Enhancement of individual and community competence: the older adult as community worker.

This paper presents an evaluation of a primary prevention program designed to enhance individual and community competence in older adult community workers and in community residents with whom they worked. A total of 22 community workers and 97 community residents participated in the study; 30 residents constituted as posttest-only control group. Pre-post changes included increased knowledge of community services among all participants, as well as increased number of community information channels and increased life satisfaction for the workers. Residents, particularly black residents, became more internal, and their increased sense of personal control was related to their increased knowledge of services. Thus, the helper-therapy principle was supported for these older adult, mostly female, community workers, and their helping role had a net empowering effect.

Clinical applications of nuclear cystography.

Nuclear cystography is a sensitive test for detection of vesicoureteral reflux. The value of this sensitivity has not always been appreciated fully. A significant degree of reflux that warrants surgical repair seems readily demonstrable by conventional x-ray methods. Our experience with x-ray and nuclear cystography illustrates the frequent inadequacies of the conventional x-ray study. In 15 of 86 patients reflux of significant clinical degree was demonstrated by the nuclear study alone. This information has proved valuable in the eventual management of these patients.

Decreased polyamine content of concanavalin a stimulated lymphocytes in Down's syndrome subjects.

Increased polyamine content is associated with increased rates of cell growth. Several Down's syndrome (D.S.) tissues have been shown to have decreased growth rates. Studies were undertaken to determine if the polyamine content of stimulated D.S. lymphocytes was similar to that of stimulated normal cells. Lymphocytes were isolated and cultured in the presence of Concanavalin A for 4 or 5 days. Polyamines were than extracted and quantitated. After 4 days spermidine content for normal cells was 930.9 +/- 127 and for D.S. cells 489.2 +/- 113.1 nmoles/10(9) cells (P less than 0.025). Spermine content of normal cells was 1152.8 +/- 157.4 and for D.S. cells 533.9 +/- 82.0 (P less than 0.005). After 5 days in culture spermidine content of normal cells was 803.0 +/- 75.9 and for D.S. cells 446.2 +/- 76.5 nmoles/10(9) cells (P less than 0.005). Spermine content was 1155.7 +/- 121.9 for normal cells and 555.1 +/- 68.4 nmoles/10(9) for D.S. cells. Decreased content of polyamines in D.S.-stimulated lymphocytes is most probably due to decreased rate of polyamine synthesis. Decreased content of polyamines in response to stimulation may be a factor in decreased growth rates and altered immune function seen in D.S. patients.

High-performance liquid chromatographic determination of components of bleomycin preparations.

A fast and sensitive method was developed for the quantitative determination of at least 10 components of pharmaceutical bleomycin sulfate preparations. The method is based on the reversed-phase high-performance liquid chromatographic (HPLC) separation of the components on a muBondapak C18 column with a mobile phase having a linear gradient of 10--40% methanol in aqueous 0.005 M 1-pentanesulfonic acid at pH 4.3. With this assay, the average standard deviations for components A2 and B2 are 0.92 and 0.87, respectively, for a 7.5--22.5 x 10(-3)-mg sample. Regulatory agencies presently use the official Code of Federal Regulations (CFR) method, which is based on CM-Sephadex column chromatography. It was demonstrated that this CFR method does not separate the bleomycin A2 component from some other minor bleomycin components. After elution from the CM-Sephadex column, the "A2 component" was separated into five components by the HPLC method. Bleomycin A2 is stable under these HPLC conditions.

Quantitation of daunorubicin, doxorubicin, and their aglycones by ion-pair reversed-phase chromatography.

A fast and sensitive high-pressure liquid chromatographic method was developed for the quantitation of doxorubicin, daunorubicin, and their aglycones in pharmaceutical preparations. Because its higher pH extends column life while permitting determination of impurities, this system represents an improvement over previously published methods. It utilizes a C18 bonded silica gel column and a solvent system consisting of methanol-0.01 M monobasic ammonium phosphate aqueous solution (65:35) at pH 4.0 and 1600 psi of pressure. The accuracy of the doxorubicin and daunorubicin determinations, expressed as the coefficient of variation, is 1.65 and 1.27% respectively. The aglycones can be determined with a precision of less than 1.3%.

Cimetidine and gastric ulcer healing: a double-blind controlled trial.

The effect of cimetidine in the daily dose of 1200 mg on the healing rate of chronic gastric ulcer was assessed in a randomized double-blind trial in 48 patients. Cimetidine was found to accelerate the healing of chronic gastric ulcers in the ambulant patients, but it conferred no additional benefit on the patients in hospital. No significant side effects were observed.

Chronic gastric ulcer and stress. A comparison of an ulcer population with a control population regarding stressful events over a lifetime.

Stress was measured by the frequency of 31 major life events with consequent change and distress scores in 50 patients with chronic gastric ulcer and 50 control subjects, matched for age, sex and place of residence. The gastric ulcer population did not differ from the control population regarding the number of events experienced and the associated change and distress scores. However, when analysed according to social grade, the ulcer patients in the lower status suburbs experienced more events and more change than their controls, but the stress scores were similar. In the higher status suburbs, no difference was present between the patients and controls. Change and distress scores rose progressively with age, but there was no significant difference in the number of events experienced between the three age groups. Both men patients and their controls experienced significantly more events and higher change scores than women patients and their controls.

Preventive dentistry in private practice: a survey in Kentucky.

General practitioners in Kentucky were surveyed to establish a profile of preventive practice in the state and to identify factors influencing the profile. Both questionnaire and interview methods were used. A Kentuckian has a 55% chance of seeing a dentist who advises all his patients about prevention, and an 87% chance of seeing one who adivses some of his patients. Persons between the ages of 6 and 24 are most likely to receive preventive services. The dentist is selective about who receives a preventive-oriented examination, and he is more likely to perform the services than auxillaries. The profile shows the need for a greatly increased emphasis on preventive dentistry in undergraduate and continuing education courses and for the increased use of auxiliary personnel in the delivery of preventive services.

L-tetramisole. Enhancement of human lymphocyte response to antigen.

The anthelminthic drug L-Tetramisole enhanced in vitro lymphocyte transformation of normal young adults in the presence of PPD, Candida or measles antigen. This enhancement occurred regardless of evident pre-existing immunity to the antigen used. The increase in stimulation obtained by the drug and antigen combined, beyond that by antigen alone, varied between 1-25 and 5-99 for all three antigens in fourteen cell donors. Lymphocyte transformation was induced in seven individuals by the drug alone and suppressed in two. In levamisole dose--response tests, the optimum dose for this in vitro effect in six donors was similar for the three antigens.
