[Allergic alveolitis or intrinsic asthma? (A case report)].

We report on a 37-year old patient with a simple viral infection who presented with acute bronchitis with bronchospasm, a productive cough and severe dyspnoea. Because of his specific occupational history--the patient is a forester and was exposed to wood dust and fungi--he was at high risk for extrinsic allergic alveolitis with mucoid impaction, so that the cause of hypoxemia and cyanosis was obvious. Repeated immunological parameters including gel-precipitations were negative: The lung function pattern (reduced vital capacity, a reduced FEV1, reduced flow values at low vital capacity and with the characteristic shape of the flow volume-curve but normal airway-resistance) suggested the diagnosis of intrinsic asthma limited to the small airways - "bronchiolar asthma" - severe small airway disease.

Characteristics of transient ischaemic episodes in patients with silent and symptomatic exercise-induced myocardial ischaemia.

To evaluate transient ischaemic episodes during daily life in patients with coronary artery disease and exercise-induced myocardial ischaemia, 38 patients underwent ambulatory ST-segment monitoring over 48 h. Sixteen patients had painless ischaemia during exercise and occasional angina, and 12 patients had symptomatic ischaemia and frequent angina during daily life. Ten patients with proven coronary artery disease but normal exercise electrocardiograms served as controls. The extent of coronary artery lesions and the prevalence of myocardial infarction were similar in all groups. ST-segment monitoring revealed 817 min and 98 episodes of ST depression in 13/16 patients of the asymptomatic group and 111 min and 21 episodes in 5/12 patients of the symptomatic group (P less than 0.03). Subjective scores for physical activity during Holter monitoring were significantly higher in the first group than in the second. The majority of ischaemic episodes in both groups was asymptomatic. No ischaemic ST changes occurred in control patients. Results indicate a higher frequency of transient ischaemic episodes related to a higher level of physical activity in patients with silent ischaemia than in patients with symptomatic exercise-induced ischaemia.

[Myocardial ischemia with normal coronary vessels. Indication of a vasospastic etiology].

In a 43-year-old woman with ECG changes of an acute anterior and posterior wall infarct, associated symptoms and biochemical findings, coronary angiography demonstrated normal coronary arteries and a normal variant in the course of the left coronary artery. Vasospasms in another vascular bed (migraine) and highly positive 201-Thallium scintigraphy during acral cold provocation speak for coronary spasm as cause of the myocardial ischaemia in this patient.

Characterization of hilar lymph node by 18F-fluoro-2-deoxyglucose positron emission tomography in healthy subjects.

One hundred and seventy-nine images were collected from healthy subjects who underwent whole-body 18F-fluoro-2-d-deosyglucose positron emission tomography (FDG-PET) for health examination. Images showing visually increased FDG uptake based on a five-point visual scale in the hilar regions were included for analyses. We evaluated the sizes, standard uptake values (SUV), and lesion-to-background (L/B) ratios of the hilar lymph nodes (HLN). Fifty of 179 (28%) subjects had visually increased FDG uptake in the hilar regions with a total of 84 HLN. By a five-point visual scale, 67 out of 84 (79.8%) HLN were grade 2, 16 out of 87 (19%) were grade 3, and 1 out of 84 (1.2%) was grade 4. The average size of the HLN was 1.55 x 1.46 cm. SUV of the HLN ranged from 1.132-2.975 with an average of 1.8 +/- 0.44. L/B ratio of the HLN ranged from 1.05-2.63 with an average of 1.52 +/- 0.39. Twenty-eight % of healthy subjects who underwent whole-body FDG-PET demonstrated visually increased FDG uptake in the HLN. However, all of the 84 HLN had a size < 2 x 2 cm, SUV < 3.0 and L/B ratios < 3.
