Allergic contact dermatitis to All-trans-retinoic acid; epicutaneous and leukocyte migration inhibition testing.

Two male prison volunteers had severe cutaneous reactions to the initial patch testing of All-trans-retinoic acid containing products being evaluated for irritancy. Muliple patch testing procedures performed throughout one year, along with leukocyte migration inhibition studies, speak strongly for delayed hypersensitivity reactions in these subjects. The failure to document a previous exposure in subjects living in a controlled environment suggests that these responses were cross-reactions to a prior sensitizer, perhaps unique to this environment.

Simulation of human reactions under extreme conditions.

Lung mass determination in cooperative subjects is possible by computation from the "flow-volume" loop, if total lung resistance (Raw) is known. The model is applicable to tracking changes in weightlessness. Modifications for improving the result are discussed.

[Pain reduction by trans-articular atlanto-axial screw fixation in patients with chronic polyarthritis].

To reduce the risk of cervical myelopathy, 32 patients suffering from rheumatoid arthritis (RA) and atlanto-axial dislocation (AAD) underwent a suboccipital fusion. All patients were interviewed with a pain questionnaire after a mean postoperative follow-up of 21 months (range 2-46). The results indicate a significant postoperative pain reduction as assessed by a visual numeric analogue scale, as well as a reduction of analgesic consumption.

Circumferential fusion of the lumbar and lumbosacral spine. Comparison of two techniques of anterior spinal fusion.

Thirty-two patients with therapy-resistant low-back pain and justified indication for fusion were divided into two groups: 16 patients were treated with anterior fusion of the lumbosacral spine, with insertion of tricortical bone grafts of the iliac crest, and 16 patients with insertion of a fibular peg. Additional dorsal fusion of the same segments was performed. Comparison of the two techniques of circumferential fusion of the lumbosacral joint reveals comparable clinical results. A marked difference in roentgenographical criteria such as disk height, retrolisthesis, and anterior osteophytes in the adjacent segment between the two techniques was noted at the time of follow-up evaluation after 89 months and motivated the authors to abandon the fibular-peg technique.

[Is transcranial magnetic stimulation a helpful method in diagnosis of cervical myelopathy in patients with chronic polyarthritis?].

Fifty-seven patients with rheumatoid arthritis were examined clinically, radiologically, and by means of transcranial magnetic brain stimulation. With this method central motor conduction times of the corticospinal pathways can be measured. The aim of the study was to compare the different methods to detect a cervical myelopathy and to correlate them. In 90% of patients clinical signs of cervical myelopathy could be detected, although in two-thirds of these, the neurological deficit was minimal. Radiologically, over 50% of patients had an atlanto-axid dislocation (AAD) of more than 5 mm. The central motor latencies measured in the upper extremities were prolonged in 47% of cases. These results show the usefulness of transcranial magnetic brain stimulation as an additional noninvasive investigation for detecting compression of the cervical myelon. This is an additional useful criterion for determining the need for operative intervention.
