The regional lymph node question. To treat or not to treat: the pros and cons.

The question of whether or not to extend treatment to the regional lymph nodes when treating malignant disease remains unanswered. The clinician must individually approaise the pros and cons and derive a satisfactory treatment plan for each patient. Six reasons which are generally stated as indications for regional lymphadenectomy and seven potential arguments against lymphadenectomy are presented.

Alveolar-arterial oxygen gradient in the burn patient.

1)A presumptive diagnosis of inhalation injury by the occurrence of a "closed-space" burn and the presence of facial burns, singed nasal vibrissae, ronchi and wheezes as clinical criteria was often in error in our study group. 2) A-a O2 gradient and change in A-a gradient in our group of patients was of value in predicting the development of pulmonary dysfunction.

Compression neuropathy following brachial arterial puncture in anticoagulated patients.

1) Seven patients were seen and treated over a 1-year span with forearm hematoma and median nerve neuropathy following brachial artery puncture. All seven were anticoagulated. 2) A careful examination established the diagnosis. Prompt operative intervention is recommended. 3) The brachial artery should be avoided as a source for arterial blood gas sampling in the anticoagulated patient.

Long-term results of the island flap palatal pushback.

We analyzed all 104 island flap pushback palatal repairs done through our Facial Rehabilitation Clinic in the period from 1965 to 1971. The results were compared to those in a group of 109 standard pushback repairs. The island flap group had a higher incidence of operative complications, of velopharyngeal insufficiency, and of secondary procedures to correct the latter. (The differences between the two groups were not statistically significant, however.) In this group of patients the island flap repair offered no particular advantage over the V-Y or the Dorrance pushback; in fact, it may have been deleterious. A hypothetical explanation for these results is offered, based on possible continuing osteogenesis by the transplanted mucoperiosteum, to produce an inflexible and poorly functioning velum.

Developmental patterns of acid hydrolases during differentiation of fetal mouse skin.

The activities of acid phosphatase and beta-glucuronidase were assayed in fetal and neonatal mouse epidermis by microanalytical methods. The level of acid phosphatase activity was low in the epidermis on Day 15 of gestation. Acid phosphatase activity increased 10-fold between Day 15 of gestation and neonatalhood. On the other hand, beta-glucuronidase activity was high on Days 14 and 15 of gestation and low after Day 17 of gestation. The relative ratio of acid phosphatase to beta-glucuronidase activity appeared to represent a marker for the degree of differentiation in whole epidermis.

Peroxide induced chemiluminescence in an in vitro proline hydroxylation system.

This communication describes a hydrogen peroxide (HOOH) induced chemiluminescence (CL) in an in vitro aromatic (proline) hydroxylation system. The reactive components of the system are ascorbic (AA), ethylene diamine tetraacetic disodium salt (EDTA), ferrous sulfate, and HOOH. The CL is (1) nearly dissipated within three minutes, (2) enhanced and/or sustained by proline and polylysine to a greater degree than by alanine, (3) partially inhibited by a,a' dipyridyl, EDTA, and ethanol, (4) most dependent upon the presence of Fe2+, AA, and HOOH. The in vitro proline hydroxylation system is more effective than ground state oxygen in terms of the CL produced and the percent of hydroxyproline formed.

Enzyme activities in the epidermis of zinc-deficient rats.

Fifteen enzymes participating in epidermal energy metabolism in zinc-deficient and -supplemented rats were assayed utilizing fluorometric microchemical techniques. In the zinc-deficient group, the activities of six enzymes catalyzing glycolysis decreased by 30 to 50% of the control; the most dramatic decreases were found in phosphofructokinase and glyceraldehyde-3-phosphate dehydrogenase. Zinc deficiency caused a 31% decrease in the activity of glucose-6-phosphate dehydrogenase, a 63% decrease in fumarate hydratase, a 46% decrease in glutamate dehydrogenase, and a 30 to 40% decrease in aminotransferases.

Where are we going?

The foregoing study attempts to document the course of the American Society of Plastic and Reconstructive Surgeons, Inc. during the period 1964-1973, inclusive. A special attempt has been made to delineate the current trends. Trends, when recognized, may be enhanced, modified, or prevented-as deemed appropriate by the membership.

Location of the levator veli palatini insertion following levator retropositioning, palatal pushback, and pharyngeal flap procedures.

A long-term cineradiographic follow-up study of twenty patients with velo-pharyngeal incompetence fails to demonstrate predictable retrodisplacement of the levator insertion following combined levator retropositioning, pushback, and pharyngeal flap procedures. Simple levator retropositioning gave posterior displacement in the two patients evaluated. Patients with normal or posterior levator insertions pre-operatively all demonstrated post-operative anterior displacement following pharyngeal flap procedures, either alone or in combination with pushback. Anterior levator displacement may be the result of scar contraction or division of the levator sling (during insetting of a pharyngeal flap). Pre-operative and post-operative speech evaluation demonstrated substantial improvement in all except 3 patients; 2 of the poor speech results were patients with demonstrated levator retrodisplacement on post-operative cineradiography.
