Gilbert's syndrome: evidence of morphological heterogeneity.

Hepatic ultrastructure was examined by electron microscopy in 25 patients with Gilbert's syndrome and the changes in the smooth endoplasmic reticulum quantified by grid technique. Thirteen patients showed gross hypertrophy of the smooth endoplasmic reticulum (SER). These were designated Gilbert's EM Positive. The remaining 12, designated Gilbert's EM Negative, did not differ significantly from normal controls. The EM Positive group showed a significantly greater percentage response to caloric restriction (P less than 0.01) and an exaggerated response to nicotinic acid stimulation when compared with the EM Negative group and normal controls. These results suggest that SER hypertrophy is not, as previously suggested, a constant feature of Gilbert's syndrome but rather a characteristic of a distinct subpopulation.

Cardiovascular anomalies in thoracopagus twins and the importance of preoperative cardiac evaluation.

Four pairs of thoracopagus twins have been described. Cardiac catheterisation was performed in all the cases. Angiocardiographic and necropsy findings suggest that the most common abnormality was some form of univentricular heart. The communication between the 2 hearts was at atrial level in 2 cases. Separation was performed in 1 of these cases but only 1 of the twins survived for 14 hours after operation. It is suggested that full cardiac catheterisation with selective angiocardiogram is essential before separation is considered. Identical heart rates were observed in each pair and there was invariably a major communication between the hearts of the twins.

Socio-economic differences in size-judgments of discs and coins by Chinese primary VI children in Hong Kong.

The present experimental data provide some cross-cultural support for Bruner and Goodman's (1947) hypothesis about perceptual accentuation. 20 Hong Kong Chinese children from a lower socio-economic level over-estimated valued coins more than 15 children from a high socio-economic level. The two Primary Class VI samples used were matched for education and age, and both spoke the Cantonese dialect. There was also some evidence for Tajfel's (1957) "interserial" and "intraserial effects" more particularly for the lower socio-economic children, although similar trends were apparent for the smaller group of higher socio-economic children.

Giant duodenal ulcer.

Twenty-five cases of benign giant duodenal ulcer have been studied. In every case a barium meal examination showed an ulcer crater with a radiographic diameter of at least 2 centimetres. Abdominal pain was the commonest symptom but less than half of the patients had had pain characteristic of chronic peptic ulcer. Haemorrhage from the ulcer occurred in a large majority of them. The radiological appearances are described, and it has been shown that the giant ulcer may be missed through being mistaken for the duodenal cap or else misdiagnosed.It appears that the death rate in this condition has been falling but it is still high. The management of patients with giant ulcers is discussed.
