Sperm populations in the female genital tract of the rabbit.

Spermatozoa were recovered from the uteri and oviducts of mated does between 0 and 14 h post coitum (p.c.). The proportion of spermatozoa binding IgG before and after treatment with normal rabbit serum was assessed by immunofluorescence microscopy. Most spermatozoa recovered from the oviduct and from the uterus < 3 h p.c. did not bind IgG, in contrast to those of the ejaculate which did, and those from the uterus 4--10 h p.c. which were already coated with IgG when recovered. Spermatozoa transferred at 2 h p.c. to a recipient doe did not bind IgG 4 h later although a 6 h sample of the donor's spermatozoa did so; only approximately 150 of these spermatozoa were needed for all fertilized eggs/doe, whereas 10(4)--10(5) ejaculated spermatozoa were required when similarly inseminated. We suggest that the small numbers of spermatozoa that do not coat with IgG and are first through the cervix are those which normally fertilize. Whether they are are separate population in the ejaculate is uncertain.

[Human lymphocyte receptors for the Fc ends of IgG].

Receptors for Fc IgG can be demonstrated by the binding of aggregated IgG or erythrocyte-IgG antibody complexes (EAG) onto subsets of B, T and "nul" lymphocytes. Among such cells are the effectors of antibody-dependent cell-mediated cytoxicity, and suppressor T cells. The binding of insoluble complexes induces a reversible modulation of the receptors associated with impaired proliferative T cell responses and transient inhibition of IgM receptors expression by adjacent T cells. Soluble receptors for Fc IgG bear a membrane binding site; they inhibit in vitro B cell differentiation induced by-T-dependent or T-independent polyclonal B cell activators.

[A study of the prognosis at one year after restorative tubal surgery in the treatment of sterility (author's transl)].

A cooperative study has been carried out on 301 women who were operated on for tubal sterility in 9 centres in different countries between 1969 and 1974. 19 per cent of these started an intra-uterine pregnancy and 4 per cent an extra-uterine pregnancy during the first year they were followed up. The results reaffirm the poor prognosis of genital tuberculosis, of tubal blocks found on hysterosalpingography and of adhesions discovered on laparoscopy. If the type of operation has little relationship to the results, these do change according to the treatment given around the time of operation. There were more intra-uterine pregnancies in women who had treatment systemically with corticoids and with antibiotics than in those who only had corticotherapy locally. This result has been confirmed when results have been compared taking into account the Centre where the operation was carried out and the type of lesions that were discovered on hysterosalpingography and on laparoscopy.

[Maturation and in vitro fertilisation of human oocytes recovered after spontaneous ovulation or after induction of ovulation by use of gonadotrophins (preliminary results) (author's transl)].

Maturation and/or in vitro fertilisation of human oocytes was carried out on 70 oocytes recovered from 56 young women who had normal periods and were having laparoscopy for tubal sterility. Five oocytes were recovered in the pre-ovulatory phase of spontaneous cycles by the use of ultrasound determination of follicular growth and the presence of an LH peak estimated from urine levels. 65 oocytes were obtained by puncture of follicules that had been stimulated using HMG/HCG 32-36 hours after HCG had been injected. 66 per cent of these eggs showed signs of degeneration before culture. The immature oocytes which were cultured for 42-46 hours at 36 degrees C with or without gonadotrophins underwent meoisis in 80 per cent of cases and 41 per cent expelled their first polar body. None of these oocytes could be fertilised in vitro, whereas 50 per cent of the oocytes that were recovered in metaphase II and fertilised in vitro had reached the stage of pronuclei 20 hours later. Ultrastructural and cytogenic studies will be carried out to ascertain the quality of these eggs before any attempt is made to reimplant.

The so-called unresolved Osgood-Schlatter lesion: a concept based on fifteen surgically treated lesions.

Of 118 patients with 151 knees treated for Osgood-Schlatter disease, fourteen patients (fifteen knees) had a distinct and separate ossicle at the proximal aspect of the tibial tubercle. This ossicle appeared after the child was first seen in all but three of the fifteen knees. When the ossicle failed to unite with the tubercle, the non-union was associated with local discomfort during activity and when direct pressure was applied on the tubercle. The symptoms did not respond to conservative treatment for an average of 3.8 years. Resection of the ossicle along with the adjacent bursa was followed by prompt relief of symptoms. Histological studies showed no evidence of avascularity. All ossicles were attached to the distal part of the undersurface of the ligamentum patellae and were separated from the tubercle by a bursa or scar tissue. The findings strongly support the concept that avulsion of the proximal cartilaginous part of the tibial tubercle is the cause of Osgood-Schlatter disease and they also suggest that once a separate ossicle is formed and becomes symptomatic, surgical excision is necessary to relieve the symptoms.

Structural consequences of psychic trauma: a new look at "Beyond the Pleasure Principle".

Freud's theory of repetition compulsion is complicated by his reliance on a "death instinct" which has been justly rejected by most subsequent analysts on theoretical grounds. An alternate theory based on traumatic neurosis as a model is proposed here which does away with the death instinct concept, ordering the data instead in terms of levels of organization of structure. It is argued that repetition complusion functioning is qualitatively different from wish-fulfillment (pleasure-principle) functioning. The differences are clinically detectable and are explainable theoretically in terms of differences in organization of memory, drive, and affect. Psychic trauma interferes with the satisfactory linking of drive with experience, and thus with the attainment of the organization of mentation we designate as the pleasure-unpleasure principle. Under analytic influence the progressive structuration of memory, drive, and affect results in the conversion of repetition compulsion functioning into pleasure principle functioning, a process of importance in a wide variety of psychopathological conditions. These considerations lead to a revised theory of the structure of the id, and of the development of psychic structure in general. Primary process organization develops through experience, rather than being a given. The ideal of "unstructured energy" in the id is replaced by a scheme of levels of organization of drive-representatives.
