Ambras syndrome: delineation of a unique hypertrichosis universalis congenita and association with a balanced pericentric inversion (8) (p11.2; q22)

Congenital hypertrichosis universalis is a rare autosomal dominant disease. We report the further development of a Greek girl, now aged 3 years, the first case associated with a balanced structural chromosomal aberration. She was described as a neonate by Sigalas et al. (1990). Her persistent generalized hypertrichosis is most excessive on the face, ears and shoulders. Her fine silky hair is of the vellus, not the lanugo type. The syndrome features are characterized, referring to nine further published case reports. It is distinguished from other types of congenital hypertrichoses, which have been described in the literature under different synonyms. To avoid confusion in the terminology, we propose to name this type of hypertrichosis Ambras syndrome in reference to the first documented family with congenital hypertrichosis universalis in the 16th century.

[Screening for psychogenic factors in functional dysphonia].

Psychosocial influences are now increasingly accepted to be important factors in the etiology of many diseases. The application of these findings is often not possible in clinical routine, especially because there is a lack of practical instruments for the diagnoses of psychogenic disorders. Based on previous investigations determining relevant psychogenic factors causing functional dysphonia, the goal of this study was to develop an easy-to-manage screening sheet for the estimation of psychogenic factors in the etiology of functional dysphonia. With this approach, physicians not trained in psychology are able to prove a suspect psychogenic disorder. Patients are forced to reflect on their disease by completing the sheet and thus may limit resistance to recognizing the true nature of their disturbance. Patients with pathological results can be sent effectively for further investigations and specialized therapy.

[Occupational reintegration and subjective quality of life after inpatient rehabilitation in myocardial infarct].

The present study was carried out on an unselected population of 107 myocardial infarction patients admitted for immediate post-acute rehabilitation. The patients were assessed (a) at discharge from multidimensional in-patient treatment, and (b) after a period of 12 months back home. Our aim had been to trace those psychological and general cardiological parameters that contribute to a patient's reintegration into work life as well as improved quality of life. It was found that patients with good physical coping did not suffer a reinfarction in the subsequent twelve months period. Younger or middle-age, male patients significantly more often returned to work. Remarkably, no correlation was found between good psychological coping with the infarction and return to work. As had been expected, a higher quality of life was achieved by those patients whose psychological status is only slightly impaired, who do not attach too great importance to their job, and who reported good physical coping as well as social reintegration and successful adjustment in their lifestyle.

[Psychogenic aspects of functional dysphonia].

In order to estimate the significance of the psychogenic factor in the etiology of functional voice disorders on the basis of relevant psychological dimensions we compared patients with functional dysphonia, aphonia and a normal control group by means of an empirical-psychological test procedure. Psychogenic conditions proved to be a major factor in the etiology of aphonia, whereas such an interrelation turned out only partially in patients with functional dysphonia. The conclusion can be that psychogenic aspects are not necessarily of exceeding importance in the etiology of functional dysphonia. If no differentiation is made between the two kinds of voice disorders an overestimation of psychogenic influence might occur.

Controlled trial of hyposensitisation in children with food-induced hyperkinetic syndrome.

Food intolerance seems to be an important cause of the hyperkinetic syndrome, but restricted diets are expensive, socially disruptive, and often nutritionally inadequate. Enzyme-potentiated desensitisation (EPD) may overcome some of these difficulties. EPD was tested in a double-blind placebo-controlled trial among 40 children with food-induced hyperkinetic behaviour disorder. A total of 185 children with established hyperkinetic syndrome underwent oligoantigenic dietary treatment for four weeks. 116 whose behaviour responded had provoking foods identified by sequential reintroduction. Foods that reproducibly provoked overactivity were avoided. 40 patients who were then invited to take part in the hyposensitisation trial were randomly assigned to treated and control groups. Treated patients received three doses of EPD (beta-glucuronidase and small quantities of food antigens) intradermally at two-monthly intervals. Controls received buffer only. Thereafter, patients were allowed to eat known provoking foods. Of 20 patients who received active treatment, 16 became tolerant towards provoking foods compared with 4 of 20 who received placebo (p less than 0.001). Our results show that EPD permits children with food-induced hyperkinetic syndrome to eat foods that had previously been identified as responsible for their symptoms. These results also support the notion that food allergy is a possible mechanism of the hyperkinetic syndrome.

[MR angiography in neuropediatric problems: the technic and the clinical results].

31 children aged between 6 months and 14 years with various neurovascular abnormalities were examined by MR angiography in a prospective study. In order to test the sensitivity and specificity of the MR results, these were compared in 8 cases with the findings of digital subtraction angiography or conventional angiography. In 24 patients arterial MR angiography was performed; in 17 these showed anomalies or abnormalities of the intracranial vascular system. In 7 patients venous MR angiography was performed and demonstrated the presence of sinus thrombosis in 6 of these. Comparison between MR angiography and DSA showed agreement in 7 cases; in one patient the degree of a stenosis was exaggerated by MR. The results of this study indicate that arterial and venous MR is of great value as an additional study in paediatric neurovascular problems.

Effect of diet treatment on enuresis in children with migraine or hyperkinetic behavior.

Twenty-one children with migraine and/or hyperkinetic behavior disorder which was successfully treated with an oligoantigenic (few-foods) diet also suffered from nocturnal and/or diurnal enuresis. On diet, the enuresis stopped in 12 of these children and improved in an additional four. Identification of provoking foods was by sequential reintroduction of the foods that were avoided on the oligoantigenic diet. In eight of the 12 children who recovered on the oligoantigenic diet and in the four who improved, reintroduction of one or more foods provoked a reproducible relapse of the enuresis. Nine children were subjected to a placebo-controlled, double-blind reintroduction of provoking foods. Six children relapsed during testing with incriminated foods; none reacted to placebo. Enuresis in food-induced migraine and/or behavior disorder seems to respond, in some patients, to avoidance of provoking foods.

[From psychobiological stress research to neuropsychoimmunology].

Psychoimmunology--a neologism--is a short term for the growing evidence that there are multiple interactions between central nervous system, vegetative nervous system, endocrinological system and immunological system. In the history of research the psychobiological stress concept of Hans Selye has been the most powerful basis for the ongoing empirical work in investigating aspects of systemic processes of psychoimmunology. Nowadays there is no doubt that the immunological system must not be seen as an independently functioning organic system. Simple causal links (like stressor-strain-relations) are not useful in explaining the complex interactions that we observe when biological phenomena simultaneously are taken into consideration. Especially with psychological stressors--the most frequent kind of troubles in our contemporary way of life--we have to stress the importance of the individual's evaluation of potentially stressful situations. In a wide variety a stressor becomes the potential of provoking strain because of my own appraisal. This multiple-step-process of appraisal is dependent on my coping abilities and coping strategies regarding an objectively or subjectively dangerous situation. Although there are common genetic and predispositional factors influencing the stress reaction we have to focus more on psychological phenomena, especially the possibility of conditioning stress reactions. As we know by empirical data some reactions of the immunological system are also influenced by learning processes like classical conditioning procedures. This knowledge may be used for a better understanding of a lot of chemical findings.

[Psychosexual behavior as a risk factor for cervix cancer].

Seventy-two patients with cervical cancer (n = 24), ovarian cancer (n = 24), or no disease (n = 24) were questioned about sexual activity and personal relationships. The interviews were conducted in a semistandardized fashion according to the Tübingen Sexual Therapy Scale. Patients with invasive cervical cancer did not have a larger number of sexual partners, an earlier age of first intercourse, greater sexual activity or less stable relationships than patients with ovarian cancer of healthy women. There was no statistical difference among the groups with regard to the relationships. However, patients with cervical cancer reported greater degrees of satisfaction with both sexuality and relationships.

[Importance of locus of control in the disease adjustment process in women with genital cancer].

This study deals with psychological aspects of coping observed in gynecological cancer patients--on the one hand with behavioral, emotional and cognitive reactions to the disease and on the other hand with the efficiency value of these mechanisms in handling the disease. As one important aspect, the attitude towards control of the disease should be correlated with the coping process. Accordingly, the results demonstrate a correlation between high self-responsibility and positive appraisal of the disease: Women who believe they are in control of the disease try harder to fight the disease and interpret their own situation more positively than women with a fatalistic attitude and this enables them to make more sense of the disease; they experience more positive changes in their own lifestyle due to the disease. Furthermore, women who ascribe responsibility for the course of the disease to other interpret their situation in a more positive light. They actively search for social support and obviously experience this trust in the power of other people as a helpful measure. A fatalistic attitude to the disease is combined with rumination and social withdrawal.

[Precancerous stages of cervix cancer (CIN)--attitude and coping with the disease].

The subjective reactions and the mental state of 50 patients with a diagnosis CIN I, II and III were studied. The data were gathered after gynaecological follow-up examinations in semistructured interviews and with a list of complaints (B-L) according to Zerssen (1975). Patients with CIN I or II were examined and interviewed 3 months after diagnosis, patients with persistent CIN I or II were studied after 6 months, and patients with CIN III before conization. Patients with CIN reacted to the diagnosis in a similar manner to patients confronted with the diagnosis of cancer. Effective coping with the disease depended on adequate medical information being received by the women. The patients who were satisfied with the medical information had a more positive approach to the course of the disease and found it less life-threatening than those patients who were dissatisfied with the information received. The results of this study indicate that subjective experience of the disease is largely independent of its objective severity.

[Induced abortion--a critical life change event].

In the present survey the psychic and physical mastering of abortion is considered to be a subject of the research of a "critical life-event". Special interest was devoted to the consequences of the process of mastering before and after abortion on the mental and physical condition of the examined human females (n = 39). The diagnosis showed that especially before abortion in a time of a serious state of inner conflict, interference concerning the physical condition and psyche of women arise, while actually after abortion the mood is more euphoric. The inconvenience increased in dependence on experience of social welfare and support. Three months after abortion specific complaints such as pains in the lower abdomen, bleeding and sudden urge to cry occur. The administration of contraception after abortion is becoming significantly safer.
