Ilio-femoral and femoro-femoral crossover grafting. Analysis of an 11-year experience.

Experience with 131 crossover arterial bypass grafts for lower limb revascularization has been reviewed. Ninety-nine grafts were placed as a primary procedure for unilateral iliac artery occlusive (97) or aneurysmal (2) disease. In contrast 32 grafts were placed as a secondary procedure following the failure of a previous unilateral or bilateral conventional aorto-iliac reconstruction. Fifteen patients were not suitable for a crossover procedure without the addition of a concomitant donor limb inflow reconstruction. The one month operative mortality rate was 6.1%. The five-year cumulative patency rate was 71.4% +/- 6.0 in primary procedures and 38.1% +/- 11.3 in secondary procedures (p less than 0.05). No difference in the success rate was shown when the vein graft patency curve was compared with the synthetic graft patency curve. However, the use of a small caliber vein (6 mm or less) resulted in the lowest patency rate. No other factors affecting patency were found. This report tends to support the fact that ilio-femoral or femoro-femoral crossover bypass is a safe and effective procedure for managing unilateral iliac artery occlusive disease. With proper selection of patients, a wider use of the method is justified.

[Anti-anaphylactic properties of verapamil. First experimental assessment].

1. In order to confirm the potential anti-anaphylactic properties of verapamil, the effects of this calcium antagonist were assessed on several experimental models: anaphylactic shock in mice, passive cutaneous anaphylaxis in Rats, and degranulation of Rat peritoneal mast cells. 2. At a dose of 5 mg/kg IP, half-an-hour before challenge, verapamil decreased systemic anaphylaxis-induced lethality significantly. On the reverse, in the same conditions, it was unable to inhibit passive cutaneous anaphylaxis at 0.1-0.5-1 or 10 mg/kg; it did even increase it at a dose of 5 mg/kg IP over the last 3 days. However, specific degranulation of rat peritoneal mast cells was significantly decreased after in vitro incubation with verapamil (0.25 and 1.25 micrograms/ml or after pretreatment with 5 mg/kg/day over 3 days). 3. These early results confirm the hypothesis of verapamil anti-anaphylactic properties whose consequences warrant further investigation.

[Evaluation of the potential cardiotoxicity of propranolol-lithium gluconate association (author's transl)].

The association B blocking agent-lithium salt is used for the therapy of some severe hyperthyroidism with cardiotoxicosis. Separately, both drugs depress intracardiac conduction. However by recording the potentials of the His bundle in anesthetized dogs, a potential cardiotoxicity of this association is unlikely since sinusal automaticity alone is importantly slowed. But, these results need further confirmation in man.

Dystrophy of the media with aneurysmal tendency in the abdominal aorta-iliac segments (13 cases).

The authors report 13 cases of aneurysm of the abdominal aorta and common iliac arteries similar to those described by Leriche. The main morphological abnormality was major dystrophy involving the media, giving a special pathological appearance to these structures, long considered a mere morphological anomaly. In this part of the arterial system at least, they seem to evolve towards aneurysm and rupture (thin-walled aneurysm). This course requires echotomographic supervision of cases not submitted to immediate surgery, owing to the widespread nature of the process and the fact that some lesions resemble large arteries rather than aneurysms. The classic surgical technique of aortic and iliac aneurysms should often also include special technical devices. First, unilateral by-pass operation before the resection stage, and secondly aortic suture on a prosthesis to avoid tearing the arterial wall which is affected by media-spongiosis. The ten cases operated here all had a favourable course with a follow-up period of 1 to 9 years.
