Aminophylline preferentially inhibits chloroethylclonidine-insensitive alpha-adrenoceptor-mediated contractions in rat aorta.

1. In rat thoracic aortae, contractions induced by methoxamine were inhibited by chloroethylclonidine, whereas oxymetazoline-induced contractions, which were more dependent on Ca(2+)-entry, were insensitive to chloroethylclonidine. 2. Aminophylline inhibited the contractions and 45Ca(2+)-uptake induced by both methoxamine and oxymetazoline. However, oxymetazoline-induced contractions were more sensitive to inhibition by aminophylline and D600. 3. Thus, the partial selectivity of aminophylline for the chloroethylclonidine-resistant, highly dependent on extracellular Ca2+, oxymetazoline-mediated responses may be explained by a preferential inhibition of agonist-induced Ca2+ entry as compared to inhibition of other transduction pathways.

Effects of oleuropeoside in isolated guinea-pig atria.

The effects of oleuropeoside were studied on the electromechanical properties of isolated guinea-pig atria. In spontaneously beating right atria, oleuropeoside decreased the amplitude of contractions (IC50 = 1.3 +/- 0.2 x 10(-4) M), slightly decreased atrial rate, and lengthened sinus node recovery time. Oleuropeoside also inhibited peak contractile force in electrically driven left atria incubated in normal (IC50 = 1.5 +/- 0.5 x 10(-4) M) and in 27 mM K+ Tyrode solution (IC50 = 1.7 +/- 0.4 x 10(-4) M). These negative inotropic effects were not accompanied by significant changes in the characteristics of action potentials recorded in atria incubated in either normal or depolarizing solutions. These results indicate that oleuropeoside produced an electromechanical uncoupling that cannot be attributed to an inhibitory effect on Ca2+ entry through L-type channels. Therefore, the negative inotropic effect may reflect an action of oleuropeoside perhaps at an intracellular level.

Combined therapy of coarctation and coronary heart disease in an adult.

The association of aortic coarctation and ischaemic cardiopathy is rare. We present a case of such an association in a 58-year-old patient with established spinal-cord damage and describe a one-stage surgical approach for the correction of both pathologies. Coarctation is solved by means of a supracoeliac to thoracic aortic bypass, and then coronary bypass grafting is performed under CPB. In this case grafts were carried out using the left internal mammary artery (IMA) even though it was hypertrophied. There were no postoperative complications and at 10 months the patient was asymptomatic.

Effects of lisinopril on electromechanical properties and membrane currents in guinea-pig cardiac preparations.

1. The effects of the angiotensin-converting enzyme inhibitor, lisinopril, were studied in guinea-pig atria and papillary muscles and in single isolated ventricular cells. 2. In isolated right atria, lisinopril (0.001-10 microM) decreased the amplitude and rate of the spontaneous contractions. In electrically driven left atria this negative inotropic effect was accompanied by a shortening of the time to peak tension and time for total contraction. 3. Lisinopril did not modify the electrophysiological characteristics of the ventricular action potentials recorded in papillary muscles perfused with normal Tyrode solution or elicited by isoprenaline in papillary muscles perfused with 27 mM K Tyrode solution. 4. In single ventricular cells, lisinopril (10 microM) had no effect on the inward L-type Ca2+ (ICa,L), the inward rectifier (IK1) or the delayed rectifier K+ currents (IK). However, it abolished the stimulation-dependent facilitation of the L-type Ca2+ current. 6. These results indicate that the negative inotropic effect of lisinopril cannot be explained by a decrease in Ca2+ entry through L-type channels and suggest that lisinopril may possibly act at an intracellular site to reduce contractile force.

Diagnosis of X-adrenoleucodystrophy phenotypic variants.

We report the radiological and biochemical data of a familial X-adrenoleucodystrophy with an extreme phenotypic variability, in which the diagnosis of several affected members was delayed for several years. The propositus developed a progressive dementia, while two of his brothers were diagnosed of primary Addison's disease several years previously. MRI in two cases with different phenotypes revealed hyperintense diffuse white matter lesions, and the diagnosis was confirmed by increased serum levels of very long chain fatty acids. We conclude that X-adrenoleucodystrophy should be included in the differential diagnosis of adult Addison's disease even though no neurological involvement or family history is recorded, and that MRI is a useful tool for diagnosis and follow-up of neurological involvement in this disease.

Valproate-induced coma: case report and literature review.

OBJECTIVE: To report a case of hyperammonemia without hepatic dysfunction as a possible cause of lethargy, stupor, and coma in a woman after valproic acid (VPA) administration, and discuss the possible different mechanisms of ammonia elevation and coma. CASE SUMMARY: A woman diagnosed with complex partial seizures that secondarily generalize was treated with phenytoin (PHT) 250 mg/d for 18 years. Three months before admission, this dosage was increased to 300 mg/d and phenobarbital (PB) 100 mg/d was added because the seizures were incompletely controlled. The patient developed a progressive inability to walk. She was diagnosed as having PHT intoxication. VPA therapy was begun while PHT was being tapered and progressive impairment of consciousness occurred. This evolved into a coma without focal neurologic signs, and was accompanied by isolated hyperammonemia without hepatic failure. DISCUSSION: Adverse effects attributable to VPA were reviewed in the literature. Occasionally, VPA may lead to severe secondary effects such as hepatic failure and coma. In these cases increased blood concentrations of transaminases, bilirubin, and ammonia have been found. Several reports have stressed the existence of hyperammonemic coma without biochemical evidence of hepatic failure, which is what occurred in our patient. This suggests that isolated hyperammonemia and hepatic failure after VPA treatment may have a different biochemical basis. CONCLUSIONS: VPA-induced coma with hyperammonemia and without evidence of hepatic failure should be considered in patients being treated with PHT or PB when VPA is administered concomitantly. This case report shows the importance of clinical monitoring and immediate drug discontinuation when drowsiness, gastrointestinal symptoms, or lethargy occur.

Q fever mimicking herpetic encephalitis.

We describe a patient with temporal lobe encephalitis associated with primary Coxiella burnetii infection who presented with CT and MRI findings suggestive of herpes simplex encephalitis and an initial improvement during treatment with acyclovir. Q fever should be considered in the differential diagnosis of patients whose manifestations suggest herpes encephalitis.

[Efficacy of the proteic redistribution diet (PRD) in the antiparkinsonian effect of L-dopa].

The efficacy of PRD was evaluated in 21 patients with Parkinson's disease who presented motor fluctuations or a bad quality "on" time despite treatment with L-dopa or dopaminergic agonists. Eighteen of the 21 patients completed three months of treatment and only 14 (66.6%) were able to complete 12 months. The four patients who stopped treatment in this period did so because of subjective complaints despite obtaining good clinical response to PRD. Only three patients suspended the dietetic regime during first month of treatment. The 14 who completed the year of treatment showed a decrease in the "off" time (4.10 to 1.65: p < 0.01) and an improvement in motor response (14.89 to 10.39; p < 0.05) during the follow up period. Improvement appeared at the end of the second week of initiation of treatment. The diet was well tolerated with scarce complications, with only one patient presenting visual hallucinations. Other secondary effects were mild and did not require discontinuation of the diet. We conclude that proteic redistribution diet improves the "on" time in patients with Parkinson disease receiving L-dopa and delays the possibility of increasing the doses of L-dopa. The dietetic regimens with proteic redistribution seems a simple, useful and cheap approach without morbidity for the patients with Parkinson's disease showing a poor response or oscillation to L-dopa treatment.

Effects of aminophylline on contractions and 45Ca uptake in isolated rat vascular smooth muscle.

1. Aminophylline inhibited the contractions induced in rat isolated aorta by high K and noradrenaline (NA) as well as the transient contraction induced by NA in Ca-free media and suppressed the spontaneous myogenic activity in portal vein segments. 2. Aminophylline had no significant effects on 45Ca influx in resting aorta but inhibited 45Ca influx stimulated by high K or NA. 3. It is concluded that aminophylline inhibited Ca entry through voltage- and receptor-operated channels and NA-induced Ca release from intracellular stores. A part of the vascular inhibitory effects of aminophylline may be mediated by a cAMP-dependent mechanism.

Improvement of the hydrodynamic response of a ventricular assist device: a false auricle solution.

We propose a new design of the auricular cannula in ventricular assist devices (VADs), consisting of a collapsible large cannula with a capacity approximately equal to the ejection volume of the VAD. The device works as a false auricle and filters the negative pressure that occurs during diastole. We show that the use of this type of cannula improves the hydrodynamic efficiency and the stability of the working conditions. Moreover, during chronic implantation in sheep, hemolysis stays well below pathological levels. These improvements result in much better controllability of the whole VAD.
