Neurocognitive and symptom correlates of self-reported childhood sexual abuse in schizophrenia spectrum disorders.

This study examined whether self-reported childhood sexual abuse in schizophrenia spectrum disorders is linked with severity of neurocognitive deficits. The Wisconsin Card Sorting Test, California Verbal Learning Test, and select WAIS III subtests were administered to 15 participants with schizophrenia or schizoaffective disorder who had been sexually abused and to 28 participants with no abuse history. Controlling for age and premorbid IQ, a MANCOVA indicated there were group differences (f(9, 31) = 5.53, p < .001). Subsequent ANCOVA indicated that the sexual abuse group performed more poorly on tests of working memory and information processing speed. Childhood sexual abuse is associated with more severe working memory deficits in adults with schizophrenia spectrum disorders.

Intragastric balloon in the treatment of patients with morbid obesity.

BACKGROUND: Morbid obesity is a major health problem. This study evaluated the effectiveness and safety of an intragastric balloon (IGB) for the treatment of morbid obesity. METHODS: Sixty-nine IGBs were inserted endoscopically over a 3-year period in 63 consecutive patients (59 women; median age 41 (range 24-67) years). Median weight and body mass index were 124.5 (range 89.0-177.8) kg and 46.3 (range 36.2-72.4) kg/m(2) respectively. Significant coexistent disease was present in 34 patients. Median American Society of Anesthesiologists score was 3 (range 1-4). Data were recorded following retrospective review of patient case notes. RESULTS: Mean operating time was 22 (range 15-30) min and median inpatient stay was 1 (range 1-6) day. Vomiting was the commonest early complication following 31 procedures and necessitated early removal of four IGBs. Of 58 patients with long-term follow-up, 18 suffered displacement of the IGB after at least 6 months in situ and three required a laparotomy for intestinal obstruction. Fifty patients (86 per cent) lost weight; median weight loss was 15.0 kg (P < 0.001). Median excess weight loss was 16.4 (range - 49.0 to + 4.8) and 18.7 (range - 51.5 to 12.6) per cent by 4 and 7 months after IGB insertion respectively. CONCLUSION: The IGB represents a useful device for the treatment of morbid obesity, particularly in preparation for definitive antiobesity procedures. Early IGB replacement is essential to minimize complications.

Detailed tissue expression of bcl-2, bax, bak and bcl-x in the normal human pancreas and in chronic pancreatitis, ampullary and pancreatic ductal adenocarcinomas.

BACKGROUND: The aim of this study was to evaluate expression of the bcl-2 family of apoptosis regulating proteins in normal and diseased human pancreatic tissues. METHOD: Expression of bcl-2, bax, bcl-x, bak and p53 was determined in formalin-fixed paraffin wax-embedded archival specimens of normal pancreatic tissue (n = 7), chronic pancreatitis (n = 7), pancreatic ductal adenocarcinoma (n = 23) and ampullary cancer (n = 7) by immunohistochemistry using specific antibodies. RESULTS: In normal pancreas and chronic pancreatitis tissues, bcl-2, bax and bcl-x were predominantly expressed in ductal epithelial cells while p53 was not detected. In pancreatic ductal adenocarcinoma and ampullary cancer, bcl-2 was not detected compared with expression seen in normal acini (p < 0.01), minor (p < 0.001) and major ducts (p < 0.01), bax expression was reduced with respect to minor ducts (p < 0.01) but no different from normal acini or major ducts. bak and bcl-x were more strongly expressed in malignant epithelia compared with acini and major ducts but reduced when compared with minor ducts (p < 0.01). Overexpression of p53 was identified in 11 (48%) of 23 pancreatic adenocarcinomas and 4 (57%) of 7 ampullary cancers. Differential survival of individual patients was predicted by the relative level of bcl-x expression but not bax or bak, such that strong expression of bcl-x was associated with a median postoperative survival of 171 days when compared with 912 days for diminished expression (p < 0.001) of bcl-x. CONCLUSION: Pancreatic and ampullary cancer are associated with absent bcl-2 expression. bax, bak and bcl-x expression was reduced compared with normal minor ducts whilst bak and bcl-x expression was increased when compared with major ducts. bcl-x expression correlates with survival following resection and may represent a potential prognosis marker.

Childhood sexual trauma and psychosocial functioning in adults with schizophrenia.

OBJECTIVE: This study explored the association between sexual abuse in childhood and the severity of psychosocial deficits in adults with schizophrenia. METHODS: The Quality of Life Scale, which assesses current interpersonal and work function, and the NEO personality inventory, which assesses personality dimensions relevant to social functioning, were administered to 54 individuals who had been diagnosed as having schizophrenia. Nineteen of the individuals reported having been sexually abused as children, and 35 reported no history of abuse. RESULTS: A multiple analysis of variance indicated that there were differences between the group of individuals who had a history of sexual trauma and the group that did not. Subsequent univariate analysis of variance indicated that the individuals with a history of abuse had poorer current role functioning and fewer of the basic psychological building blocks necessary for sustaining intimacy. They also demonstrated higher levels of neuroticism, which is a measure of vulnerability to emotional turmoil. The two groups did not differ in frequency of interpersonal contacts or level of extroversion. CONCLUSIONS: Sexual abuse in childhood is associated with poorer psychosocial functioning in adults with schizophrenia.

Expression profiles during honeybee caste determination.

BACKGROUND: Depending on their larval environment, female honeybees develop into either queens or workers. As in other polyphenisms, this developmental switch depends not on genomic differences between queens and workers but on the differential expression of entire suites of genes involved with larval fate. As such, this and other polyphenic systems can provide a novel tool for understanding how genomes and environmental conditions interact to produce different developmental trajectories. Here we use gene-expression profiles during honeybee caste determination to present the first genomic view of polyphenic development. RESULTS: Larvae raised as queens or workers differed greatly in their gene-expression patterns. Workers remained more faithful than queens to the expression profiles of younger, bipotential, larvae. Queens appeared to both downregulate many of the genes expressed by bipotential larvae and turn on a distinct set of caste-related genes. Queens overexpressed several metabolic enzymes, workers showed increased expression of a member of the cytochrome P450 family, hexameric storage proteins and dihydrodiol dehydrogenase, and young larvae overexpressed two putative heat-shock proteins (70 and 90 kDa), and several proteins related to RNA processing and translation. CONCLUSIONS: Large differences in gene expression between queens and workers indicate that social insect castes have faced strong directional selection pressures. Overexpression of metabolic enzymes by queen-destined larvae appears to reflect the enhanced growth rate of queens during late larval development. Many of the differently expressed genes we identified have been tied to metabolic rates and cellular responses to hormones, a result consistent with known physiological differences between queen and worker larvae.

K-ras oncogene subtype mutations are associated with survival but not expression of p53, p16(INK4A), p21(WAF-1), cyclin D1, erbB-2 and erbB-3 in resected pancreatic ductal adenocarcinoma.

Previous studies of molecular prognostic markers following resection for exocrine pancreatic cancer have produced conflicting results. Our aim was to undertake a comprehensive analysis of potentially useful molecular markers in a large, multicentre patient population and to compare these markers with standard pathological prognostic variables. Formalin-fixed, paraffin-embedded specimens of pancreatic ductal adenocarcinoma were analysed from 157 patients [100 men and 57 women with a median (range) age of 60 (33-77) years] who had undergone pancreatectomy. Immunohistochemistry was used to detect expression of p16(INK4), p53, p21(WAF1), cyclin D1, erbB-2 and erbB-3. Mutations in codons 12 and 13 of the K-ras oncogene were detected by SSCP and sequencing following DNA extraction and amplification by PCR. The median (range) survival post-resection was 12.5 (3-83) months. Abnormalities of p16(INK4), p53, p21(WAF1), cyclin D1, erbB-2 and erbB-3 expression were found in 87%, 41%, 75%, 72%, 33% and 57% of cases, respectively. There was no significant correlation between expression of any of these markers and patient survival. K-ras mutations were found in 73 (75%) of 97 cases with amplifiable DNA. The presence of K-ras mutation alone did not correlate with survival, but there were significant differences in survival according to the type of K-ras mutation (p = 0.0007). Reduced survival was found in patients with GaT, cGT and GcT K-ras mutations compared to GtT, aGT and GaC mutations. In conclusion, survival was associated with type of K-ras mutation but not expression of p16(INK4), p53, p21(WAF1), cyclin D1, erbB-2 and erbB-3.

Effects of thymol on ruminal microorganisms.

Thymol (5-methyl-2-isopropylphenol) is a phenolic compound that is used to inhibit oral bacteria. Because little is known regarding the effects of this compound on ruminal microorganisms, the objective of this study was to determine the effects of thymol on growth and lactate production by the ruminal bacteria Streptococcus bovis JB1 and Selenomonas ruminantium HD4. In addition, the effect of thymol on the in vitro fermentation of glucose by mixed ruminal microorganisms was investigated. Neither 45 nor 90 microg/ml of thymol had any significant effect on growth or lactate production by S. bovis JB1, but 180 microg/ml of thymol completely inhibited growth and lactate production. In the case of S. ruminantium HD4, 45 microg/ml of thymol had little effect on growth and lactate production; however, 90 microg/ml of thymol completely inhibited growth of S. ruminantium HD4. Thymol also decreased glucose uptake by whole cells of both bacteria. When mixed ruminal microorganisms were incubated in medium that contained glucose, 400 microg/ml of thymol increased final pH and the acetate to propionate ratio and decreased concentrations of methane, acetate, propionate, and lactate. In conclusion, thymol was a potent inhibitor of glucose fermentation by S. bovis JB1 and S. ruminantium HD4. Even though thymol treatment decreased methane and lactate concentrations and increased final pH in mixed ruminal microorganism fermentations of glucose, concentrations of acetate and propionate were also reduced.

Molecular phylogenetics and the classification of honey bee viruses.

We present the phylogenetic relationships of several picorna-like RNA viruses found in honey bees, with respect to 13 additional plant and animal positive-strand RNA viruses. Most of the honey bee viruses fall into an unnamed family of insect RNA viruses typified by the Drosophila C virus. Different bee viruses are broadly distributed within this group, suggesting either that the ability to infect honey bees has evolved multiple times, or that these viruses are generalistic in their abilities to infect insect hosts. At least one major change in gene order has occurred among the bee viruses, based on their phylogenetic affiliations. At the amino-acid level, the bee viruses differed by 15-28% at three conserved loci. Most differed by greater than 50% at the RNA level, indicating that sequence-based methods for bee virus identification must be tailored to at least three different virus clades independently.

Relative activity and referral patterns for diabetes and non-diabetes in general practice.

AIMS: To describe and compare general practice (GP) activity for patients with and without diabetes using a survey of general practices representing over 10% of the Welsh population. METHODS: The Welsh GP Morbidity Database Project (GPMDP) collected data including demographic and lifestyle information and consultation data such as diagnosis, referral and surgical procedures. These data were analysed to establish the annual period prevalence of diabetes and compare the relative number of consultations and referrals. RESULTS: A total of 4,182 diabetic patients were recorded (prevalence 1.41%) and accounted for 77,371 (4.4%) consultations. Patients with diabetes were four-times more likely to be referred to community services (relative risk (RR) 4.1, 95% CI 3.7-4.7), in particular district nursing (RR 3.8, 1.9-7.7), optician services (RR 8.9, 5.0-15.7), chiropody (RR 8.2 6.4-10.5) and dietician services (RR 21.2, 17.6-25.5). Patients with diabetes were also more likely to be followed-up in general practice (RR 6.7, 6.2-7.2) both within 1 month (RR 6.7, 6.2-7.2) and 1 month to 1 year (RR 9.7, 8.9-10.7). Emergency admissions were also more likely for patients with diabetes (RR 6.8, 6.2-7.5) as were elective admissions to general medicine (RR 5.6, 4.6-6.7), surgery (RR 1.8, 1.5-2.0) and opthalmology (4.2, 3.4-5.2). CONCLUSION: The increased utilization of health services apparent in secondary care was confirmed in primary care. Further research is required to determine levels of community activity after initial referral.

Demographically corrected norms for the California Verbal Learning Test.

The California Verbal Learning Test (CVLT) is designed to quantify components of verbal learning, retention and retrieval. The present study used multiple regression analyses to correct for demographic characteristics on CVLT performance measures. There were 906 subjects, of whom 549 were Caucasians (61%) and 357 were African Americans (39%). Age, education, ethnicity, and gender were found to be significant predictors of performance on several CVLT indices, including Total Words Recalled, Trial 1, Trial 5, List B, Short Delay Free Recall (SDFR), and Long Delay Free Recall (LDFR). Demographically corrected T-scores were calculated for a base sample of 672 subjects and cross-validated on 234 separate subjects. Tables and regression equations are offered to convert raw scores into T-scores corrected for age, gender, education, and ethnicity. Demographically corrected Recognition Discriminability cutoff scores were calculated for age and education levels. In order to provide some indices of important memory processes, we also computed indices of retrieval, Short-Delay forgetting and Long-Delay forgetting and present normative information for them.

Fatal intestinal perforation secondary to fragmentation of a Celestin tube.

BACKGROUND/AIMS: Oesophageal intubation remains one of the principal methods of palliation for an obstructing oesophageal carcinoma. We present a case which illustrates a rare but fatal complication of this procedure. METHODS: A 60-year-old female with oesophageal cancer presented with total dysphagia 9 months following insertion of a Celestin tube for palliation. Oesophagoscopy revealed a bolus obstruction which was successfully cleared. Two days later she developed generalised peritonitis and subsequently died. RESULTS: A post-mortem examination demonstrated fragmentation and displacement of the distal part of the Celestin tube resulting in perforation of the small bowel. CONCLUSION: Celestin tube disintegration is a risk associated with long-term use, and routine replacement is indicated in patients with a prolonged survival to avoid this complication.

Gastrointestinal autonomic nerve sarcoma presenting as a giant intra-abdominal cyst.

Gastrointestinal autonomic nerve sarcomas are rare tumours. A 36-year-old man presented with abdominal pain, distension and constipation. Abdominal ultrasound and CT scanning demonstrated a giant cystic intra-abdominal mass. Laparotomy confirmed a large cystic mass arising from the ileum with multiple metastases. Immunohistochemical staining was positive for vimentin, neuronal-specific enolase and PGP9.5. This is the first reported case to present as a giant intra-abdominal cyst. Specialist histopathological and immunohistochemical analysis is essential to establish the diagnosis of this rare tumour.

Effect of sugars and malate on ruminal microorganisms.

The objective of this study was to examine the effects of a commercial feed supplement that contains sugars and malate on lactate fermentation by Selenomonas ruminantium grown in batch culture. Experiments also were conducted to examine the effects of this feed supplement on the mixed ruminal microorganism fermentation of ground corn and soluble starch in the presence and absence of 5 mg/kg of monensin. When S. ruminantium strains HD4 and H18 were incubated in basal medium that contained DL-lactate, some DL-lactate was utilized by both strains after 24 h. In the presence of 1 g/L of sugars plus malate commercial feed supplement, both strains used most of the carbohydrate associated with the feed supplement between 6 and 8 h, and lactate was the main end product. In ground corn fermentations by mixed ruminal microorganisms, 2.25 and 3.25 g/L of sugars plus malate commercial feed supplement increased concentrations of acetate, propionate, and total volatile fatty acids, while 3.25 g/L increased lactate and decreased final pH and butyrate. Fermentation of soluble starch in the presence of both concentrations of sugars plus malate commercial feed supplement increased concentrations of acetate, propionate, and total volatile fatty acids and decreased the acetate:propionate ratio. In the presence of 5 mg/kg of monensin, sugars plus malate treatment increased concentrations of propionate and total volatile fatty acids in ground corn and soluble starch fermentations. Collectively, these results suggest that the sugars plus malate commercial feed supplement stimulates the ruminal fermentation.

Pseudoaneurysm of the medial inferior genicular artery following anterior cruciate ligament reconstruction.

Pseudoaneurysm is a rare complication of surgery or trauma around the knee. A 30-year-old man presented 10 days following anterior cruciate ligament repair with a 2 cm pulsatile swelling on the medial side of the knee. Angiography demonstrated a pseudoaneurysm of the medial inferior genicular artery. Surgical exploration and ligation of the feeding vessel to the aneurysm was performed and the patient made a full recovery. Vascular injury must be suspected in patients presenting with a haemarthrosis or pulsatile swelling following surgery on the knee.

Analysis of a multiple equivalent cylinder model with generalized taper.

A multiple equivalent cylinder somatic shunt cable model for passive neurones is considered in which one or more tapering equivalent cylinders emanate from a uniformly polarized soma. Each tapering equivalent cylinder approximates the loss of dendritic trunk parameter in the one or more dendritic trees that it represents, relaxing certain symmetry conditions necessary for the Rall equivalent cylinder reduction and in particular allowing terminal branches to end at different electrotonic distances from the soma. The types of taper belong to a class of six specific forms which can account for strict taper, strict flare as well as tapering and flaring combinations. As such they can account for a wide variety of taper forms that arise in practice upon collapse of a dendritic tree to an appropriate equivalent cylinder involving taper. The uniform (i.e. non-taper) and exponential taper forms are two of the six taper types.
