[Premature rupture of fetal membranes in the midtrimester (II): Antibiotic treatment in a case of intraamniotic infection prior to fetal viability].

A case of a 33 year old woman G7P6A1, with preterm premature rupture of membranes at 20 weeks of gestation and asymptomatic intramniotic infection is reported. At admission, the presence of Klebsiella oxytoca in amniotic fluid was demonstrated by amniocentesis. Parenteral antibiotics were administered with a following demonstration of aseptic fluid. The patient delivered at 29 1/2 weeks of pregnancy. A newborn weighing 1520 with an Apgar score of 8-8 was obtained. The mother had a normal puerperal period without any evidence of infectious morbidity. A clinical sepsis was demonstrated in the newborn with appropriate response to antibiotic therapy. No further complications were observed.

[A psycho-spiritual model for the physician-patient relationship].

The recent advances in Medicine, specially during the last years, have been important in the fight against disease. However, the dimension of patients as human beings has been forgotten. In this article, some ideas about the physician-patient relationship are exposed. Some case histories are depicted to clarify this issue. The importance of an emotional and spiritual support of patients and relatives in situations of emotional breakdown in emergency rooms, terminal disease and cardiopulmonary resuscitation in emphasized.

Humans

Neural pathway mediating somatic evoked responses in the caudate nucleus of cats.

The course of somatic afferents to caudate nucleus (CN) was investigated. In curaized cats discrete lesions in intralaminar nuclei, n. centralis medialis (NCM) affect evoked response (ER) in CN to radial nerve stimulation. NCM lesions totally abolish these somatic responses, while after subthalamic lesions ER appear only when a high stimulus intensity is used. Lesions in the specific relay nuclei, ventralis posterior lateralis (VPL) have no effect on somatic ER in CN. Barbiturates (10 mg/kg) abolish somatic ER in CN. Stimulation of the ponto-mesencephalic reticular formation (RF) in a region where visual and somatic responses are recorded shows lowest threshold values for eliciting evoked responses in NCM and CN. The potentials in CN to RF stimulation are restricted to this nucleus and disappear in the internal capsule.
