglutinin and concanavalin-A responsiveness was found in patients with acute viral hepatitis (VH) when a suboptimal mitogen stimulus was used. Normal responsiveness was observed with optimal mitogen stimulation. These findings were independent of extrinsic serum inhibitors. When viral hepatitis lymphocytes were preincubated before mitogen addition an enhanced responsiveness similar to the control group occurred. These in vitro findings are in favour of a primary defect in lymphoproliferation in viral hepatitis and do not suggest the presence of reversible suppressive influences such as an excess of short-lived suppressor cells or the presence of cell bound inhibitors. In chronic active hepatitis (CAH) lymphoproliferation induced by immediate mitogen stimulation was similar to control studies. However when CAH cells were preincubated before mitogen addition, enhanced responsiveness significantly greater than in controls occurred. It is suggested that suppressive influences are present in CAH and that their effect can be reversed by cellular preincubation.

In vitro studies of suppressor cell function in human peripheral blood mononuclear cells.

Peripheral blood mononuclear cells (PBMC) from normal donors, pre-cultured at 37 degrees C for 24 hr before the addition of mitogen, demonstrated an enhanced proliferative response. This may be due to the loss of a subpopulation of suppressor cells during the incubation period. Still further enhancement was observed when pre-culturing was prolonged for 48 hr, while cells pre-incubated at 4 degrees C showed no increased responsiveness. Concanavalin A (Con A) pre-activated PBMC supressed the mitogen response of responder cells. More marked suppression was observed when the concentration of Con A used to induce the suppressor cells was increased. It was not possible to activate suppressor function in cells which had been kept in vitro for longer than 48 hr. These findings support the concept of the existence and function of suppressor cells, and that the suppressive influence is short-lived in vitro culture.

Cold agglutinins in a case of chronic lymphatic leukaemia. A study of the lymphocyte surface.

A case of chronic lymphatic leukaemia with cold agglutinins is presented in which peripheral blood lymphocytes of varying maturity appeared to possess T- and B-cell markers. A cold agglutinin IgM in type is identified in the serum and at the cell surface. This seems to be responsible for the spontaneous rosetting of sheep red blood cells (SRBC); A single clonal origin of these cells is suggested with maturation arrest. It is also suggested that with configurational change in lymphocyte surface cryoglobulins may sometimes produce spontaneous rosetting with SRBC.
