Kinetics of antipyrine and sulfamethazine in patients recovered from dihydralazine-hepatitis.

Pharmacokinetic studies with sulfamethazine (500 mg) and antipyrine (15 mg/kg) were performed in 27 hypertonic patients (16 females, 11 males, 37-78 years) who had recovered from a dihydralazine-induced hepatitis, and 21 patients with essential hypertension (13 females, 8 males, 18-74 years) treated with antihypertonics excluding dihydralazine. 20 patients of the hepatitis group (74%) and 12 patients of the control group (57%) were slow acetylators. With regard to the pharmacokinetic parameters no differences were found in both slow and rapid acetylators between the sulfamethazine group and the antipyrine group.

[Bone marrow aplasia: a rare complication in virus-induced hepatitis].

Bone marrow aplasia associated with acute viral hepatitis is a rare, in the rule late complication. A prognostic relation between the latency of acute hepatitis and the development of aplastic anemia is described. We report a case of nearly symptomless hepatitis in a 25 year old women with a fatal course of aplastic anemia. First clinical symptoms were caused by aplastic anemia. The pathogenesis of aplastic anemia associated to viral hepatitis is obscure. Genetic and immunological causes are discussed. A direct viral action is possible. Because the main part of the described cases is caused by non-A, non-B hepatitis, it seems apparent that at least a non-A, non-B virus strain could have an affinity to bone marrow cells. Ensuring this, relations will be impossible up to the identification of the virus.

[Rational diagnosis of functional hyperbilirubinemia and management of patients with Gilbert-Meulengracht syndrome].

Between 1981 and 1986 we have investigated 143 outpatients with symptoms of G.M.S. Analysis of the different diagnostic procedures aimed at standardization of diagnostic measures in patients with mild unconjugated hyperbilirubinaemia without other abnormal physical findings. In these patients and in persons with only anamnestic disorder, diagnosis was ensured by oral application of nicotine acid. The principles of the specific diagnostic procedure are described. In general, diagnosis of G. M. S. is possible without histological investigation of liver tissue.

[Hepatitis B risk in dental personnel].

Dependent on the profession, medical personal is at different risk of hepatitis B infection. 505 staff members of 48 Berlin dental clinics were investigated for hepatitis B virus markers and demonstrated a significantly lower prevalence of HBsAg and its antibody than other hospital personal: anti-HBs positivity (EIA) was found in only 5.5 per cent of them (dental technicians: 1.6%, dental assistants: 3.8%, dentists: 8.0%). We conclude that in the GDR dental staff is not at higher risk of hepatitis B infection than other non-risk medical personal. Legal aspects of insurance and measures for hygienic prevention in dental practice are discussed.

[Course control and significance of circulating HBsAg-specific immune complexes in acute viral hepatitis B].

The occurrence and behaviour of circulating HBsAg-specific immune complexes were investigated in 52 patients suffering from acute viral hepatitis B. In 6 patients the illness took a chronic course. The determination was carried out by precipitation with polyethylene glycol 6,000 and the increase of HBsAg concentration after antibody splitting from the complexes by trypsin treatment. The results allow following conclusions: The early presence of immune complexes suggests a specific antibody response also against HBsAg before development of clinical illness. The timing and strength of this humoral immune reaction is sex dependent; females show anti-HBs earlier and in higher titers. The course of the concentration of circulating immune complexes depends on the immunoglobulin class of antibody and the quickness of virus elimination. The determination of circulating unspecific or HBsAg-specific immune complexes does not allow to draw any prognostic conclusion to the further course of acute hepatitis B. Only in chronificating cases, the future activity of the chronic HBV infection may be expected by the early behaviour of HBsAg immune complexes. Also patients developing chronic course of the HBV infection demonstrated a "peak" of the immune reaction 6-8 weeks after the beginning of disease, which could offer the point of time for starting an immunomodulating therapy.

[Effectiveness of combined plasma exchange treatment in fulminant viral hepatitis].

The plasma exchange is an effective measure in the treatment of acute liver failure. The authors report their own first experiences in this field in the management of fulminant viral hepatitis. Four out of 7 treated patients survived. So, the survival rate was augmented from 25 per cent in a control group of 8 patients without plasma exchange to 57.1 per cent. Other necessary therapeutic measures are high energetic nutrition, branched chained amino acids, stimulation of liver cell regeneration, correction of coagulation alterations, and intensive care. The success depends on coma stage in the beginning of treatment. Patients with signs of praecoma should be admitted to specialized centers. The number of such centers should be limited because of the necessary experiences and the high costs of combined plasma exchange treatment of this rare hepatitis complication.

[Immunoprevention of hepatitis B in children of HBsAg-positive pregnant patients].

Children of hepatitis-B-surface-antigen-positive women are, depending upon the degree of maternal infectivity, exposed to a very different risk of hepatitis B. In the GDR the rate of infectious pregnant women is low. The serious consequences of a perinatal hepatitis B, which very often leads to a chronic course and virus carriers, may widely by prevented by immunoprophylaxis. Various methods for the establishment of a risk are described. The hepatitis-B-surface-antigen screening in 28 127 pregnant woman showed 20 = 0,07% of carriers of findings, 4 out of them HBeAg-positive. From the performance of the passive immunisation in 6 babies of HbeAg-positive mothers and 2 newborn without maternal HBe- or anti-HBe-proof and from the results of the control of 17 children, who were not immunized, when there was an anti-HBe-positiveness or when there was no proof of HBe-markers, result recommendations for practice.

[Paracetamol poisoning--clinical aspects and pharmacokinetics].

The analgesic and antipyretic effect of paracetamol and phenacetin is to be traced back to the same pharmacologic active substance. When no therapeutic doses are applied, paracetamol causes severe lesions of the liver parenchyma with often fatal termination. It is reported on the paracetamol intoxication of a 38-year-old landlady, in whom as a sequel of the intoxication a severe lesion of the liver parenchyma and an acute toxic renal failure developed. The understanding of the paracetamol-conditioned lesions takes the knowledge of the course of the reaction of its biotransformation for granted. Since the pharmacokinetics underlies numerous influence factors, there is no foreseeable correlation between toxicity and doses. It seems that also in therapy paracetamol does not cause less side-effects than phenacetin. The discussed use of paracetamol instead of phenacetin is, therefore, critically to be considered.
