Video-tape in teaching and examining clinical skills: a short case format.

The need for a compact psychiatric skills test usable in medical examinations is outlined. The parallel need to develop training procedures in clinical skills is noted and relevant research data is briefly reviewed. The authors then describe the trial run of a teaching and testing procedure, together with preliminary results of a new rating instrument. The relevance of this procedure both to examinations and skills training is critically discussed.

Life events before myocardial infarction.

One-hundred and-twenty-one men between the ages of 35 and 65 who had been admitted to a Coronary Care Unit were interviewed within a mean of 2.3 days of that admission, concerning their experience of certain specified predesignated life events in the previous three months before myocardial infarction. Ninety-one of them who were proven to have sustained a myocardial infarction were randomly matched individually with 91 men from an industrial payroll for sex, age and occupational level. Both groups matched, as groups, on marital status and household size. Significantly more patients (p less than .01) than comparison subjects reported these life events in the three weeks before infarction, whether this was experienced acutely or in an anginal setting. Most events were apparently independent of patients' or comparison subjects control; these were reported significantly more often by patients, both during the entire 12 weeks prior to illness (p less than .01) and during the three weeks immediately before infarction (p less than .02). Methodological difficulties are delineated which hinder credence as to the role of life events before illness.

Anti-HL-A7 lymphocyte-dependent antibody.

Target cell specificity of a human cytotoxic lymphocyte-dependent antiserum correlated with the presence of HL-A7 on target cells. Target cells from 10 of 11 HL-A7-positive donors were damaged in vitro by this antiserum together with lymphocytes from normal donors. Target cells from six donors lacking HL-A7 were not damaged. Cytotoxic activity was removed by adsorption of the antiserum with lymphocytes from any one of four HL-A7-positive donors. Adsorption with lymphocytes from any one of three HL-A7-negative donors failed to eliminate cytotoxic activity.

Characteristics of a cytotoxic human lymphocyte-dependent antibody.

Lymphocyte-dependent antobody (LDA) from the blood of a patient with choriocarcinoma was characterized in an allogeneic in vitro system. The antibody reacted with specific traget cells, including cells from the patient's husband. Antibody-coated traget cells were damaged by normal lymphocytes from man and other species. Neither cytotoxic, complement-dependent antibody nor sensitized cytotoxic lymphocytes were found in the patient. LDA activity was associated with IgG gamma-globulin by G-200 Sephadex filtration, electrophoretic mobility, and reactivity with anti-IgG, but there was no activity associated with IgM immunoglobulin. Sera from 14 of 16 normal donors lacked detectable LDA activity, and only low levels were found in sera from the other two donors.

Biochemical and genetic basis of tetracycline resistance in Staphylococcus aureus.

Both the genetic and biochemical basis of tetracycline resistance in a number of staphylococcal strains was investigated. The strains examined could be classified into three groups: (i) those possessing a high basal level of resistance and in which resistance could be induced to higher levels (macro-inducible); (ii) those which had a high uninduced level of resistance, but which were virtually uninducible (macro-constitutive); (iii) one derivative which had a low basal level of resistance and was also uninducible (micro-constitutive). Resistance in macro-constitutive strains was plasmid mediated and typical of organisms possessing wild-type plasmids. The macro-constitutive pattern of resistance appeared to be correlated with a chromosomal location for the resistance genes, whereas the micro-constitutive pattern was correlated with loss of a region from the wild-type plasmid. Analysis of membrane proteins by sodium dodecyl sulfate polyacrylamide gel electrophoresis suggested that a number of membrane polypeptides became unstable in staphylococci possessing high-level tetracycline resistance. In particular, the absence of a polypeptide of 22,000 daltons was always associated with high-level resistance. There was no evidence that multiple gene copies are required for expression of tetracycline resistance in Staphylococcus aureus.

Psychiatric nurse as therapist.

Under supervision five nurse-therapists have treated phobic patients as successfully as have psychiatrists and psychologists using similar psychological treatments in comparable psychiatric populations. Nurses have also had good results in other neurotic disorders. Intensive training is required. Nurse-therapists find their work rewarding, but the present Salmon gradings make no provision for their advancement should they retain their clinical function. Results suggest that the use of selected psychiatric nurses as skilled therapists can ease the current critical shortage of treatment personnel in psychiatry.
