Positron emission tomographic studies of changes in cerebral blood flow and oxygen metabolism in neuroleptic malignant syndrome.

Three patients with neuroleptic malignant syndrome were studied by positron emission tomography (PET), using the steady-state technique with C15O2 and 15O2. In 2 patients the PET examination was repeated after resolution of the syndrome. In 2 patients, under treatment with bromocriptine, high blood flow and oxygen metabolism were demonstrated in the striatum, cerebellum and occipital cerebral cortex during the neuroleptic malignant phase, showing that not only the dopaminergic system is disturbed but also other neurotransmitter systems are involved. In the 3rd case the PET scan findings were not as conclusive.

Histological and histochemical study of paraoxon myopathy in the rat.

Time and dose dependency of paraoxon-induced myopathy in rats was studied in relation to esterase inhibition and clinical symptoms. High-dose poisoning resulted in a major cholinergic crisis with concomitant acetylcholinesterase inhibition in the first few hours with rapid restoration thereafter. Dose-dependent segmental muscle fiber necrosis occurred in clusters around the end-plates and was more frequent in diaphragms as compared to gastrocnemius muscles. However, in low-dose poisoned rats without major cholinergic symptoms or end-plate cholinesterase inhibition, necrotic fibers were also present. This indicates that not only end-plate cholinesterase inhibition but also neural or neuronal factors might be responsible for acetylcholine overflow and muscle fiber degeneration.

[Parkinsonian syndrome caused by carbon monoxide poisoning. Preliminary results of the treatment with bromocriptine].

Parkinsonism is the most frequent neurological complication of carbon monoxide intoxication. Its prognosis is severe and Dopa is ineffective. We treated 9 patients (mean age 60.89 +/- 6.10 years) with bromocriptine (5 à 30 mg daily). Webster's scores were improved under treatment. Our study suggests that early administration of bromocriptine might be an effective therapy of carbon monoxide induced parkinsonism.

Cytophotometric DNA determination in human meningiomas.

Cytophotometric DNA determination was performed on 23 biopsies of meningiomas. The DNA distributions were compared with the histological criteria of malignancy used by the World Health Organisation. Most of the meningiomas studied were diploid or slightly hyperdiploid. In one case, in which eight recurrent tumours were studied, regional differences in histology and DNA pattern were found. Histological anaplasia should be suspected in advance. When it is present, a considerable risk exists that the tumour will recur.

Regional distribution of potassium, calcium, and six trace elements in normal human brain.

Eight elements (i.e. K, Ca, Mn, Fe, Cu, Zn, Se, and Rb) were measured in 50 different regions of 12 normal human brains by particle-induced X-ray emission (PIXE) analysis. The dry weight concentrations of K, Fe, Cu, Zn, Se, and Rb were consistently higher for gray than for white matter areas. The K, Zn and Se concentrations for the regions of mixed composition and, to some extent, also the Rb concentrations, were intermediate between the gray and white matter values, and they tended to decrease with decreasing neuron density. The mean dry weight concentrations of K, Ca, Zn, Se, and Rb in the various brain regions were highly correlated with the mean wet-to-dry weight ratios of these regions. For Mn, Fe, and Cu, however, such a correlation was not observed, and these elements exhibited elevated levels in several structures of the basal ganglia. For K, Fe, and Se the concentrations seemed to change with age. A hierarchical cluster analysis indicated that the structures clustered into two large groups, one comprising gray and mixed matter regions, the other white and mixed matter areas. Brain structures involved in the same physiological function or morphologically similar regions often conglomerated in a single subcluster.

Positron emission tomography studies of changes in cerebral blood flow and oxygen metabolism in arteriovenous malformation of the brain.

Two patients with arteriovenous malformation were studied by position emission tomography, using the steady-state technique with 15O2. In the first patient, who had seizures, preceded by auditory hallucinations, an area of decreased cerebral blood flow and oxygen consumption was shown just behind the malformation. In the second patient, who had repeated attacks of right hemiparesis, an area of decreased blood flow and oxygen metabolism was shown at distance of the malformation. These areas of low flow and metabolism are most probably the result of a vascular steal phenomenon and the origin of the transient neurological symptoms in both patients.

Alpha-naphthyl-acetate-esterase activity in cerebrospinal fluid cells.

Mono-histiocytes and T-lymphocytes were assessed by the cytochemical alpha-naphthyl-acetate-esterase (ANAE) stain in 50 CSF samples of patients with various neurological diseases. The ANAE-activity of lymphocytes was decreased in multiple sclerosis and subacute sclerosing panencephalitis, while the activity of mono-histiocytes was increased in the group of infarctions and bacterial and viral infections of the central nervous system. In bacterial meningitis and viral meningo-encephalo-radiculitis the number of ANAE-positive lymphocytes increased after treatment and clinical improvement. ANAE staining appears to be a useful additional tool in CSF cytology in these conditions.

Steroid treatment in acute ischaemic stroke. A comparative retrospective study of 556 cases.

The files of 556 patients with a history of ischaemic stroke were reviewed. The disability and mortality rates were compared between 277 patients with a steroid treatment in the acute stage and 279 without. Comparison of the whole group showed that the steroid-treated patients had less improvement of their disability and a higher mortality rate than the non-steroid group. However, in the former more patients with a severe stroke were present. Statistical analysis of a subset of 208 patients with completed stroke in the middle cerebral artery territory showed no marked difference in the outcome and the adverse reactions between the steroid- and non-steroid-treated patients.

Influence of isaxonine on reinnervation of the rat gastrocnemius muscle.

Isaxonine is reported to have a favourable effect on nerve regeneration. The histological and histochemical changes were compared in the gastrocnemius muscles of 20 rats, in which the left sciatic nerve was frozen-crushed for 10 s at the level of the buttock. The animals received a daily dose of intraperitoneal isaxonine and ten others the solvent only. After 8 weeks small type grouping was seen in rats receiving solvent only and large type grouping in rats treated with isaxonine. After 10 weeks a checkerboard pattern of the fibre types was observed in the muscles of animals receiving solvent only but fibre type grouping was present in those treated with isaxonine. This experiment indicates that isaxonine prevents synaptic remodelling by suppression of the redundant nerve endings from additional axons, which have regenerated more slowly, in making contact with motor endplates.

Influence of isaxonine on the target phenomenon, muscle fibre size and neuromuscular junction in the tenotomized and denervated gastrocnemius muscle of the rat.

Isaxonine is reported to accelerate the rate of peripheral nerve regeneration, stimulate axonal sprouting and promote motor and sensory function recovery. The target phenomenon in muscle fibers is the morphological expression of increased stretch activity in a diseased muscle. It can be produced in the gastrocnemius muscle of the rat by tenotomy and is inhibited by simultaneous denervation. The influence of isaxonine on this experimental model was studied. Twenty Wistar rats were submitted to bilateral section of the Achilles tendon and section of the left sciatic nerve. In ten rats, treated with isaxonine, the qualitative and quantitative changes in the gastrocnemius muscle were compared with the control group over a period of 3 weeks. No statistical differences were observed in the occurrence and inhibition of the target phenomenon, the muscle fibre size and the neuromuscular junction. Isaxonine has no direct effect on a diseased muscle with an intact nerve supply, nor in the early stages after complete denervation.

The use of semiautomatic morphometry in the study of normal rat gastrocnemius muscle fibers.

Based on a fixed number of EM photomicrographs, a semiautomatic image analysis is made of normal gastrocnemius muscle fibers to obtain reference values for comparison with pathologic data. Z disc, mitochondria, tubular system, and sarcoplasmic reticulum are measured. It is stated that no definite fiber type differentiation is possible based on these quantitative data. However, measurements of Z disc and mitochondria are suggestive of such a differentiation. On the other hand, all data reach a Gaussian distribution on a logarithmic scale after a 5% correction factor is introduced.

Epsilon amino caproic acid myopathy: additional features.

In a patient with a proximal myopathy due to Epsilon Amino Caproic Acid (EACA) bedrest led to marked improvement even before withdrawal of the drug. A biopsy showed a selective necrosis of type I fibers. Plasma level of EACA was low and plasma level of lysine was normal. A 99mTechnetium-MDP showed marked uptake in the affected muscles, whereas a 67Gallium scan was negative.

Influence of stretch on the target phenomenon in the tenotomized and immobilized gastrocnemius muscle of the rat.

The influence of traction on the target phenomenon was analysed in the tenotomized gastrocnemius muscle of Wistar rats. Target fibres were constantly observed at the medial side of the heads of the non-immobilized and tenotomized gastrocnemius muscles used as control. In the muscles fixed in flexion, no target fibres could be detected, while in the muscles fixed in hyperextension some target fibres were seen. The latter were less numerous than those present in the non-immobilized muscles. This experiment proves the importance of stretch mechanisms in the pathogenesis of the target phenomenon.

Brain maturation and types of perinatal hypoxic-ischemic encephalopathy.

The degree of brain maturation and the location of hemorrhage in the germinal layer and of periventricular leukomalacia were analyzed in 85 newborn brains. The average age of the premature brains with hemorrhage was 31 weeks and of those with leukomalacia 37 weeks. Hemorrhage was more frequently found in the frontal regions, while periventricular leukomalacia predominated in the parieto-occipital regions. This study confirms that the type and the location of the hypoxic-ischemic perinatal cerebral damage are dependent on the rate of regression of the embryonic type of periventricular arterial vascularization in the premature brain.

Intracerebral osteoma: a clinicopathologic and neuropsychologic case study.

This case report deals with a 16-year-old girl, admitted for headache and Jacksonian seizures. The clinical examination showed no gross disturbances, but the neuropsychologic examination revealed difficulties in speech-linked abstract thinking. Extensive radiologic investigations revealed the presence of a frontal intracallosal bony tumor, protruding into the lateral ventricles. Histologic examination of the surgical specimen showed a large benign osteoma, without any connection with dura or skull. In view of the paucity of symptoms, a dysontogenetic etiology of the tumor seems obvious.

Granulomatous angiitis of the nervous system: a clinicopathological study of one case.

The clinical history is presented of a 69-year-old man with a disease starting with a herpes zoster infection and an acute ascending myelopathy, and ending with an intracerebral hemorrhage. The postmortem examination revealed multiple angiitis lesions, restricted to the central nervous system. In review of the 31 previously described cases there were four other patients in whom the granulomatous angiitis of the nervous system (GANS) was associated with a herpes zoster infection. The relation between both disorders is discussed.
