Alteration of brain dopamine and serotonin levels in cocaine users: a preliminary report.

OBJECTIVE: This study tested the hypothesis that cocaine depletes dopamine levels in the brain. METHOD: Levels of dopamine, serotonin, and their metabolites in the frontal cortex and striatum of nine cocaine users and nine matched control subjects who had never used cocaine were determined from postmortem autopsy material. RESULTS: The cocaine users had significantly lower dopamine levels and higher serotonin levels in the frontal cortex than the control subjects. Levels in other brain areas did not differ between the two groups. CONCLUSIONS: Alterations of dopamine and serotonin levels might contribute to symptoms of cocaine withdrawal and craving.

Fentanyl patches left on dead bodies -- potential source of drug for abusers.

We report a fatal case of fentanyl poisoning in which the decedent apparently obtained the drug from a used transdermal patch removed from a deceased nursing home patient. Fentanyl drug patches, even those previously used, contain a potentially lethal amount of this potent narcotic analgesic and provide a source of fentanyl for drug abusers. This case demonstrates the importance of proper disposal of these drug patches and the need for strict policies and guidelines in patient care settings regarding their disposal. Since fentanyl derivatives may not be detected on routine toxicologic analysis, specific assays should be utilized for the detection of fentanyl in cases of suspected drug overdose.

Intracellular concentrations of antibacterial agents and related clinical implications.

Intracellular penetration, accumulation and disposition of antibacterial agents is crucial for effective treatment of infections caused by intracellular bacteria. Intracellular concentrations and locations of both antibacterials and bacteria remain poorly understood and further research is needed to establish the importance of these concepts. For example, concepts that have been shown to be important outcome predictors when applied to concentrations of antibacterial agents in the serum include: (i) the effect of the peak antibacterial serum concentration to minimum concentration inhibitory to 90% of organisms (MIC90) for bacteria; (ii) the effect of length of time the serum antibacterial concentration remains above the MIC90 curve; or (iii) the area under the MIC90 curve, but above the serum antibacterial concentration curve. Further research should determine whether or not these concepts have important applications in an intracellular environment. Intracellular pathogens have been increasingly contributing to respiratory infections in the community. Therefore, on the basis of intracellular activity against bacteria causing respiratory infections, macrolides are favoured as the most broad spectrum class of oral antibacterial agents available for the treatment of patients with community-acquired respiratory infections.

Nonlinear theophylline pharmacokinetics. A preventable cause of iatrogenic theophylline toxic reactions.

When theophylline is used for the treatment of patients with obstructive pulmonary diseases, most clinicians attempt to maintain serum levels between 55 and 110 mumol/L because higher levels are associated with an increased risk of serious toxic effects. Nonlinear theophylline kinetics are known to occur in animals, in some pediatric patients, and at very high toxic levels in adults. However, within the usual therapeutic range of serum levels, first-order kinetics are assumed to operate, and, thus, a one-compartment model or a model-independent approach is routinely used for dose adjustments. We have recently encountered two adult patients in whom nonlinear theophylline kinetics existed within the subtherapeutic and therapeutic range of serum levels. In both cases this was not immediately recognized by the clinician, resulting in prolonged use of subtherapeutic doses of theophylline. In addition, in one case our empiric attempts to achieve therapeutic serum levels resulted in serious theophylline toxicity. We present only the data from this latter patient to be used as a case study. Based on this example and a review of the literature, we propose that to avoid such a potentially fatal complication the following steps should be taken when dealing with a patient in whom serum theophylline levels fail to rise as expected with increasing oral doses: (1) supervised administration of oral theophylline to rule out noncompliance; (2) discontinuation of further empirical increases of the oral dose of theophylline; (3) obtention of steady-state serum levels on at least two different oral doses of theophylline; and (4) calculation of the appropriate maintenance dose of theophylline for that individual patient using any of the methods cited in this report.

Accidental deaths involving derringer handguns: a report of three cases.

Accidental deaths due to discharge of dropped handguns have been previously reported. In this report, the authors review the history and design of the derringer handgun and describe three cases of accidental death due to discharge of derringer-type handguns. In two of these cases, the gun was dropped, resulting in a fatal discharge. The design of the derringer and the absence of a safety feature are cited as underlying factors. If the alleged weapon can be obtained, testing may demonstrate that discharge could occur if the gun were dropped.

The effect of heating on the functional activity of iota carrageenan.

Iota carrageenan is toxic to cells of the reticuloendothelial system. If carrageenan is dissolved at 40 degrees C and injected intraperitoneally, it will induce a significant reduction of alveolar macrophages. Heating the carrageenan preparation to 121 degrees C, however, results in a marked diminution of this in vivo cytotoxicity. This observation offers an explanation for the variability of effects previously attributed to carrageenan, and suggests that preparation methods should be standardized and carefully monitored.

Morphological characteristics of a unique intracytoplasmic neuronal inclusion body.

Unique intracytoplasmic inclusion bodies unaccompanied by a clinically apparent neurologic disease were found in the neurons of the hippocampus, subiculum, and mammillary body of a man who died suddenly, presumably of cardiac disease. The eosinophilic inclusions were round and often targetoid , with a central core that had the staining characteristics of RNA. Ultrastructurally, the inclusions were mainly composed of osmiophilic granular material. Rabies viral antigen was not detected by immunofluorescence, and heavy metals were not detected by energy dispersive spectrometry. These neuronal inclusions should be distinguished from Negri bodies.

Sexual development and behavior in black preadolescents.

As part of a longitudinal study of the sociosexual development of black preadolescents, sexual maturation and sociosexual behaviors were assessed and the relationships between these variables determined in a sample of 101 nine- to eleven-year-old middle- and low-income boys and girls. Sexual maturation was measured by Tanner's staging criteria of specific secondary sex characteristics. Involvement in heterosexual behaviors was elicited via self reports and was classified on a five-point heterosexual physical activity scale (HPA). The data corroborate other studies in demonstrating that girls were more advanced than boys in the process of sexual maturation. Considerable variation in stages of maturation for chronological age existed in both boys and girls, but was more pronounced for girls. In girls, there was no significant association between HPA and degree of biologic maturation. However, genital development in boys was significantly related to their sexual behavior. Income level was not significantly associated with the HPA score. A baseline from which to chart the progress of sociosexual behaviors in these developing preadolescents was established.

Vitreous humor magnesium in alcoholics.

Postmortem vitreous humor magnesium concentrations were determined in 27 alcoholics and in 27 nonalcoholics who died of various causes. No significant differences in magnesium values were present between these two groups. The concentrations in those dying with alcohol withdrawal syndrome or in those with postmortem findings limited to hepatic fatty change did not differ significantly from those in the nonalcoholic group or from those of the alcoholics dying of other causes. There was no correlation between vitreous humor magnesium concentrations and postmortem interval in any group. We conclude that vitreous humor magnesium determination is not helpful in the postmortem evaluation of suspected alcohol-related deaths or in determining postmortem interval.

Effect of a forced diuresis on the distribution and excretion (via urine and bile) of 195mplatinum when given as 195mplatinum cis-dichlorodiammineplatinum(II).

195mPlatinum cis-dichlorodiammineplatinum(II) (195mPt cis-DDP) was injected iv with furosemide or mannitol into male Sprague-Dawley rats. The effect of such a diuresis on the distribution, excretion, and retention of cis-DDP was measured. The mannitol-treated animals had a significant increase in the urinary excretion of 196mPt over control animals for the first five 15-minute collection times and for the 24-hour cumulative urine for the 0.01-mg/kg dose. At the 6-mg/kg dose, only the first three 15-minute and 24-hour cumulative urine collections were significant. This increased excretion, however, did not affect tissue or blood concentrations of this agent. Furosemide did not affect the excretion of 195mPt. A biliary excretion of 195mPt given as 195mPt cis-DDP is documented. The excretion is biphasic, with a rapid phase followed by a slower protracted phase. Forced diuresis, if it affords renal protection, does so during the first 90 minutes of urinary excretion.

Altered states of consciousness.

Medicine, sex, and religion are presented as related areas of human thought and behavior in which people traditionally have sought temporary release from daily living. In essence, these areas represent a search for altered states of consciousness. The harmful way is through drug addiction.Five common characteristics are cited for the three areas. Examples of their universality are traceable by their omnipresence and their appearance in most childhood games-especially those taking on sexual nuances-which are usually hidden from adults.If Eastern knowledge and control of bodily processes were geared to Western technology, mankind would benefit.
