Prognostic factors and management of axillary node negative breast cancer.

Axillary node negative breast cancer has become the most frequently occurring stage of breast cancer among American women. Even in spite of early diagnosis and optimal local management, 20-30 percent of all women diagnosed with Stage I breast cancer will recur and succumb to the disease. Standard management approaches to treatment as well as prognostic determinants are in a state of rapid evolution. Entry of patients to appropriate clinical trials is highly recommended. However, if this option is not available to the physician or is not desired by the patient, further therapeutic approaches must be guided by the prognostic variables. For patients with small (less than 1 cm) tumors, low nuclear grade and/or low proliferation rate with diploid DNA characteristics, no further therapy may be a reasonable option. However, patients with tumors equal or greater than 2 cm, high or poor nuclear grade, with a high proliferation rate and aneuploid tumor should be considered candidates for adjuvant chemotherapy if the tumor is ER negative, or treatment with tamoxifen if the tumor is ER positive.

A computer data base for information on cancer treatment.

The Physician Data Query (PDQ) system is a clinically oriented computer data base developed to make recent information on cancer treatment widely available to the medical community. It represents an effort by the National Cancer Institute to promote diffusion of information about the treatment of cancer throughout the country, facilitate access to clinical trials, and accelerate the practical application of advances in research. The computer system provides information about state-of-the-art cancer treatment, which is updated monthly by an editorial board. It also includes a file of active cancer-research protocols and a directory of physicians and organizations providing cancer care to which physicians can gain access by geographic location as well as other features. PDQ was designed for physicians who may not be familiar with computers, to permit them to search for and display information without learning a specialized search language. PDQ uses a computer mainframe, which allows a large amount of data to be stored and made available to physicians rapidly and accurately. Transmission of information about cancer over commercial telecommunication networks gives health professionals access to PDQ by means of a computer terminal and local telephone lines.

Multimodal treatment of primary breast carcinoma: analysis of accomplishments and problem areas.

The management of primary breast carcinoma has undergone a successful change during the decade of the 1970s. Chemotherapy administered after mastectomy in patients with involved axillary nodes has not only prolonged disease-free survival in certain patient groups but has lengthened their over-all survival in comparison to patients treated with mastectomy alone. The concepts for use of chemotherapy after surgery, established in murine tumor models, have been extended to human subjects with breast cancer and proved of value. It remains to be determined which drug regimens have the best therapeutic index and which are most useful in specific subgroups of patients. Other breast carcinoma patients besides those with involved axillary nodes may also benefit from this surgery-plus-chemotherapy mode of treatment. We review the past, present and future clinical trials of chemotherapy after mastectomy and discuss the various aspects which bear on their successes and failures.

The evolution of primary multimodality treatment in resectable breast cancer.

Based on both laboratory and clinical data, two major prospective randomized clinical trials were initiated in 1972 and 1973 by the NSABP and the Instituto Nazionale Tumori, of Milano, Italy, under the sponsorship of the National Cancer Institute, to test the value of administering chemotherapy to patients with Stage II breast cancer in the postoperative period. With a follow-up time of nearly five years, both studies continue to demonstrate a significantly prolonged disease-free interval for treated premenopausal women. The Milano trial also indicates an increase in survival from 74% to 90% in the treated group of premenopausal women. A recent analysis of the postmenopausal women treated in the Milano study show that those treated patients who received 75% of the calculated drug dose have significantly prolonged disease-free intervals. These positive results from these trials and byproducts of other basic and clinical research, particularly the value of the estrogen receptor as a prognostic factor of disease-free interval survival and possible treatment selector, have stimulated the scientific community to undertake additional adjuvant therapy studies. The design and rationale for these subsequent studies are discussed. Preliminary indications exist that these trials will lead to a reduction in the national mortality and the overall economic impact from this disease will be lessened within the next five years. Based on the positive results of these studies, rationale currently exists for designing clinical trials that have the potential to extend similar benefit to the 60,000 patients with Stage I disease.

Role of antitumor antibiotics in current oncologic practice.

The antitumor antibiotics have thus made a major impact on oncologic practice. The continued search for productive strains of these organisms should be encouraged. In addition, the activity and toxicity spectrum suggests the need for vigorous analog development. An active anthracycline devoid of cardiotoxicity; a bleomycin with no effect on pulmonary tissue; an analog of streptozotocin devoid of nephrotoxicity; these would be advances of inestimable benefit to the cancer patient of the future.

VM 26 and VP 16-213: a comparative analysis.

VM 26 and VP 16-213 are epipodophyllotoxin analogs. This paper presents a comparative analysis of their experimental and clinical features but fails to disclose any significant difference between these drugs. Similar studies with other classes of compounds might help to define some rationale for the development of analogous cytotoxic agents.

Cyberpharmacies and the role of the US Food And Drug Administration.

The sale of consumer products over the Internet has grown rapidly, including the sale of drugs. While the growth in online drug sales by reputable pharmacies is a trend that may provide benefits to consumers, online drug sales also present risks to purchasers and some unique challenges to regulators, law enforcement officials and policy makers. The Food and Drug Administration (FDA or the Agency) is concerned about the public health implications of Internet drug sales, and we are responding to these concerns as part of our overall goal of developing and implementing risk-based strategies to protect public health and safety. Although other products regulated by the Agency, such as medical devices, medical test products, foods, dietary supplements and animal drugs also are sold online, this paper focuses on online drug sales. We discuss the advantages and risks of online drug sales, outline FDA's authority and enforcement activities in this area, and describe new initiatives we are taking to better respond to the regulatory challenges we face.
