Left ventricular volume, mass, and function following surgical correction of chronic aortic regurgitation.

Following surgical correction of chronic aortic regurgitation, left ventricular (LV) volume falls to normal or near normal in most patients; this change generally occurs within the first week after surgery. During the ensuing six months, a significant regression of LV hypertrophy occurs, but most patients do not achieve a normal LV mass. The systolic ejection fraction may increase while the LV end-diastolic pressure almost uniformly falls to normal. Postoperative studies of LV volume mass and function should be aimed at identifying those patients which do not undergo these favorable changes so that medical therapy can be directed at this high risk group.

Blood platelet 5-hydroxytryptamine accumulation and migraine.

The active uptake of 5-hydroxytryptamine (5-HT) by platelets from migraine patients was measured and the results compared with 5-HT uptake of platelets from normal subjects. No significant differences in platelet 5-HT uptake were found between migraine patients and control subjects. A significant correlation was found between the length of time since the patients last migraine attack and Vmax (maximal rate of 5-HT transport). Patients who had a headache within five days prior to the test had significantly lower values of Vmax than patients without recent headaches. The implications and possible causes of this reduction in platelet 5-HT transport are discussed.

[The influence of hormones on headaches in women and the associated endometrial patterns (author's transl)].

Headaches appear to be a reaction to changes in either exogenous or endogenous levels. We are now investigating serum immunoglobulins in women with menstrual migraine and have found that in 22 women, 6 have low immunoglobulin A levels, all below the normal range, and 5 have high immunoglobulin M levels, above the normal range. The hereditary aspect of migraine may depend on inheriting a particular immune pattern which might cause a special sensitivity to hormone effects on blood vessels. This might account for the suppression of menstrual migraine by cortisone or large doses of progesterone. Deficiency of progesterone is unlikely to be responsible for the premenstrual syndrome as the week following menstruation is usually the time which is most often free from symptoms and at this part of the cycle there are very low levels of progesterone. The most reactive women are also the most sensitive to the side effects of drugs or hormones given to treat migraine, which makes the treatment of migraine difficult.

Pizotifen (BC 105) in migraine prophylaxis.

Pizotifen, an antaminic, was investigated in a double-blind placebo controlled trial to evaluate its efficacy in migraine prophylaxis. It was found to be and effective prophylactic in a proportion of migraine patients, reducing the frequency as well as the severity of headaches. Some patients showed weight gain. Several patients have continued treatment for up to 2 years with good control of their migraine and a reduction in the amount of medicament required for the acute attack.

The process values of university research. A new research funding system is needed for federal support of the process values of university research.

The process values of university research are important in the context of several significant social and economic trends in American life. These values are being taken into account in the design and administration of federal programs. Federal programs to support these values should be distinguished from programs to support the product values of research. Failure to distinguish between these two types of programs will lead to a further dilution of quality in programs designed to advance science as a legitimate end in itself. The scientific community can best protect its own interests by helping federal agencies and universities and colleges develop a funding system to support the process values of university research on a cooperative, regional basis.
