[Hip-shelf procedure in children and adolescents].

Forty-six hips treated by a shelf operation in childhood and adolescence have been reviewed. The mean follow-up was 8 years 1 month (4Y-26Y) after operation. The indications were residual dysplasia of the acetabulum, neurologic hips and sequelae of osteochondritis. All patients were operated after the age of 8.5 years except 2. In poliomyelitis and sequelae of osteochondritis the results were good. In dysplasia we had two bad results in 18 cases (1 painful hip after operation and 1 ankylosing hip) and in cerebral palsy 4 bad results in 13 cases, due to difficulty of maintaining a correct reduction for a long period. After a review of the literature to compare the results of this intervention with pelvic osteotomies we conclude that the shelf procedure is beneficial in childhood and adolescence, due to the quality of acetabular cover, to its improvement in time and to ease of intervention.

[Femoro-patellar instability in children and adolescents].

93 knees in 74 patients between 9 and 20 years of age were operated because of patello-femoral instability. In 76 cases, it was due to an intractable patello-femoral syndrome and in 17 cases one or several episodes of patellar dislocation had occurred. Treatment consisted, after failure of physical therapy, 14 times in a leavering of vastus medialis with a lateral capsular release and medial capsularrophy. In 71 cases it was associated to a medial transfer of the patellar ligament. Results were studied in 85 knees after an average follow-up of 6 years. 45 knees were completely symptomless, an excellent result. 34 knees remained with some kind of pain and 6 patellae redislocated. It is concluded that the transfer of the patellar ligament should be associated to the dynamic realignment achieved by the vastus medialis transfer for better results.

[Acute osteomyelitis of the ilium. A study of 2 cases with review of the literature].

The authors have made a review of the literature about two cases of osteomyelitis of the iliac ala. They insist on rarity of the disease and how often the right diagnosis may be delayed because of misleading clinical symptoms. Any tumefaction of the buttock associated with pelvic pain and with an infectious syndrome necessarily evoke an iliac osteomyelitis. The bone scan may be negative which need not exclude the possibility. The evolution may be the formation of intra or extra or sometimes simultaneous sub-periosteal abscesses which must be drained. This is the reason why the authors advise to verify systematically the other side of the ilium by its surgical trepanning in case of a sub-periosteal abscess.

[Value of monoclonal antibody study of lymphocyte subpopulations in newborn infants of HIV1 seropositive mothers].

The outcome of infants born to HIV1 seropositive (HIV1+) mothers is still uncertain is spite of current progress in techniques for viral detection. Using the almost elective tropism of HIV1 virus for CD4 lymphocytes and the central role of these cells in the immune regulation, we studied blood lymphocyte populations with 7 monoclonal antibodies during the first month of life in 28 newborns of HIV1 seropositive mothers. Our data are compared to an age-matched population of 35 control infants. According to our results, the coexistence of a low CD8 lymphocyte percentage together with a high CD4/CD8 ratio seems to be correlated with an unfavourable outcome.

[Open reduction by the Dunn technique in upper femoral epiphysiolysis. Report of 14 cases].

Fourteen of Dunn's operations for slipped upper femoral epiphysis were analysed in their indications and their results in comparison with others series. The open operation of the femoral head is a good method for the chronic highest displacements. The notion of an acute displacement leads us to ask for a previous bone scintigraphy in order to make sure of the integrity of the vascularisation. Thus, there was no complication of necrosis in this surgery, but unfortunately, there were two chondrolysis and one mechanical accident.

[Isolation of LAV/HTLV III from tears of patients with the ARC syndrome].

Isolation of Human Immunodeficiency Virus (HIV) was attempted from the tears of seven patients with ARC. For two of these patients the fluid from cell cultures showed significant reverse transcriptase activity. One strain was maintained on C.E.M. (T lymphocytes cell line). Indirect fixed cell immuno fluorescence and Western Blot tests have shown this strain to be a HIV. This result confirms other already published data. Although there is no evidence of transmission through tears, the presence of the virus should not be forgotten.

Epiphyseal distraction and centrally located bone bar: an experimental study in the rabbit.

The purpose of this study was to analyze the force required for an epiphysiolysis in vitro in the distal femoral growth plate of rabbits with different sized central physeal bone bars. The bars were operatively induced by drilling through the physis. An epiphyseal distraction was then performed, and the response was evaluated. Forty rabbits (67 femurs) divided into five groups were used for experimentation. Group A had not been operated on and manifested no physeal bone bridge. Groups B, C, D, and E manifested some bone bridges obtained surgically by drills 2, 2.5, 3.5, and 4.5 mm in diameter, respectively. The average force required to obtain an epiphysiolysis is not statistically significant in the different groups. However, we noted that the epiphysiolysis differed depending on the diameter of the surgically performed bridge. Group E was Salter-Harris type II fractures, whereas groups A, B, and C were type I fractures. Group D comprised both types.

Partial splenectomy in a child with human immunodeficiency virus-related immune thrombocytopenia.

Immune thrombocytopenia (IT) is a frequently occurring disease in childhood and a well known complication of HIV infection. Splenectomy is a part of the treatment strategy for severe chronic IT. However, overwhelming infections after splenectomy have limited its use, especially in young children. A 7-year-old child with maternal-fetal HIV-1 infection and related thrombocytopenia underwent splenectomy after previous treatment failed to improve her platelet count. Approximately 75% of the spleen was removed. The postoperative period was uncomplicated, and the platelet count increased significantly to greater than 500,000/mm3. Ultrasonographic examination performed 3 months later showed a stable volume of the spleen stump (40 x 40 x 20 mm) with effective "vascularization." The platelet count 12 months after surgery showed a sustained increase greater than 150,000 cell/mm3. Subtotal splenectomy may be a safe and effective alternative for patients with HIV and immune thrombocytopenia.

Prevalence of nonnucleoside reverse transcriptase inhibitor (NNRTI) resistance-associated mutations and polymorphisms in NNRTI-naïve HIV-infected patients.

BACKGROUND: The efficacy of treatment containing nonnucleoside reverse transcriptase inhibitors (NNRTIs) could be compromised in NNRTI-naïve patients already harboring a virus resistant to NNRTIs. On the contrary, hypersusceptibility to NNRTIs in patients having failed nucleoside reverse transcriptase inhibitor (NRTI)-containing regimens has been described and has been associated with improved outcome. METHOD: We assessed the prevalence of NNRTI resistance-associated mutations or polymorphisms in 146 antiretroviral-naïve patients and in 181 HIV-infected patients who were given an NNRTI-based regimen. We phenotypically evaluated the NNRTI susceptibility of 41 strains presenting with amino acid substitutions at positions involved in NNRTI resistance. RESULTS: In the 268 genotypically analyzable samples, the overall prevalence of NNRTI resistance-associated mutations was 2% (6/268 patients). The prevalence of strains with amino acid substitutions at reverse transcriptase (RT) gene positions (A98, K101, K103, V106, V108, V179) involved in NNRTI resistance was 15%. Hypersusceptibility to NNRTI was rare (2%, 1/41) in those samples. RT substitutions at positions involved in NNRTI resistance were not associated with a significantly worse virologic outcome in NNRTI-treated patients. Our understanding of small shifts in IC50 values (higher or lower) toward NNRTI is very limited. The significance of many RT mutations on NNRTI susceptibility is not clear. CONCLUSION: In contrast to resistance mutations, RT substitutions at positions involved in NNRTI resistance are frequent. They are not associated with a worse virologic outcome or with decreased phenotypic susceptibility to NNRTIs. It may be prudent not to rule out the use of NNRTIs in patients with small shifts in IC50 values or poorly understood mutations.
