Meta-analyzed heart rate variability, exposure to geomagnetic storms, and the risk of ischemic heart disease.

The aim was to examine how heart rate variability (HRV) relates to the risk of ischemic heart disease (IHD) and may provide a means to assess effects of exposure to geomagnetic storms. In Stockholm, the 24-hour SD of hourly estimates of heart rate (HR) were obtained by Holter monitoring from 50 men who had had an acute myocardial infarction or had angina pectoris and compared to that of 50 clinically healthy men of similar age. In Tokyo, the HR 121 normotensives and 176 treated hypertensives was monitored. The incidence of IHD was recorded prospectively for 6 years. These results are aligned with those of a retrospective analysis of archived data on all crews of the Soyuz spacecraft for 1990-1994 focused on ECG from cosmonauts (47 male and 2 female) at times corresponding to geomagnetic storms. The results clearly indicate a decrease in HRV in association with IHD (20.5%, p=0.002 in Stockholm, 20.0%, p=0.04 in Tokyo). By comparison, the about 30% decrease (p=0.041) in rms SD of HR in cosmonauts studied during a geomagnetic storm as compared to cosmonauts monitored on quiet days adds supportive evidence to the proposition that exposure to geomagnetic disturbances increases cardiovascular disease risk.

Differential chemosensitivity of leukemic cells in the myeloid and lymphoid phases of stem cell leukemia (a case report).

A five-year-old girl, initially diagnosed as having acute lymphoblastic leukemia (ALL; FAB-L1) relapsed with ALL 4 months after completion of chemotherapy (BFM 83). The initial ALL presentation and subsequent ALL relapse were analyzed using conventional morphology, cytochemistry, cytogenetics and immunophenotyping. The results were consistent with a diagnosis of B-lymphocyte precursor ALL. Bone marrow leukemic cells revealed a 46, XX karyotype at diagnosis and a 46, XX, del(7) (q22; qter) when the girl first relapsed. The case was managed with a BFM REZ-ALL 90 protocol. Upon completion of the first cycle of the protocol, severe myelosuppression developed. This was treated with GM-CSF. Three days later, however, GM-CSF was stopped because the WBC reached 1.1 x 10(9) per liter with 60% of blasts in peripheral blood. Laboratory characteristics were typical of AML. Cytogenetic analysis revealed 46, XX, del(7) (q22; qter) karyotype as before. The bcr-abl fusion gene was not detected. Myeloid blasts were placed in a culture and maintained at 37 degrees C and 7.5% CO2 for two weeks. During this period, formation of hemopoietic colonies was observed and subsequently analyzed using histology and electron microscopy. This showed that the colonies consisted of differentiating erythroid, megakaryocytic and myeloid cells. Further, the chemosensitivity of leukemic cells was examined in both "lymphoid" and "myeloid" relapse instances. While the "lymphoid" phenotype was characterized by good sensitivity to corticosteroids, a typical feature of the "myeloid" phenotype was a high resistance to corticosteroids with marginally increased sensitivity to ARA-C.

[Clinical characteristics of mycosis fungoides and Sézary syndrome].

Mycosis fungoides and Sézary syndrome are typical representatives of malignant lymphomas of the T-series affecting the skin. Both affections have a varied clinical picture, a different spectrum of histological and laboratory findings and lack a uniform prognosis. In the submitted review the authors discuss in more detail all mentioned aspects of the disease and draw attention to new therapeutic possibilities.

[Antineoplastic effects of natural substances].

The present paper surveys natural substances with antitumour activity, particularly primary metabolites--polysaccharides, lectins and lipids tested in vitro or on laboratory animals. The presented data cover the period of 1990-1995.

[24-hour ambulatory monitoring of blood pressure in patients with essential hypertension: the effectiveness of enalapril therapy].

The aim of the present paper was to study 24-h blood pressure profile in twenty patients with essential hypertension (mild to moderate) before and after treatment with ACE inhibitor enalapril in one day dose in the morning (15.6 +/- 6.1 mg). 24-h ambulatory blood pressure monitoring was examined after 14 days of placebo administration and after one month of enalapril therapy. Enalapril treatment decreased systolic blood pressure of 11.3 mmHg during daytime and of 9.4 mmHg at night, diastolic blood pressure of 6.1 mmHg during daytime and 5.8 mmHg at night, all differences were significant in comparison to placebo (p < 0.01). The through-peak ratios were 63.2% for systolic blood pressure and 54.0% for diastolic blood pressure. We can conclude that enalapril treatment in one day morning dose is efficient to decrease blood pressure during 24-h span without altering the diurnal profile.

[Natural substances with antioxidant activity].

The review summarizes information about natural substances of phenolic nature (flavonoids, hydroxycinnamic acids, tannins), in which antioxidant effects and ability to quench free radicals have been demonstrated. A number of substances seem to be perspective in the treatment of cardiovascular diseases, cancer and liver diseases.

Fulminant hepatitis caused by a hepatitis B virus infection in the patients with haematologic malignancies. Report of 5 cases.

We describe 5 cases of fulminant hepatitis caused by the HBV infection in patients with haematological diseases, mostly malignancies (ALL, lymphoma, aplastic anemia, AML) following intensive chemotherapy. Infection was confirmed by serological examination (HBsAg positivity) and by electron microscopy (viral particles). After termination of chemotherapy fulminant hepatitis developed with hepatic failure and very high levels of AST and ALT. Autopsy revealed massive necrosis without signs of regeneration. We suggest that immunosuppressive therapy increases the risk of severe infection of hepatocytes with HBV and subsequent withdrawal of chemotherapy causes "immunological rebound" leading to massive necrosis.

[The history and present status of biotechnology in the Czech Republic].

The paper outlines the historical milestones in the development of biotechnology in the Czech Republic in the fields of research, education and manufacture. The section dealing with the present state describes fundamental research carried out in the institutes of the Academy of Sciences of the Czech Republic and the universities and technical colleges, the institutes of applied research subsidized by the pertinent ministries as well as privatized institutes. The numbers of institutes and researchers before and after the political changes in 1989 are compared and the grant agencies for support of science and research are listed. The section dealing with biotechnological industry characterizes the firms manufacturing the products and devices for pharmacy, agriculture, food industry and ecology.

[Congenital dyserythropoietic type II anemia complicated by extramedullary hematopoiesis in the posterior mediastinum].

In a 48-year-old woman investigated on account of congenital dyserythropoietic anaemia type II (HEMPAS) on the X-ray of the chest a polycystic sharply defined shadow in the posterior mediastinum was detected and extramedullary haematopoiesis was suspected. The diagnosis was confirmed cytologically. According to the authors' knowledge this is the second case of this rare complication in CDA-II in the world and the second case of EMH in the mediastinum in this country. In the discussion the authors summarize the diagnostics of congenital dyserythropoietic anaemias. They discuss also the pathogenesis of extramedullary haematopoiesis, diagnosis and differential diagnosis of EMH in the posterior mediastinum, possible complications and treatment.

[Natural substances causing allergies and dermatitis].

On the basis of the literature search from the journal Chemical Abstracts (1988-1992) the present paper processes the data about 142 plants from 49 families which are the causes of the development of contact dermatitides. The paper also lists the allergens responsible for the development of contact dermatitides.

[Congenital type II dyserythropoietic anemia].

Histological and ultrastructural characteristics in congenital dyserythropoetic anaemia are demonstrated on the basis of three observations. The limited diagnostic importance of isolated morphological abnormalities is stressed, with an exception of the sub-surface cistern. Morphological abnormalities vary in the percentage frequency of expression in various patients.

Spondyloepiphyseal dysplasia with nephrotic syndrome (Schimke immunoosseous dysplasia).

The first symptoms of immunooseous dysplasia were growth retardation and myopia. Nephrotic syndrome was diagnosed at the age of 8 years. Skeletal roentgenograms showed spondyloepiphyseal dysplasia. In the renal biopsy there was nodular accumulations of PAS-positive hyaline material at the base of the granular stalks. There was lymphopenia with decreased CD4 and CD8 subpopulations. The condition of the patient gradually worsened until she died unexpectedly at 10 years with clinical symptoms of encephalitis. Autopsy documented cytomegaloviral pneumonia and advanced mesangioproliferative glomerulonephritis. In the spleen there was PAS-positive hyaline material massively infiltrating the walls of the central arterioles of the splenic follicles. There was marked depletion of lymphocytes in the spleen and in lymph nodes. The differential diagnosis of immunooseous dysplasia in the framework of spondyloepiphyseal dysplasia is discussed.

[Löffler's endocarditis (with manifestations of restrictive cardiomyopathy and hypereosinophilia)].

Loeffler's endocarditis in a 47-year-old man was followed 5 years. He died of heart failure. His original diagnosis was eosinophilic leukaemia but cytogenetic investigation failed to show any chromosomal aberration and only mature eosinophilic leukocytes were present in peripheral blood. Relation between heart lesions and eosinophilic leukocyte factors (peroxidase, major basic protein, cationic protein, transforming growth factor beta) were discussed.
