[Sequential orthotopic liver transplantation--domino transplantation].

Familial amyloid polyneuropathy (FAP) is a rare indication for liver transplantation. The excised liver of the FAP patient can be transplanted into a selected patient. In the following report, sequential liver transplantation is described where a 34-year-old female FAP patient received a cadaver donor liver. The excised native liver was transplanted to a 60-year-old male patient suffering from hepatocellular carcinoma in cirrhosis due to hepatitis C.

Concentrations in plasma and safety of 7 days of intravenous itraconazole followed by 2 weeks of oral itraconazole solution in patients in intensive care units.

Pharmacokinetics and safety of a hydroxy-beta-propyl solution of itraconazole were assessed in 16 patients in an intensive care unit. On the first 2 days, four 1-h infusions of 200 mg were given at 0, 8, 24, and 32 h. From day 3 to 7, inclusive, a single 1-h infusion of 200 mg of itraconazole was given daily. The intravenous (i.v.) treatment was directly followed by repeated administrations of an oral solution of itraconazole at a dosage of either 200 mg once daily or 200 mg twice daily (b.i.d.). During i.v. treatment, steady-state concentrations of itraconazole and hydroxy-itraconazole in plasma were reached within 48 and 96 h, respectively. At the end of i.v. treatment, mean (+/- standard deviation) itraconazole and hydroxy-itraconazole trough concentrations in plasma were 0.344 +/- 0.140 and 0.605 +/- 0.205 microg/ml, respectively. After the 2-week oral follow-up of 200 mg once daily the mean trough concentration had decreased to 0.245 microg/ml, whereas after 200 mg b.i.d. it increased to 0.369 microg/ml. Diarrhea during oral treatment appeared to be dose related and may be due to the solvent hydroxypropyl-beta-cyclodextrin. More severe laboratory abnormalities were noted during the i.v. than the oral treatment phase, probably related to more severe illness in that period of intensive care, but none proved clinically important. These results suggest that plasma itraconazole levels above 0.250 microg/ml may be achieved and maintained with the 1-week i.v. schedule followed by b.i.d. oral administration, whereas the once-daily oral follow-up seems to be a suboptimal treatment.

The course of shipped livers used as full size, reduced or split grafts.

A total of 110 transplants, 66 in adults (8 retransplants) and 30 in children (6 retransplants) were analysed according to the origin of the graft (shipped n = 39-non-shipped i.e. self procured n = 71) and the way they were transplanted (as full size grafts (FS) n = 82, reduced size grafts (RED) n = 23 or split grafts (SG) n = 5). Twenty-nine transplants were performed for urgent and 81 for elective indications. There was a statistically higher incidence of 2 or more risk factors in the donors that were selfprocured (non shipped) than in donors from shipped livers (p = 0.025). The overall 3 months graft survival was 79.5% for shipped livers versus 69% for non-shipped livers and patients survival was 89.2% versus 79.0% respectively after 3 months and 82.9% versus 74.4% after 40 months. From these results that were analysed with risk factors of the donors, cold ischaemia time and liver function tests in recipients, transplanted for acute and elective indications, it is concluded that shipping of grafts is a practical and safe procedure even if size reduction or the use of SG is intended.

Analysis of abnormal jugular bulb oxygen saturation data in patients with severe head injury.

Jugular bulb oximetry provides the first bedside, continuously available information on cerebral perfusion adequacy. An extensive analysis was made of all jugular bulb oxygen saturation (SjO2) data obtained in 50 patients suffering from severe head injury. A total of 176 periods (more than 30 minutes) with reliable, abnormal SjO2-values was observed, with 62 desaturation periods (SjO2 < 55%) and 114 high SjO2-periods (SjO2 > 80%). Jugular desaturation periods were predominantly observed in the first 2 days of monitoring and seemed the most closely correlated to lowered cerebral perfusion pressure and lowered arterial carbon dioxide tension. The high SjO2-values were more equally distributed over the first 5 days of monitoring and seemed mostly correlated to increased arterial carbon dioxide tension. Highlights of the general management of severely head injured patients is discussed, focussing attention on the importance of cerebral perfusion pressure and normoventilation.

Analysis of very early jugular bulb oximetry data after severe head injury: implications for the emergency management?

Jugular bulb oximetry provides the first bedside available information on cerebral oxygen utilization. An extensive analysis was made of all initial jugular bulb oximetry data obtained in 150 patients within the first 12 h after severe traumatic brain injury. These data revealed initial abnormal jugular bulb saturation values in 57 patients (= 38% of study population), with a predominance of jugular bulb desaturation (observed in 45 patients). This confirms earlier reports that revealed a high incidence of disturbed and inadequate cerebral perfusion in the first hours after brain injury. Jugular bulb desaturation was especially related to systemic causes (such as a lowered cerebral perfusion pressure observed in 29 patients, and a lowered arterial carbon dioxide tension in 24 patients). These findings could have important implications for the emergency management of severely head-injured patients, as outcome might possibly be improved by better attention to all systemic factors that might reduce cerebral perfusion in the early hours after traumatic insult.

Rapidly progressive pulmonary artery hypertension and end-stage liver disease.

Pulmonary hypertension is a recognized but unusual complication of liver disease. It can complicate the perioperative course of liver transplantation. Mild to moderate pulmonary hypertension is generally well tolerated during the procedure and does not appear to contribute to mortality. Since the pulmonary vascular disease may progress rapidly, it may have advanced to the point of irreversibility at the time of surgery. So, patients with known moderate pulmonary hypertension should have pulmonary arterial catheterisation immediately prior to transplantation. If pulmonary artery hypertension has become severe, then a preoperative trial of vasodilators is warranted. If this fails, the procedure should be cancelled. We present a patient with alcoholic liver cirrhosis in whom a rapidly progressive pulmonary hypertension made liver transplantation impossible.

Doppler evaluation of right ventricular outflow impedance during positive-pressure ventilation.

Positive-pressure ventilation has often been advocated to increase oxygen delivery. This ventilation mode itself, however, can impair right ventricular ejection and, thus, diminish cardiac output. In this study, alterations of right ventricular outflow impedance were evaluated after stepwise increases of positive end-expiratory pressure (PEEP). Different pulmonary artery flow characteristics were evaluated with transesophageal echocardiography in mechanically ventilated postoperative coronary artery bypass surgery patients without pulmonary hypertension. A progressive decrease of pulmonary artery flow velocity and time velocity integrals was found with increasing PEEP levels. No changes in acceleration time or pre-ejection period were observed. In order to decrease the influence of heart rate, the ratios of the different pulmonary artery flow characteristics were calculated. At end-inspiration, both the ratio of acceleration time to right ventricular ejection period and the ratio of pre-ejection period to right ventricular ejection period showed progressive increases above 10 cmH2O positive end-expiratory pressure (13.3% at the level of 15 cmH2O and 8.5% at the level of 20 cmH2O). In this study, acceleration time appears not to be of importance in ventilated patients. These data strongly support the hypothesis that intermittent squeezing of the pulmonary arterial tree during inspiration, rather than positive end-expiratory pressure, creates an increase of right ventricular outflow impedance.

[Liver and liver segment transplantation in adults and children. Experiences with the first 50 cases].

The experience with 50 liver transplants performed in 42 patients within a 16 months period is reviewed. There were 15 transplantations in 12 children and 35 in 30 adults. The series was analysed regarding the source of transplant, i.e. living related (LR) n = 2 or cadaver (CAD) n = 48, graft size i.e. full size (FS) n = 34, reduced size (RED) n = 12 and split grafts (SG) n = 4. Regarding the preservation time or operating time there was no statistically significant difference between the groups. The overall function rate was 88%, 94% for FS and 73.4% for the remaining partial grafts (RED and SG). The difference was not significant (p = 0.062). Biliary leakage occurred in one (2%) reduced graft. Hepatic artery complications (kinking, intima dissection) were encountered in two (4%) patients receiving a FS. It is concluded that the use of partial liver grafts is a safe procedure to alleviate organ shortage while preservation time and operating time are not prolonged as compared to the transplantation of full size grafts.

Influence of chest physiotherapy on arterial oxygen saturation.

Two groups of ventilated patients were compared for chest physical therapy on an ICU: respiratory insufficient patients on one side and a control population on the other were submitted either to percussion or vibration therapy, and to postural drainage. Our data show a decrease in arterial oxygen saturation after CPT and after 2h monitoring; the lateral position results in a better SaO2 in the pneumonia group while SaO2 tend to decrease in the control population. However, no significant therapeutic influence of vibration nor percussion was found. Our data suggest CPT does not result in a short term respiratory benefit. Further investigations with prolonged sessions of postural drainage are required.
