[Contribution of arteriography to diagnosis of secondary arterio-digestive fistulas. Intestinal haemorrhage due to ilio-ileal fistula, a complication in vascular reconstruction (author's transl)].

The authors describe a case of secondary ilio-ileal fistula revealed by an intestinal haemorrhage which occurred ten years after vascular reconstruction. They stress the importance of arteriography which shows up the pseudo-aneurysm causing the fistularisation and vascular contrast material escaping into the digestive lumen. A complete review of the literature on arterio-colonic fistulas, and more particularly, ileo-ileal fistulas, shows that they usually express themselves clinically in the form of digestive haemorrhages occurring generally after vascular graft and after a variable lapse of time. The association of digestive haemorrhage and a previous vascular reconstruction should suggest the diagnosis of arterio-digestive fistula and should lead to early angiography to enable coherent surgical strategy to be put into operation.

[Neurologic tumors of the small intestine. Review of the literature apropos of 6 personal cases].

In connection with five cases of benign neurogenic intestinal tumors (3 solitary schwannomas, 1 schwannoma associated with a gastric schwannoma without von Recklinghausen's disease, 1 neurofibroma associated with von Recklinghausen's disease) and one malignant neurogenic tumor, the authors review the general and radiological features of these rare lesions. From a diagnostic standpoint, in view of the warning sign constituted by hemorrhage in the absence of von Recklinghausen's disease, angiography remains the most effective radiological technique. During von Recklinghausen's disease, lesions are usually small and undetectable by X-ray due to their intramural or subserosal situation. Even when radiological techniques allow a diagnosis to be made, a complete investigation of the entire digestive tract is advisable to determine whether the intestinal lesion detected is indeed solitary and responsible for any symptoms.

[Role of scanography in the diagnosis of parathyroid adenomas. Apropos of 27 cases].

Scanner imagine pre-operatively in 27 patients with hyperparathyroidism provided assessment of the sensitivity (92 p.cent) and diagnostic precision (92.5 p.cent) of this investigational method. The need for a strict methodology and the advantages gained by high resolution and angioscanning programmes are emphasized. After analyzing the results, a review of the published literature enables the advantages and the limits of the scanner X to be defined, and a strategy for exploration of parathyroid adenomas to be proposed.

[Congenital cysts of the liver in echography].

The authors present 150 cases of congenital biliary cystic pathologies of the liver: solitary congenital cyst (98 cases), multiple congenital cysts (40 cases), hepatic polycystosis (8 cases), and Caroli's disease (4 cases). The general characteristics of these congenital liver affections are reviewed, including discussion of their frequency, classification and potential for evolution. The problems raised by this type of pathology and the benefits of ultrasonography are emphasized.

[Critical evaluation of non-biological complementary examinations of the biliary tract before deciding on therapy (author's transl)].

A critical evaluation of non-biological complementary examinations of the biliary tract was conducted to assess their value when deciding on therapy. The published literature was reviewed, and the sensitivity and specificity of the different methods presented, emphasis being placed on the primary investigations of ultrasonography and oral and intravenous biligraphy. Secondary exploratory procedures (transparietal cholangiography, retrograde endoscopy with cholangiography and pancreatography, scintigraphy, gallbladder infusion parietography, scanography, and arteriography) are defined and their value discussed. Published data suggest that ultrasonography should be the initial investigation, this often being sufficient to enable suitable therapy to be chosen. Only approximately 5 p. cent of patients are unable to benefit from gallbladder exploration, and these require conventional radiological examinations before deciding on therapy.

[Bronchocele. A radiological study from 5 cases (author's transl)].

Bronchocele present a characteristic radiological appearance. Their significance varies according to the clinical context:--a picture of diffuse bronchopathy with mucoid impaction would suggest a medical treatment;--in the absence of any bronchopathy the X-ray should seek a under-jacent lesion which would respond to surgery.

[Triple ureter. One case. Diagnostic problem and review of the literature (author's transl)].

In the light of a case of right triple ureter in a 5 month old girl, the authors review the embryology of the ureter and describe the different types of triplication. A general review of the literature including 70 cases gives an indication of the circumstances of discovery and the contribution of radiology. The complications of complete ureteral triplication are the same but greater than those of complete duplication and are related to the ectopic position of the ureteral orifices with regard to the trigone. They are dominated by reflux and stasis in the case of high ectopia and by false incontinence (20% of cases) in low ectopia.
