[3-dimensional approach to spinal deformities. Application to the study of the prognosis of pediatric scoliosis].

The authors have utilized a computer for a spatial analysis of deformities of the spine using antero-posterior and lateral radiographs. The posterior limits of the sacral plateau and the centre of each vertebral plateau was demarcated on the antero-posterior radiograph. The posterior margin of each vertebra was demarcated on the lateral radiograph. With this information, the computer can make a picture as if the spine were seen from above and vertebral rotation can be assessed. The vertebrae are of different colours according to their level. Thirty cases have been studied and six types of infantile scoliosis defined - scoliosis with a localised hyper-rotation like a hairpin, scoliosis with rotary dislocation at the junction of two rotational levels, infantile scoliosis with a moderate curve, progressive scoliosis, benign scoliosis and spontaneously regressive scoliosis of the newborn. This study was shown that prognostic features were in accordance with classical assessments.

[Translocation of chromosome 22 in Ewing's sarcoma].

The chromosomal analysis of four fresh Ewing tumours shows a translocation of the band q12 of the chromosome 22 in all the clones observed. This translocation seems to preferentially involve the band q24 of the chromosome 11. These results are in favour of a consistent translocation in Ewing's sarcoma, and are to be compared with the results obtained in other malignancies. A relation between this translocation and the location of the human oncogene c-sis on the chromosome 22 should be considered.

[Dislocated hips in cerebral palsy (author's transl)].

The authors have observed 119 excentrated hips in 81 spastic children. Most of the cases were noted in non walking children under the age of ten. However some excentrated hips were seen in older patients eventually able to walk. The importance of a three dimensional approach of the problem is pointed out. The results of the surgical procedures are analysed. It appeared that it was important to restore a muscle balance before any pelvic or femoral osteotomy. Triple pelvic osteotomy was indicated of obtaining a better orientation of the acetabulum in younger children. Its complications were infrequent. Chiari's osteotomy was indicated in older children. The authors point out the importance of an early diagnosis in cases of lack of muscles balance which can be corrected by minor surgical procedures on soft tissues.

[Congenital anomalies of cranio vertebral junction in children. Indications for surgery (author's transl)].

Based the review of 55 children of whom 19 were treated surgically, the authors have designed 6 anatomo-clinic entities. Each one of these has its own treatment ranging from nothing to decompression of the neural axis with arthrodesis. Relationship between bones and joints and nervous tissues are emphasized with the manner to appreciate them clinically and with X rays or computerised tomography. This has always to be done before any surgical treatment. The goals of this surgery are: --to give perfect stability and acceptable morphologic result of the relation between head and neck, and neural decompression --to cure, and prevent perhaps, progressive myelopathy of which we know the bad prognosis when discovered and treated in adult life.

[Autotransfusion with acute hemodilution in the surgical treatment of scoliosis (author's transl)].

With the goal to reduce the number and the volume of homologous blood transfusions during surgical treatment of scoliosis and spine deformities, the authors have studied the results of 50 cases treated with immediate acute haemodilutional autotransfusion. In 40 per cent of the case neither per nor post operative homologous blood transfusion was necessary. In addition they have seen a surprising result such as a significant lowering of absolute blood loss during surgery compared to a similar series treated without haemodilutional autotransfusion. Physiopathological principles, precise technique used and limitation of the method are described.

[Spine deformities induced by irradiation of Wilm's tumors (author's transl)].

A follow-up study on spine deformities of 59 consecutive Wilm's tumors treated between 1950 and 1963 was conducted. The selected cases had no metastasis at any time and had reached end of growth (16 years old of bone age). Treatment was surgery, chemotherapy and radiationtherapy mostly was 200 KV and partially with CO 60. This study showed 65p. 100 kyphosis greater than 10 degree between T9 and L3 (normal less than 5 degree) and 40p. 100 scoliosis both induced by alteration of vertebral growth and by sclerosis of soft tissues. -- Kyphosis even slight seemed to induce more pain in adult age than scoliosis. -- This work demonstrates also that growth spurt produces increased deformities. Therefore it is necessary to wait the end of growth before the evaluation of the results. Finally the most important for prevention is to irradiate as symmetrically as possible in the 3 planes of the body. The results of surgical treatment are reported and principles of early bracing for prevention are mentioned.
