Comparison of different coefficients and units in rhinomanometry.

The total information of a rhinomanometric measurement is given by the simultaneous registration of flow and pressure during respiration. For characterization of such a measuring curve different methods and units are used. Since no normalized procedure exists up to now, it is difficult to compare measurements of different authors. In this work the relation between various procedures is discussed and methods are given for converting different quantities. The units of the International System are discussed.

[Active rhinomanometry with a minicomputer and printer].

A rhinomanometer is described which is coupled with a minicomputer and printer. The experimental results can be presented on a monitor and printed out via the printer. Besides plottings of rhinomanograms it is also possible to print out V1.5 values, the side difference, the decongestion effect, and other data of the patients in tabular form. Several measurements of the same patient, e. g. before and after decongestion, can be plotted and compared simultaneously. A nozzle system or a mask can be used. The advantages for the clinical application of this automatic computer system are described.

[Endonasal surgical technic with the argon laser].

A unit consisting of an argon-ion-laser, a flexible beam guide system and a probe for endonasal lasersurgical applications is described. The instrument has been used successfully for several years. Endonasal lasersurgical techniques are described. Pathological tissue can be coagulated or carbonised in the form of a point or a line. In particular, lasersurgical techniques are described for surgery of the lower turbinates, for coagulation of Little's area in case of chronical nose bleeding, for coagulation or carbonisation of teleangiectasia in case of Morbus Rendu-Weber-Osler, for production of a nasoantral window, and for other applications.

[Rhinomanometric measurements during gradual physical exertion in subjects with normal nasal breathing].

28 healthy volunteers, 14 males and 14 females, aged from 18 to 45 years with normal nasal breathing were measured rhinomanometrically after physical exercise on an ergometer bicycle after increase in loads by steps of 25, 50, 75, 100, 125 and 150 watt and also 2, 5 and 10 minutes after physical exercise with 150 watt. V15 (nasal air flow related to 15 mm H2O) was registered. A new rhinomanometer with a Prandtl type tube was employed for measuring a relationship between nasal airflow (V2) and transnasal pressure (delta p) yielding a straight line in the V2-delta p-diagramm using nozzles and active anterior method. An increase of V15 up to 150 watt was measured. The average V15 value of the 28 persons for the total nose without decongestion the nasal mucosa before physical exercise at rest is 0.69 l/s and after physical exercise up to a load of 150 watt 1.09 l/s, so that the increase of V15 is 0.4 l/s. This decongestion effect of the nasal mucosa is twice to three times larger than the decongestion after xylometazolin spray 0.1% (Otriven 0.1%). The medicamentous decongestion effect of V15 = 0.14 l/s is as large as the decongestion effect caused by physical exercise with a load of 25 to 50 watt. A decrease of V15 is measured reaching a normal value within 15 minutes. The rhinomanometrical normal values under physical exercise within a load of 0 to 150 watt of healthy volunteers with normal nasal breathing are readable directly in a V15-watt diagramm for the right and left nasal cavity and the total nose based on the average values of all 28 volunteers. Individual variations of the maximal decongestion effect during physical exercise are discussed.

[Normal values of nasal airflow measured with a newly developed rhinorheomanometer with modified Prandtle-type tube and linear relations].

A new rhinomanometer with a Prandtl type tube for measuring a square relationship between nasal airflow (V2) and transnasal pressure (delta p) yielding a straight line in the V - delta p - diagram is described. This so called rhinomanogram with straight lines is like an audiogram and gives a direct reading of V in relation to delta p. The active anterior method using nozzles is employed. 337 volunteers 185 females and 152 males aged 9-76 years with normal breathing function of the nose and normal endonasal mirror findings are examined with the above mentioned rhinomanometer. The normal values of nasal airflow in relation to 15 mm H2O in l./sec. (V15) of both nasal cavities before and after shrinkage of the nasal mucosa are measured during inspiration and exspiration. For all age groups, both male and female, the normal value of V15 of the entire nose is 0,67 +/- 0,16 l/sec before and 0.81 +/- 0.17 l/sec after shrinkage of the nasal mucosa. Slight differences in the normal values of the entire nose exist between males and females of different age groups as well as during inspiration and exspiration. V15 in females is 0.63 l/sec before and 0.77 l/sec after shrinkage, whereas V15 in males is 0.73 l/sec before and 0.86 l/sec after shrinkage. With increasing age, both in males and in females, there is slight increase of V15: V15 in the age-group of 9-16 years from 0.58 l/sec before to 0.72 l./sec. after shrinkage; in the age-group of 17-40 years it is 0.66 l./sec. before and 0.80 l./sec. after shrinkage whereas in the age-group of 41-76 years it is 0.75 l./sec. before and 0.88 l./sec. after shrinkage. During inspiration V15 is somewhat lower than during exspiration. Our normal V15-values are compared with all normal values of the nasal airflow mentioned in literature by other authors.

[The anesthetist and operating room hygiene].

Environmental contamination with bacteria in the course of extubation was shown to be minimal as compared to contamination of the hands and gowns. 7 out of 21 anaesthesiologists were carriers of staphylococcus aureus. Two persons spread staphylococci to their patients and the operating room environment in spite of strict adherence to conventional hygienic policies.

[On the respiration analeptic effect of fominoben HCl after administration of pethidine/promethazine (author's transl)].

The respiratory stimulatory effect of 3'chloro-2'-(N-methyl-N-[(morpholino-carbonyl)methyl]-aminomethyl)benzanilide hydrochloride (fominoben, Noleptan, PB 89) on the respiratory-depressing effect of pethidine/promethazine was investigated. In addition a double blind study was carried out on 36 patients with fominoben or placebo after pethidine/promethazine. To evaluate respiratory function the vital capacity, Tiffeneau-value, volume of inhaled air/s, maximum breathing capacity, as well as the blood gas values (pO2, pCO2 and pH according to Astrup) were measured. The results can be summarised as follows: 1. As was to be expected the combination pethidine/promethazine caused a deterioration in all parameters. This negative effect could be partially improved by fominoben. 2. A marked difference in the volume of inhaled air/s could be seen after fominoben in comparison to the placebo. The remaining differences were insignificant. 3. Pethidine/promethazine had, as had been expected, a significant effect on pain symptoms, but no analgetic effect could be shown for fominoben.

Temperature distribution in tissue during laser irradiation.

During irradiation with an Ar+-and Kr+-laser the temperature rise in living and dead tissue was measured in various depths. The contribution of the blood flow to the heat transport during irradiation was found to be small. The influence of the different depths of penetration and backscattering at the blue-green Ar+-and the red Kr+-radiation was investigated. The measurements compared with theory show reasonable agreement for an one-dimensional model below 100 degrees C. The results are discussed with respect to laser application in medicine.

A laser-endoscope with direct beam coupling.

An optical element for direct laser-beam coupling into the observation optic of a rigid endoscope was developed. Using this coupling element every suitable endoscope can be modified into a laser endoscope which can be used for laser surgery e.g. in ENT and urology. The requirements for the endoscope and the optical coupling element are discussed.

Production of a nasoantral window with an Ar+-laser.

In vitro experiments on perforating and cutting 14 human nasoantral walls using an argon laser are described. The drilling velocity is proportional to the power density up to 100 W/mm2. The cutting velocity is measured using focussed spots of 40 micrometer diameter and powers from 5 to 40 W. Histological studies were performed at the edges of the lesion. The experiments show that the technical problem for producing a window in the nasoantral wall using a laser can be solved.
