Analysis of the mechanism of action of calcium-induced exocrine pancreatic secretory changes in the dog.

In conscious dogs, provided with gastric and duodenal fistulas (Thomas cannula), the effect of an acute i.v. calcium infusion (10 mg./kg./hr.) on the plateau levels of secretin-induced (GIH, 1 CU./kg./hr.) pancreatic juice parameters were analyzed without or with previous cholingergic blockade by a continuous atropine perfusion (1.5 mg./hr.). In the former condition, i.v. calcium raised all pancreatic juice parameters, mainly protein concentration (148%) and output (173%). During atropine blockade, i.v. calcium changed neither flow rate nor bicarbonate output of secretin-induced pancreatic secretion. On the other hand, i.v. calcium exerted a reduced but still evident effect on protein concentration (85%) and output (40%). Intravenous calcium enhances secretin-induced pancreatic secretion plateau values through two mechanisms, mainly by raising the cholinergic tone of the whole "pancreon" and, partially, through a direct stimulus-secretion coupling effect on the acinar cells. Acute hypercalcemia induces the formation of protein plugs in the pancreatic secretion. They are excreted in the course of many days in both basal and secretin-stimulated pancreatic juice.

Simultaneous changes in pancreatic and gastric secretion induced by acute intravenous ethanol infusion. Effect of atropine and reserpine.

The simultaneous effects of acute i.v. ethanol administration (1.3 gm./kg.) on pancreatic and gastric acid secretion was studied on dogs provided with chronic pancreatic and gastric fistulas (Thomas cannula) and subjected to a continuous i.v. injection of GIH secretin (0.5 CU./kg./hr.) and gastrin (Eurorga hog gastrin I-II, 6 gamma/kg./hr.). Acute i.v. ethanol inhibits the pancreatic secretion of protein (concentration and output) and stimulates gastric acid secretion. Experiments were repeated: 1. Superimposing an atropine infusion (1.0 mg./hr.) on the continuous hormonal perfusion. 2. After reserpine administration for 48 hours (0.10 mg./kg./24 hr.) Atropine abolished the ethanol-mediated inhibition of pancreatic protein secretion but did not prevent the alcohol-mediated gastric acid stimulation. Reserpine did not change the ethanol-mediated pancreatic inhibition. It is assumed that in nonalcoholic dogs, i.v. ethanol inhibits pancreatic secretion by an intermediate nervous mechanism and enhances gastric acid secretion by acting directly on the oxyntic cells. Reserpine induces a high plateau level of HCl secretion which obscures the ethanol-mediated excitatory influences on the oxyntic cells.

Comparative analysis of plasma lipoprotein components assessed by ultracentrifugation in primary biliary cirrhosis and chronic hepatitis.

UNLABELLED: The evaluation of lipid and lipoprotein profiles in liver diseases has important cognitive aspects and provides practical information contributing to the diagnosis of liver pathology. There are few studies of this problem using the ultracentrifugation method. AIM OF THE STUDY: A comparison of lipid profile (obtained by plasma ultracentrifugation) in patients with primary biliary cirrhosis (PBC) and chronic hepatitis (CH). MATERIAL AND METHODS: 103 percutaneous liver biopsies were routinely performed from 1994 to 1997. Blood samples were taken from all the patients at the time of biopsy for further evaluation of lipid profile. 15 patients with PBC and 15 patients with CH (of HBV or/and HCV etiology) were studied. RESULTS: In patients with CH mean total, esterified and free cholesterol levels (166 mg%, 117.6 mg% and 48.1 mg%, respectively) were significantly lower (p < 0.002; p < 0.004; p < 0.006, respectively) than in patients with PBC (237.5 mg%, 165.7 mg% and 71.8 mg%, respectively). The phospholipid concentration in sera of patients with PBC were significantly higher (271.1 mg%, p < 0.0004) than in patients with CH (187.6 mg%), whereas apolipoprotein B and apoAII were significantly lower. Total, esterified and free cholesterol levels in LDL fraction were significantly higher in patients with PBC (175.3 mg%, p < 0.007; 117.9 mg%, p < 0.02; 57.6 mg%, p < 0.01, respectively) than in patients with CH (113.7 mg%, 78.7 mg% and 35 mg%, respectively). The concentration of phospholipids in LDL fraction in patients with PBC was significantly higher (166.8 mg%; p < 0.003) in comparison with patients with CH (96.3 mg%). The differences in other lipoprotein fractions (VLDL and HDL) were not statistically significant. CONCLUSIONS: High levels of total, free and esterified cholesterol as well as phospholipids, apolipoprotein B and AII were observed by us in patients with PBC in comparison with patients suffering from CH. The increase of cholesterol (total, esterified and free) and the phospholipid concentration in serum, are a manifestation of their higher concentration in LDL fraction.

Chronic pancreatitis in Warsaw.

The purpose of the study was an approximation of the incidence and prevalence of chronic pancreatitis (CP) in Warsaw (Poland) and the dietary habits before the onset of the disease. The number of CP cases treated in the District Hospital and in out-patients department was calculated. The incidence and prevalence of CP were analysed in ratio to 100,000 of the population of the Warsaw Bródno district. In the years 1982-1987 a group of 64 patients with CP were observed. The incidence and prevalence in 1987 were 5.0 and 17.0, respectively. Alcohol is the cause of about 73% of CP cases and its mean intake unit the development of the disease was 68 ml daily for about 16 years. Patients with CP consumed more protein and fats before the onset of CP in comparison with the general population in Poland. The intake of protein and fats before diagnosis of CP was 113 g/day and 175 g/day, respectively. The mean consumption of protein and fats in Poland in 1983 was 89 g/day and 117 g/day, respectively.

[Level of amino acids in blood plasma as a test for exocrine pancreatic function].

A well known in physiology fact is that stimulation with enterohormones (secretin, cholecystokinin) causes a steep increase in the synthesis of pancreatic enzymes, and this might affect the plasma level of amino acids. In view of this, this level was studied in healthy subjects and patients with chronic pancreatitis. Hormonal stimulation was observed to cause in healthy subjects a significant rapid fall of the levels of all amino acids, which was greatest in the 20th minute. A less evident fall of the amino acid level was observed in chronic pancreatitis. A high correlation was noted (r = 0.9) between the value of amino acid fall in plasma and the degree of failure of the exocrine pancreatic function measured with the NBT-PABA test. All results are an encouraging indication that plasma amino acid level fall may be used for the assessment of the pancreatic exocrine potential. In the analysis of individual amino acids the most significant fall was noted of methionine, serine, valine, isoleucine, glutamine and tyrosine.

[Clinical evaluation of loperamide (Imodium)].

The clinical usefulness of loperamide (Imodium) was studied. The preparation was given to 35 patients with acute diarrhoea and 34 with chronic diarrhoea. A good effect was obtained in symptomatic treatment of acute and chronic diarrhoea. The best effects were noted in the treatment of diarrhoea due to functional disturbances.

[Campylobacter pylori in diseases of the stomach and duodenum].

Campylobacter pylori is an organism living under mucus layer in the stomach and on the places of gastric mucosa metaplasia. It is known to be responsible for the development of gastritis, probably also it participates in the pathogenesis of dyspepsia without ulcer, while its role in the development of gastric and duodenal ulcer has not yet been explained completely. The authors present a characteristics of the organism and the methods of its detection. A number of investigations are discussed which demonstrate its clinical importance. In summary, it is shown that the present state of knowledge on Campylobacter pylori is insufficient for serving as a rationale for antibacterial treatment of peptic ulcer.
