Diagnosis, Differential

Catchers in the rye.

There is a great need for more research in child and adolescent psychiatry, and this must be clinically, as well as laboratory, based. There are, however, much data already available on alcohol and handguns that, if used, could substantially decrease morbidity and mortality rates for adolescents.

Adolescent

Child psychiatry program directors' ratings of residency experiences.

Child and adolescent psychiatry program directors' (PDs) were asked to complete survey forms to rate the importance of required residency experiences and express views on length of training. There was a 93% return rate of questionnaires sent to the 129 academic programs represented in the Society of Professors of Child and Adolescent Psychiatry. The majority of PDs favor the status quo and do not want to add or subtract from the present requirements. Experiences most favored were those with older children, adolescents, and parents, and those in the use of psychopharmacology. Least favored experiences were with cognitive therapy, research, psychological testing, and teaching. There was little interest in eliminating the internship year or residency time with adults.

Adolescent Psychiatry

Psychodynamics and medication.

In many training programs and for many practitioners there is conflict between the importance and use of psychotherapy and pharmacotherapy. Some of the historical, political, interpersonal, and intrapersonal roots of the problem are summarized in the hope that understanding and self-scrutiny will help break down the splitting that remains all too common.

Child

A prospective study of development of children with sex chromosome anomalies - New Haven study III. The middle years.

This is the third report of a prospective study of children with sex chromosome anomalies identified at birth in New Haven, Connecticut. Previous reports in 1974 [1] and 1979 [2] summarized data from the first 2 1/2 years and the first 9 years of the children's lives, respectively. The present report will focus on progress and evaluations in the years 9-13, concluding with an Appendix of Case Summaries which provide an overview of 6 of the cases. Accumulation of data from such children is increasingly important to assist in genetic counseling. Because of the rarity of sex chromosome aneuploidy (about 1 in 400 newborns), no one center sees enough of these children to feel confident about the validity of results. However, the periodic meeting, sharing of data, and joint publication of results by investigators in several centers, made possible by the March of Dimes provide large enough numbers of children with each karyotype for valid conclusions about range of expected growth and development through the preadolescent years. Therefore, this report is published together with those from other centers, as in the 1979 report [2].

Child

Behavior modification protocols on a pediatric adolescent unit.

The use of operant conditioning protocols for the management of behavioral disturbances of hospitalized adolescents with serious chronic medical illnesses is discussed in this paper. The use of the protocol from the point of view of changing hospital staff interaction is emphasized. Three cases are used to illustrate the process by which the protocol is created and its therapeutic effect on the patient and staff.
