[Bypass operations in reconstructive vascular surgery, using prosthetic material (author's transl)].

Age and accumulation of risk factors in patients with arterial occlusive diseases in Fontaine's Stages III and IV do not always permit direct vascular reconstruction. A further indication for prosthetic bypass procedures is given by local infections following vascular surgical interventions, severe damage to soft parts after traumata and radiotherapy, and anus praeter in the pelvic region. In the supra-aortic region carotido-subclavian, subclavio-carotid and the carotido-carotid bypasses are already routine operations. The indications for axillo-femoral, carotido-femoral, femoro-femoral, iliaco-femoral and obturator bypasses are described and discussed with reference to our own experiences.

Aortic Diseases↗

[Q-Fever and its complications (author's transl)].

Q-Fever recurs as localized epidemics in the German Federal Republic. The disease has not previously been observed in North Hesse. During a familial infection with Coxiella burneti a 15-year-old boy together with his parents and an aunt became ill with atypical pneumonia. After the pulmonary symptoms had subsided a deep left leg and pelvic venous thrombosis occurred in the boy in the fifth week after the onset of the disease. The thrombosis which is very rare in childhood could be successfully removed at operation.

[Surgical indications and results of thoracic sympathectomy].

Between the 1st of July 1971 and the 31st of December 1974, among 1,459 operations on vascular surgery, we carried out 77 thoracic sympathectomies in 46 patients. In most patients there was an oblitering angiopathie of digital type, stage II to IV, confirmed by angiography. Two patients had mixed type obstructions, two others had hyperhidrosis, one patient suffered from cleroderma, another had a thoracic outlet syndrome with digital arterial obstruction. In all cases, the transaxillary approach was that which caused the least complications and which permitted satisfactory exposure of the sympathetic nerve. The cosmetic results were good. The second and third thoracic ganglia were resected and the planes of cleavage marked with silver clips. In advanced stage III and stage IV, we noted 92 p. 100 improvements i.e. return to stages I or II. The operative mortality was nil. The most serious complication was a case of hemothorax which required later pulmonary decortication. We noted in two cases, a Horner's syndrome.

[Possibilities of surgical treatment of the postthrombotic syndrome (author's transl)].

5% of the working population of the Federal Republic of Germany are suffering from a postthrombotic syndrome. Reconstructive venous surgery is only beginning to make progress as regards technique and number of cases. Over a period of four years and among a total of 2,000 cases of vascular surgery, there were only 112 operations on the venous system. Unilateral occlusions of the iliac vein are reconstructed by Palma Operation. The results of such interventions can be improved by creating a temporary arteriovenous fistula (14 observed cases, 12 of them functionning). In cases of thrombotic occlusion of the femoral vein, the equilateral saphenous vein can be implanted into the popliteal vein, thus providing an unimpaired runoff for the pump of the calf muscles (May-Operation). In this procedure, better haemodynamic conditions for the permeability of the graft can also be created by a temporary arteriovenous fistula established between the distal saphenous vein and the posterior tibial artery behind the ankle (9 observed cases, 8 of them functionning).

Acute Disease↗

[Apoplexy and stenosis of the renal atery (author's transl)].

A 48-year old man with stenosis of the right renal artery and hypertension of the renal artery and hypertension of the renal vessels suffered an apoplectic insult under antihypertensive therapy. The cause of this insult was found to be an acute total occlusion of the right carotid artery. At the same time there were stenoses of the right vertebral, the left common carotid and the left internal carotid arteries. Thromboendarterectomy of the right internal carotid artery, subclavio-carotid bypass on the left and thromboendarterectomy of the left intenal carotid artery and aortorenal bypass on the right were performed one after the other. The blood pressure can be premanently lowered, neurological symptoms no langer appear and the patient is completely able to work again.
