[Normal lymphoid tissue and primary malignant lymphomas of the gastrointestinal tract].

Malignant lymphomas (ML) are malignant proliferations of lymph cells which have become blocked at a certain stage in their feto-embryonic development and, above all, that of their immune response. In order to understand the various forms of primary LM of the gastrointestinal tract, it is necessary to be aware of the various types of B and T cells present in the mucosae associated lymphoid tissue (MALT). After a morphologic and immunophenotypic description of the constituting elements of MALT, the different types of gastrointestinal M and T ML are reviewed. The features which distinguish them from ML of the lymph nodes and spleen are indicated. Three main types should be highlighted. Firstly, the centrocytic ML and/or MALT-type lymphoplasmocytic ML, which has low malignancy and are the most common. The cases previously described as "pseudo-lymphomas" are in fact true ML belonging to this group. Secondly, the B ML, which are highly malignant may or may not be preceded by or associated with a low-malignancy ML of the MALT-type. Thirdly, T ML, which are always unusual and highly malignant and which correspond to the malignant proliferation of intra-epithelial T lymphocytes. A malabsorption syndrome may precede or accompany this form, which may in other cases be accompanied by marked eosinophilia. The place of MALT-type ML in the Kiel classification is discussed.

[Splenic lympho-plasmocytic sarcomas discovered by exploratory laparotomy in 2 cases of cold agglutinin disease].

The 2 patients have cold agglutinin disease with lymphoid nodules in the bone marrow. A splenectomy was done because of the severity of the hemolytic anemia and because of the increasing splenomegaly. Tumoral nodules with the aspect of lymphoplasmocytic sarcoma or immunocytoma were discovered in the spleen. Those tumors can be considered either as a morphologic aspect of the cold agglutinin disease which is yet considered as a kind of chronic lymphoproliferative syndrom or as the proliferation of an other clone. In this hypothesis, the lymphoplasmocytic sarcoma habe the same signification that the Richter's syndrome in chronic lymphoid leukemia and that the sarcoma appearing in Waldenström's macroglobulinemia or in alpha heavy chain disease.

[Pseudolymphoma and lymphoproliferative syndromes in Gougerot-Sjögren syndrome (author's transl)].

Five cases of pseudolymphoma in Sjögren's syndrome were reported. Pseudolymphoma's pathological aspects described by Rothman then Talal are discussed. After recognition of the Sjögren's syndrome and a pseudolymphoma appearance, the risk of lymphomatous evolution enjoin a clinical close attention. Among our five cases, three had a peculiar evolution: one has presented and immunoblastic lymphoma (case 2). In an other case (case 3) the pathological aspect was very near to the angioimmunoblastic lymphadenopathy features. Waldenström's macroglobulinemia was cause of death of the last one (case 5).

[Histopathological findings in Hodgkin's disease (author's transl)].

The histological classification of Hodgkin's disease, as proposed by Lunkes and Butler, is incomplete and does not include all the morphological findings. The distinction between sub-varieties of the nodular sclerosing type and particular forms of the disease (interfollicular with necrosis, with xanthelasma, or epithelioid pseudo-metastatic cells) clearly demonstrates the large polymorphism of the ganglionic lesions. A study of the different types encountered in the same patient shows that there is probably a form which evolves from type 1 towards type 3 and then type 4, an another form characterized by nodular sclerosis. The distribution of the various varieties and sub-varieties were studied by the author in 442 ganglia and results were similar to those reported in other series: nodular sclerosing form: 38.2%; form without nodular sclerosis: 61.8% made up of H1: 10.6%, H3: 45.5%, H4: 5.6%.
