Circulating immune complexes in the sera and ascites of hepatocellular carcinoma or chronic hepatitis patients.

Circulating immune complexes (CIC) in the sera or ascites of hepatocellular carcinoma (HCC), chronic hepatitis patients and normal healthy persons were measured by polyethylene glycol (PEG) and C1q solid-phase microassay (C1q-SPMA). Both the PEG and C1q-SPMA methods showed the serum CIC levels of HCC patients were significantly higher than those of chronic hepatitis patients and of normal persons. The CIC levels of chronic hepatitis patients were also significantly higher than those of normal persons as detected by PEG method but not by C1q-SPMA. The ascites from HCC patients also had CIC. But the amount of CIC in ascites was significantly lower than those of the serum from the same HCC patients. These results suggest that the increase of CIC may play some pathological role in the HCC patients.

Tamoxifen decreases progesterone and nuclear androgen receptors in the human prostate.

Androgen (AR) and progesterone (PR) receptors were measured in resected prostate tissues of patients with benign prostatic hypertrophy. One group of patients received an anti-estrogen, tamoxifen (Tm 20 mg b.i.d.) for 10 days prior to prostate resection; a second group served as controls and were untreated. Plasma levels of Tm were 200-500 pmol/ml at the time of surgery. Statistically significant decreases (P less than 0.05) were found in cytosol PR (154 fmol/mg DNA +/- 33 SE in 14 Tm-patients vs 266 +/- 40 SE in 13 untreated patients) and in nuclear AR (103 fmol/mg DNA +/- 70 SE in 18 Tm-patients vs 257 +/- 62 SE in 17 controls). Cytosol AR was not significantly different in Tm-treated patients (257 fmol/mg DNA +/- 79 SE in 15 Tm-patients vs 346 +/- 130 SE in 17 controls, P greater than 0.6). Although receptor recycling is one of several possible explanations, these decreases in progesterone and nuclear androgen receptors in Tm-treated patients suggest that estrogen has a role in the biological regulation of steroid receptors in the human prostate.

Thoracic roentgenologic manifestations in primary carcinoma of the liver.

The most common extrahepatic metastasis of primary carcinoma of the liver is pulmonary metastasis. The discrepancy in the incidence of metastasis between clinical and postmortem series is quite obvious. The rapidly fatal course of hepatoma, the limitations of the chest x-ray film in detecting the small implantation, and incomplete follow-up of patients lead to the lower incidence in clinical series. From January 1974 to December 1982, there were 470 cases of primary carcinoma of the liver at Taipei Municipal Jen-Ai Hospital. Among them, we found 439 cases of hepatocellular carcinoma and 31 cases of cholangiocellular carcinoma. We also found 50 cases (11 percent) of hepatocellular carcinoma with pulmonary metastasis and ten cases (2 percent) of hepatocellular carcinoma with bone metastasis. Seven of the ten cases of bone metastasis were intrathoracic. Additionally, we found that it was rare to have pulmonary metastasis in cholangiocellular carcinoma; it only occurred once. Hematogenous and lymphatic spread and direct invasion are the principal ways for metastasis in primary hepatoma. Multiple nodulation and pleural effusion are the main manifestations in the chest x-ray film. Additionally, we saw lymphangitic carcinomatosis and miliary lesions in some of our patients. The common characteristic of these metastases is that most of the lesions arise from or are prominent in the right lower pulmonary field. Osteolysis appears in the patients with bone metastasis. Most of the metastases form a big protruding mass of tumor.

Long-term follow-up observations of the therapeutic effect of PVNO on human silicosis.

The effect of PVNO treatment on human silicosis was observed in 7 cases over a period of 9 years lasting from 1965 to 1974. The drug was given intramuscularly and by way of inhalation, respectively. The comparison of the radiographs taken before, during and 3 years after the administration of PVNO showed that during treatment the tendency of fibrotic progression was delayed in 4 cases while 2 cases were slightly progressive, and one was markedly progressive, 3 years after discontinuation of the PVNO treatment, only one case remained stable, two cases were slightly and 4 cases markedly progressive. These observations indicate that PVNO treatment may have an effect on the delay and control of silicosis progression in a portion of patients, but after discontinuation of the treatment further progression is possible.
